  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

  • Surgical site infection after gastrointestinal surgery in children: an international, multicentre, prospective cohort study

    • Surgical site infection (SSI) is one of the most common healthcare-associated infections (HAIs). However, there is a lack of data available about SSI in children worldwide, especially from low-income and middle-income countries. This study aimed to estimate the incidence of SSI in children and associations between SSI and morbidity across human development settings.MethodsA multicentre, international, prospective, validated cohort study of children aged under 16 years undergoing clean-contaminated, contaminated or dirty gastrointestinal surgery. Any hospital in the world providing paediatric surgery was eligible to contribute data between January and July 2016. The primary outcome was the incidence of SSI by 30 days. Relationships between explanatory variables and SSI were examined using multilevel logistic regression. Countries were stratified into high development, middle development and low development groups using the United Nations Human Development Index (HDI).ResultsOf 1159 children across 181 hospitals in 51 countries, 523 (45·1%) children were from high HDI, 397 (34·2%) from middle HDI and 239 (20·6%) from low HDI countries. The 30-day SSI rate was 6.3% (33/523) in high HDI, 12·8% (51/397) in middle HDI and 24·7% (59/239) in low HDI countries. SSI was associated with higher incidence of 30-day mortality, intervention, organ-space infection and other HAIs, with the highest rates seen in low HDI countries. Median length of stay in patients who had an SSI was longer (7.0 days), compared with 3.0 days in patients who did not have an SSI. Use of laparoscopy was associated with significantly lower SSI rates, even after accounting for HDI.ConclusionThe odds of SSI in children is nearly four times greater in low HDI compared with high HDI countries. Policies to reduce SSI should be prioritised as part of the wider global agenda.

  • Quality of life among health care workers in Arab countries 2 years after COVID-19 pandemic

    • Background: Assessment of the quality of life (QoL) among healthcare workers (HCWs) is vital for better healthcare and is an essential indicator for competent health service delivery. Since the coronavirus disease 2019 (COVID-19) pandemic strike, the frontline position of HCWs subjected them to tremendous mental and psychological burden with a high risk of virus acquisition. Aim: This study evaluated the QoL and its influencing factors among HCWs residing in the Arab countries. Methods: This was a cross-sectional study using a self-administered online questionnaire based on the World Health Organization QoL-BREF instrument with additional questions related to COVID-19. The study was conducted in three different languages (Arabic, English, and French) across 19 Arab countries between February 22 and March 24, 2022. Results: A total of 3,170 HCWs were included in the survey. The majority were females (75.3%), aged 18–40 years (76.4%), urban residents (90.4%), married (54.5%), and were living in middle-income countries (72.0%). The mean scores of general health and general QoL were 3.7 ± 1.0 and 3.7 ± 0.9, respectively. Those who attained average physical, psychological, social, and environmental QoL were 40.8, 15.4, 26.2, and 22.3%, respectively. The income per capita and country income affected the mean scores of all QoL domains. Previous COVID-19 infection, having relatives who died of COVID-19, and being vaccinated against COVID-19 significantly affected the mean scores of different domains. Conclusion: A large proportion of the Arab HCWs evaluated in this study had an overall poor QoL. More attention should be directed to this vulnerable group to ensure their productivity and service provision.

    • In alternate partial root-zone irrigation (APRI) a significant amount of irrigation water can be saved without considerable yield reduction. In this paper, Hydrus-2D/3D was used to investigate the impact of geometric design of alternate partial root-zone subsurface drip irrigation (APRSDI) with brackish water for growing tomato on soil moisture and salinity distribution. Three inter-plant emitter distances (IPED; 20, 30, and 40 cm), two emitter depths (10 and 20 cm), and three irrigation water salinity levels (0, 1, and 2 dS m-1) were used to implement the proposed simulation scenarios in loamy sand soil during a 40-day simulation period. The simulation results showed that higher soil moisture content was found beneath the plant trunk in case of 20 cm (short IPED) and near the domain border in case of 30 and 40 cm IPED. Short IPED guarantees more water in the maximum root density zone. A deeper wetting front occurred for deep emitter depth, while the wetting front reached the soil surface for shallow emitter depth. Salinity results revealed that as irrigation water salinity increased, the salinity in the top soil increased. In addition, the salinity at the soil surface increased as IPED and emitter depth increased. Higher root water uptake rates were recorded in the case of 20 cm IPED while the emitter depth did not show any considerable effect on root water uptake rates. Moreover, the applied irrigation water was fully consumed by the plant in case of short IPED. Emitter depth and salinity of irrigation water had negligible effect on amount of irrigation water extracted by plant roots and percolated amount below the bottom boundary of the flow domain. Overall, short IPED is recommended in APRSDI with or without brackish irrigation water regardless of the emitter depth.

    • In this study, an assessment for a proposed irrigation system in the El-Salam Canal cultivated land, Egypt, was conducted. A numerical model (HYDRUS-2D/3D) was applied to investigate the effect of irrigation amount, frequency, and emitter depth on the wetted soil volume, soil salinity levels, and deep percolation under subsurface trickle irrigation (SDI) of tomato growing with brackish irrigation water in three different soil types. The simulations indicated that lower irrigation frequency increased the wetted soil volume without significant increase in water percolate below the plant roots. Deep percolation decreased as the amount of irrigation water and emitter depth decreased. With the same amount of irrigation water, the volume of leached soil was larger at lower irrigation frequency. The salinity of irrigation water under SDI with shallow emitter depth did not show any significant effect on increasing the soil salinity above tomato crop salt tolerance. Based on the results, it appears that the use of SDI with brackish irrigation water is an effective method for growing tomato crop in El-Salam Canal cultivated land especially with shallow emitter depth.

  • Genetic Variations among Different Variants of G1-like Avian Influenza H9N2 Viruses and Their Pathogenicity in Chickens

    • Since it was first discovered, the low pathogenic avian influenza (LPAI) H9N2 subtype has established linages infecting the poultry population globally and has become one of the most prevalent influenza subtypes in domestic poultry. Several different variants and genotypes of LPAI H9N2 viruses have been reported in Egypt, but little is known about their pathogenicity and how they have evolved. In this study, four different Egyptian LPAI H9N2 viruses were genetically and antigenically characterized and compared to representative H9N2 viruses from G1 lineage. Furthermore, the pathogenicity of three genetically distinct Egyptian LPAI H9N2 viruses was assessed by experimental infection in chickens. Whole-genome sequencing revealed that the H9N2 virus of the Egy-2 G1-B lineage (pigeon-like) has become the dominant circulating H9N2 genotype in Egypt since 2016. Considerable variation in virus shedding at day 7 post-infections was detected in infected chickens, but no significant difference in pathogenicity was found between the infected groups. The rapid spread and emergence of new genotypes of the influenza viruses pinpoint the importance of continuous surveillance for the detection of novel reassortant viruses, as well as monitoring the viral evolution.

    • We have investigated the nonlinear optical properties of indium tin oxide (ITO) thin film coated on soda-lime glass substrate using the z-scan technique. With 100 fs laser pulses, the nonlinear absorption coefficient and nonlinear refractive index are measured at different excitation wavelengths and at different incident intensities. The nonlinear optical absorption shows a competition between saturable absorption and reverse saturable absorption at the different excitation wavelengths and incident intensities. A transition from saturable to reverse saturable absorption was observed with increasing excitation wavelengths. The measured nonlinear refractive index was found to be dependent on wavelength with a maximum value of 5.34 x 10(-12) cm(2)/W at wavelength of 900 nm.
