- Caresam är ett samarbetsprojekt mellan Malmö högskola, Roskilde universitet, Kommunförbundet Skåne och Professionshöjskolen Metropol under projektledning av med Malmö Högskola. Syftet med projektet, som finansieras av Interreg IV A, är att utbyta erfarenhet och bidra till utvecklingen av framtidens äldreomsorg. I antologin publiceras bidrag som alla på ett eller sätt presenterats inom ramen för Caresams verksamheter. Föreläsare och intresserade har bjudits in att medverka. Antologin rör ett flertal teman som är aktuella på flera håll i en Öresundsregional äldreomsorg och forskningen kring denna. Här har praktiker, forskare och utbildare bidragit med sina reflektioner utifrån egna erfarenheter och forskning, men också som resultat av aktivt deltagande i Caresam. Ett viktigt resultat som kan iakttas utifrån de olika bidragen är att vi tycks kunna tala om en äldreomsorg som visar nationella skillnader vad gäller organisation och strukturer. Men framför allt kan vi skönja idémässiga likheter och inte minst bevittnar vi äldreomsorg och äldrepolitik som är dynamisk i så måtto att äldres och omsorgspersonals behov får en alltmer framskjuten plats. Om detta bevittnar de talrika lokala projekt som redovisats i boken.

- This dissertation examines how perceptions of competence affect the care of older persons, and the values and vocational conceptions of care work and care recipients that are tied to the perceptions of what skills that are needed and why. Competence is defined by three parts with relevance for the content of care work: what knowledge the care workers should have, what tasks they should perform and how they should respond to care recipients. The aim of this dissertation is to analyse how the care managers' and care workers' perceptions of competence form the direction of the care work and how that direction relates to the care recipients’ perceptions of and expectations on the care work. The care work is characterised by what is analysed as an activation of the recipients - that the recipients themselves should perform as much of the care work as possible. The activation aims at managing the decay of the aging body and is motivated by a vocational conception of the care recipients as different from others, with designations as grateful and passive. This conception is analysed as generation character based on an idea among the care managers and care workers that the society in which the recipients grew up gave them certain cognitive prerequisites that have to be managed in the practical care work. The care recipients in the study have opposing conceptions and perceptions of care work compared with the care managers and care workers. The recipients’ central objective with the care work is to prevent what has been interpreted as the disrepair of the home. The care recipients’ position in the organisation raises the problem of loyalty towards the care workers and in some sense the care managers. Their loyalty reduces their ability to complain and to influence their situation to the better. - Vad innebär det att arbeta med äldre utlandsfödda i skandinavisk äldreomsorg? Vilken betydelse för äldrevården och omsorgen får de erfarenheter som omsorgspersonal med invandrarbakgrund har?I den här boken problematiseras de tre begrepp som internationaliseringen av äldrevården och omsorgen tangerar: etnicitet, invandrarskap och ålder, och hur de förhåller sig till varandra.Författarna diskuterar vilka bilder som ges av äldre invandrare i socialpolitiska sammanhang och hur dessa påverkar de tvärkulturella möten som dagligen äger rum i äldrevård och omsorgssammanhang. Andra teman i boken är hur föreställningar om personal med invandrarbakgrund påverkar personalens möte med varandra, hur äldre invandrares vård- och omsorgsbehov bemöts och hur olika professioner hanterar invandrarskap i vården och omsorgen av äldre.

The quest for cultural sensitivity : how cultural sensitivity can be practised while mitigating othering in Swedish eldercare

- The need to provide culturally sensitive care for older adults with immigrant backgrounds has been expressed by care professionals. Meanwhile there is a lack of consensus about how this should be done, and critical perspectives in research highlight the danger of causing this group to be viewed as problem bearers. This thesis explores the possibility of using the welfare theory of health to achieve cultural sensitivity while limiting the risk of othering. It uses a goal-oriented perspective of health that could be useful in an active ageing context. Study I used a mixed-methods design and the Delphi method. Study II used a quantitative design and survey method. Study III used a qualitative design and semi-structured interviews. Study IV used a qualitative design and vignette method.Study I showed that municipal decision-makers in eldercare find it hard to agree on what cultural sensitivity entails. They prefer to consider individual preferences in less complex cases, but look for fixed cultural categories in more complex cases. Study II showed that the welfare theory of health can be used as a viable, personal, holistic approach to health among older adults, and that it can be measured with the HACT questionnaire. Study III built on the questionnaire in Study II, expanding it to include immaterial capital theory. It elucidated how immaterial resources of older adults with immigrant backgrounds, such as social capital, influence their goals related to good ageing. Study IV showed that assistant nurses could use the welfare theory of health to increase cultural sensitivity in their work. However, like the decision makers, they looked for fixed cultural categories in more complex cases.The thesis shows how the welfare theory of health can be used to conceptualize cultural sensitive care with a starting point in the personal goals of older adults with immigrant backgrounds. With the help of immaterial capital theory, the thesis can shed light on the importance of immaterial resources, such as social capital, both in forming those goals and as resources to achieve them. With this knowledge, decisions and practices aiming to provide culturally sensitive care could be based on assessments of needs and resources without relying on preconceived and fixed ideas about other cultures.

Identity, Old(er) Age and Migrancy : A Social Constructionist Lens

- ldentity research in relation to ethnicity and migration has tended to focus an younger people whilst identity research in relation to ageing and old(er) age has not focused an migrants. This inadvertent mutual neglect has led to a lack of identity research that examines the identity categories of old(er) age and migrancy together, a lacuna that this dissertation aims to redress. This dissertation departs from a social constructionist understanding of identity as situationally accomplished in the interplay between how one defines oneself (internally) and how others define one (externally). The questions raised by this perspective and addressed in this dissertation are: When (in what situations) and in relation to whom do old(er) age and migrancy (respectively) seem to become meaningful for identification? How do the identity categories of old(er) age and migrancy seem to be negotiated? The empirical material consists of in-depth interviews with 24 older migrants (13 men, 11 women) aged between 55 and 79 who have been living in Sweden for 18 to 61 years. Interviewees come from 12 different countries that vary in perceived cultural distance from Sweden. The findings suggest that identifications with old(er) age and migrancy seem to be dynamic and flexible rather than necessarily permanently meaningful, thus gaining meaning in specific situations and in relation to particular Others. External definitions furthermore do not always seem to match with internal ones. Regardless of how old(er) age and migrancy are constructed, they seem to be negotiable. This dissertation thus contributes to identity research by studying old(er) age and migrancy together and furthermore sheds light onto how the social constructionist lens allows us to see variability where stability otherwise would be presumed.
