    • DN DEBATT 15/3. Sedan industrialiseringens början har vi använt omkring fyra femtedelar av den mängd fossilt kol som får förbrännas för att vi ska klara Parisavtalet. Vi har bara en femtedel kvar och det är bråttom att kraftigt reducera utsläppen. Det har Greta Thunberg och de strejkande ungdomarna förstått. Därför stödjer vi deras krav, skriver 270 klimatforskare.

    • Så enkelt kan det vara – får automatisk ett förslag på träningsövningar via en strukturerad och systematisk utredning av individens behov för fysisk aktivitet Dagens dokumentation ska vara enhetlig och ändamålsenligt för att möjliggöra informationsutbyte som behövs för att säkerställa kvalitet och säkerhet. WHO:s Klassifikationen av funktionstillstånd, funktionshinder och hälsa (ICF) ingår bland annat i Socialstyrelsens fackspråk för vård och omsorg. ICF har ett bio-psyko-socialt perspektiv på hälsa och omfattar kroppsfunktioner och -strukturer, aktiviteter och delaktighet men även omgivningsfaktorer. Fysioterapeuterna och arbetsterapeuterna men även omsorgspersonal är några av yrkeskategorier som använder mer och mer ICF:s tankemodell och dess enhetliga begrepp och struktur i dokumentation. Individens behov för fysisk aktivitet kan till exempel omfatta kondition, rörlighet, balans, muskelstyrka, ledstabilitet och gångmönster. Träningsövningar kan delas in och systematiseras med samma struktur som till exempel vilken kroppsdel det ska tränas och vad som är syfte med träning (t.ex. rörlighet, styrka, kondition). Skulle journalsystemet skicka information till träningsprogrammet då skulle träningsprogrammet svara med ett förslag på urval av träningsövningar anpassad till individens behov. Tekniska standarder är en förutsättning för att kunna dela information och kunskap med andra system och är ett annat nyckelområde bredvid den semantiska interoperabiliteten inom ramen för Sveriges e-hälsa vision 2025. Ett exempel utifrån träningsprogrammet Mobilus Digital Rehab AB ges.

  • Noncardiac Surgery in Patients With Aortic Stenosis: A Contemporary Study on Outcomes in a Matched Sample From the Danish Health Care System

    • BackgroundPast research has identified aortic stenosis (AS) as a major risk factor for adverse outcomes in noncardiac surgery; however, more contemporary studies have questioned the grave prognosis. To further our understanding of this, the risks of a 30-day major adverse cardiovascular event (MACE) and all-cause mortality were investigated in a contemporary Danish cohort. HypothesisAS is not an independent risk factor for adverse outcomes in noncardiac surgery. MethodsAll patients with and without diagnosed AS who underwent noncardiac surgery in 2005 to 2011 were identified through nationwide administrative registers. AS patients (n=2823; mean age, 75.5years, 53% female) were matched with patients without AS (n=2823) on propensity score for AS and surgery type. ResultsIn elective surgery, MACE (ie, nonfatal myocardial infarction, ischemic stroke, or cardiovascular death) occurred in 66/1772 (3.7%) of patients with AS and 52/1772 (2.9%) of controls (P=0.19), whereas mortality occurred in 67/1772 (3.8%) AS patients and 51/1772 (2.9%) controls (P=0.13). In emergency surgery, 163/1051 (15.5%) AS patients and 120/1051 (11.4%) controls had a MACE (P=0.006), whereas 225/1051 (21.4%) vs 179/1051 (17.0%) AS patients and controls died, respectively (P=0.01). Event rates were higher for those with symptoms (defined as use of nitrates, congestive heart failure, or use of loop diuretics), compared with those without symptoms (P<0.0001). ConclusionsAS is associated with high perioperative rates of MACE and mortality, but perhaps prognosis is, in practice, not much worse for patients with AS than for matched controls. Symptomatic patients and patients undergoing emergency surgery are at considerable risks of a MACE and mortality.

  • Incidence of Ischemic Stroke in Individuals with and without Aortic Valve Stenosis : A Danish Retrospective Cohort Study

    • Background and Purpose - Aortic valve stenosis may lead to atrial and ventricular remodeling, predisposes to atrial fibrillation, and may also be an independent risk factor of ischemic stroke. However, information on stroke rates among persons with aortic valve stenosis are sparse. We aimed to determine the incidence rates and relative risks of ischemic stroke in individuals with diagnosed aortic valve stenosis compared with age- and sex-matched controls. Methods - All patients with incident aortic valve stenosis aged >18 years (n=79 310) and age- and sex-matched controls were identified using the Danish nationwide registries (1997-2017). Incidence rates per 1000 person-years (PY) and multivariable adjusted hazard ratios with 95% CIs were reported. Results - In total, 873 373 individuals (median age 77 years, 51.5% men, 9.1% with aortic valve stenosis) were included. Ischemic stroke occurred in 70 205 (8.0%) individuals during 4 880 862 PY of follow-up. Incidence rates of ischemic stroke were 13.3/1000 PY among the controls compared with 30.4/1000 PY in patients with aortic valve stenosis, corresponding to a hazard ratio of 1.31 (95% CI, 1.28-1.34). In all age-groups, the incidence rates and relative risks were significantly increased in patients with aortic valve stenosis compared with controls, but the relative risk was greater for younger individuals (eg, age group, 18-45 years: hazard ratio, 5.94 [95% CI, 4.10-8.36]). In patients with aortic valve stenosis above 65 years of age, the risk of ischemic stroke was markedly lower after aortic valve replacement (30.3 versus 19.6/1000 PY before and after valve replacement). Among people with atrial fibrillation the incidence rate of ischemic stroke was 1.5 times higher when aortic valve stenosis was present (33.0/1000 PY versus 49.9/1000 PY). Conclusions - People with aortic valve stenosis have a significantly increased risk of ischemic stroke compared with age- and sex-matched controls. Future studies are warranted to explore whether antithrombotic therapy may be beneficial in some individuals.

    • In this paper, we discuss the challenges in modelling and simulating infinitesimal and large deformations of cellulose fiber networks, mainly in the context of the prediction of quality controls for paper.Understanding the influence and sensitivity of macroscopic production parameters like grammage and thickness of paperboard and understanding the influence of the fiber suspension on the quality of paper is important for the development of better papers and for preserving raw materials and energy.The new simulation framework consists of the virtual stochastic paper structure generator PaperGeo, that was integrated in the GeoDict 1 software suite, and the finite element solver FeelMath (Finite Elements for Elastic Materials and Homogenization) for solving the equations of elasticity. The fibers and the contacts are modelled by using geometrically exact beams of Simo-type [1].The microstructural model and the fiber network model are validated against standard measurements of existing papers in the following way: At first we perform tensile and bending tests to measure the macroscopic stress-strain relations. In the next step we apply a representative macroscopic stress or strain onto the boundaries of realizations of the stochastic fiber network model and compute by homogenization the effective (stiffness) coefficients. Finally we compare the numerical results with the measurements.This procedure can also be used for an identification of elastic parameters on the microscale and to study the sensitivity of the effective (macroscopic) stiffness with regard to the parameters of the microstructure

    • This is the first report of pure rotational coherent anti-Stokes Raman spectroscopy (CARS) measurementsin an internal combustion (IC) engine. Single-shot, dual-broadband rotational CARS (DB-RCARS) spectra were recorded both prior to ignition and in the postcombustion gases. From these spectra, both temperature and relative oxygen concentrations were evaluated. The pressure was registered simultaneously with the CARS measurements in the spark-ignition engine burning natural gas and air. Prior to ignition, normally at temperatures below 1000 K and pressures below 2 MPa, a rotational CARS spectrum is very temperature sensitive, and the technique can be used for temperature measurements with high accuracy. Evaluated temperatures show a negligible dependence on uncertainties in parameters such as the nonresonant susceptibility of the gas and the slit width. Moreover, no collisional narrowing of the lines has to be taken into account. The relative standard deviation of evaluated temperatures and of relative oxygen concentrations from single-shot measurements were as low as 1, and 1.4-1.9%, respectively. In the postcombustion gases at temperatures above 2000 K and pressures above 1.5 MPa, the nonresonant CARS background gave a large contribution to the total spectrum. In this temperature and pressure range, the evaluated values of temperature and nonresonant susceptibility are not independent, and the nonresonant susceptibility had to be fixed at a precalculated value to get a reliable temperature evaluation. The avantages and disadvantages of rotational CARS in comparison with vibrational CARS for IC engine measurements are discussed.

    • Coherent anti-Stokes Raman scattering of pure rotational transitions, rotational CARS, is demonstrated as an efficient method for temperature determination in sooting flames. The dual broadband CARS approach was used to measure temperature profiles in premixed, sooting ethylene flames at atmospheric pressure by probing the nitrogen gas. The recorded spectra were of equally high quality in non-sooting and sooting flames with volume fractions of soot of up to 7 x 10 7 cm3 soot/cm3The advantages of rotational CARS in comparison with several other techniques for the measurement of temperatures in sooting flames, and the general applicability of the technique to different combustion conditions, are discussed. Potential limitations in the application of rotational CARS to sooting flames that are more heavily sooting than the ones investigated in this study, are outlined.
