Effects of oxygen on post-surgical infections during an individualised perioperative open-lung ventilatory strategy : a randomised controlled trial

- Background: We aimed to examine whether using a high fraction of inspired oxygen (FIO2) in the context of an individualised intra- and postoperative open-lung ventilation approach could decrease surgical site infection (SSI) in patients scheduled for abdominal surgery. Methods: We performed a multicentre, randomised controlled clinical trial in a network of 21 university hospitals from June 6, 2017 to July 19, 2018. Patients undergoing abdominal surgery were randomly assigned to receive a high (0.80) or conventional (0.3) FIO2 during the intraoperative period and during the first 3 postoperative hours. All patients were mechanically ventilated with an open-lung strategy, which included recruitment manoeuvres and individualised positive end-expiratory pressure for the best respiratory-system compliance, and individualised continuous postoperative airway pressure for adequate peripheral oxyhaemoglobin saturation. The primary outcome was the prevalence of SSI within the first 7 postoperative days. The secondary outcomes were composites of systemic complications, length of intensive care and hospital stay, and 6-month mortality. Results: We enrolled 740 subjects: 371 in the high FIO2 group and 369 in the low FIO2 group. Data from 717 subjects were available for final analysis. The rate of SSI during the first postoperative week did not differ between high (8.9%) and low (9.4%) FIO2 groups (relative risk [RR]: 0.94; 95% confidence interval [CI]: 0.59-1.50; P=0.90]). Secondary outcomes, such as atelectasis (7.7% vs 9.8%; RR: 0.77; 95% CI: 0.48-1.25; P=0.38) and myocardial ischaemia (0.6% [n=2] vs 0% [n=0]; P=0.47) did not differ between groups. Conclusions: An oxygenation strategy using high FIO2 compared with conventional FIO2 did not reduce postoperative SSIs in abdominal surgery. No differences in secondary outcomes or adverse events were found.

- A high intake of dietary antioxidant compounds has been hypothesized to be an appropriate strategy to reduce gastric cancer (GC) development. We investigated the effect of dietary total antioxidant capacity (TAC) in relation to GC in the European Prospective Investigation into Cancer (EPIC) study including 23 centers in 10 European countries. A total of 521,457 subjects (153,447 men) aged mostly 3570 years old, were recruited largely between 1992 and 1998. Ferric reducing antioxidant potential (FRAP) and total radical-trapping antioxidant parameter (TRAP), measuring reducing and chain-breaking antioxidant capacity were used to measure dietary TAC from plant foods. Dietary antioxidant intake is associated with a reduction in the risk of GC for both FRAP (adjusted HR 0.66; 95%CI (0.460.95) and TRAP (adjusted HR 0.61; 95%CI (0.430.87) (highest vs. lowest quintile). The association was observed for both cardia and noncardia cancers. A clear effect was observed in smokers with a significant reduction in GC risk for the fifth quintile of intake for both assays (highest vs. lowest quintile: adjusted HR 0.41; 95%CI (0.220.76) p for trend <0.001 for FRAP; adjusted HR 0.52; 95%CI (0.280.97) p for trend <0.001 for TRAP) but not in nonsmokers. In former smokers, the association with FRAP intake was statistically significant (highest vs. lowest quintile: adjusted HR 0.4; 95%CI (0.210.75) p < 0.05); no association was observed for TRAP. Dietary antioxidant capacity intake from different sources of plant foods is associated with a reduction in the risk of GC.

Second international consensus report on gaps and opportunities for the clinical translation of precision diabetes medicine

- Precision medicine is part of the logical evolution of contemporary evidence-based medicine that seeks to reduce errors and optimize outcomes when making medical decisions and health recommendations. Diabetes affects hundreds of millions of people worldwide, many of whom will develop life-threatening complications and die prematurely. Precision medicine can potentially address this enormous problem by accounting for heterogeneity in the etiology, clinical presentation and pathogenesis of common forms of diabetes and risks of complications. This second international consensus report on precision diabetes medicine summarizes the findings from a systematic evidence review across the key pillars of precision medicine (prevention, diagnosis, treatment, prognosis) in four recognized forms of diabetes (monogenic, gestational, type 1, type 2). These reviews address key questions about the translation of precision medicine research into practice. Although not complete, owing to the vast literature on this topic, they revealed opportunities for the immediate or near-term clinical implementation of precision diabetes medicine; furthermore, we expose important gaps in knowledge, focusing on the need to obtain new clinically relevant evidence. Gaps include the need for common standards for clinical readiness, including consideration of cost-effectiveness, health equity, predictive accuracy, liability and accessibility. Key milestones are outlined for the broad clinical implementation of precision diabetes medicine.

- Det här är en handbok som skrivits för att användas praktiskt i kampen om hyresrättens framtid. Boken har växt fram ur gemensamma erfarenheter av att ta strid mot renoveringar med höga hyreshöjningar som följd. Det har gett oss både kunskaper om lokal organisering och en övertygelse om vikten av att hyresgäster ska ha rätt till att påverka framtiden i de bostadsområden de bor i. De senaste åren har vi kunnat se hur hyreslägenheter renoveras med omfattande hyreshöjningar som följd. Konsekvenserna har blivit att många hyresgäster har tvingats flytta, ibland från bostadsområden de bott i under lång tid, då de inte längre har råd att betala hyran. Om inte hyresgäster går samman och kräver sina rättigheter kommer inget förändras och flera tusentals människor tvingas flytta. Den här handboken ska användas som ett praktiskt verktyg för lokal organisering. Vi som har gjort den här boken är engagerade i olika rörelser, några av oss är också forskare med inriktning mot bostad- spolitik och stadsutveckling. Boken har tillkommit genom en kollektiv process, där många varit inblandade för att bidra med sina erfarenheter och perspektiv. En lista med alla medverkande inklusive de rörelser som står bakom den här boken finns lägst bak.

Evidence for production of single top quarks and first direct measurement of vertical bar V-tb vertical bar

- The D0 Collaboration presents first evidence for the production of single top quarks at the Fermilab Tevatron < p(p)over bar > collider. Using a 0.9 fb(-1) dataset, we apply a multivariate analysis to separate signal from background and measure sigma(< p(p)over bar >-> tb+X,tqb+X)=4.9 +/- 1.4 pb. The probability to measure a cross section at this value or higher in the absence of a signal is 0.035%, corresponding to a 3.4 standard deviation significance. We use the cross section measurement to directly determine the Cabibbo-Kobayashi-Maskawa matrix element that describes the Wtb coupling and find 0.68 <= 1 at 95% C.L. within the standard model.

- We present the first experimental discrimination between the 2e/3 and 4e/3 top quark electric charge scenarios, using top quark pairs (t (t) over bar) produced in p (p) over bar collisions at root s =1.96 TeV by the Fermilab Tevatron Collider. We use 370 pb(-1) of data collected by the D0 experiment and select events with at least one high transverse momentum electron or muon, high transverse energy imbalance, and four or more jets. We discriminate between b- and (b) over bar -quark jets by using the charge and momenta of tracks within the jet cones. The data are consistent with the expected electric charge, |q|=2e/3. We exclude, at the 92% C.L., that the sample is solely due to the production of exotic quark pairs Q (Q) over bar with |q|=4e/3. We place an upper limit on the fraction of Q (Q) over bar pairs rho < 0.80 at the 90% C.L.

Lifetime difference and CP-violating phase in the B-s(0) system

- From an analysis of the decay B-s(0)-> J/psi phi, we obtain the width difference between the light and heavy mass eigenstates Delta Gamma equivalent to(Gamma(L)-Gamma(H))=0.17 +/- 0.09(stat)+/- 0.02(syst) ps(-1) and the CP-violating phase phi(s)=-0.79 +/- 0.56(stat)(-0.01)(+0.14)(syst). Under the hypothesis of no CP violation (phi(s)equivalent to 0), we obtain 1/Gamma=tau/(B-s(0))=1.52 +/- 0.08(stat)(-0.03)(+0.01)(syst) ps and Delta Gamma=0.12(-0.10)(+0.08)(stat)+/- 0.02(syst) ps(-1). The data sample corresponds to an integrated luminosity of about 1.1 fb(-1) accumulated with the D0 detector at the Fermilab Tevatron collider. This is the first direct measurement of the CP-violating mixing phase in the B-s(0) system.
