  • Characteristic Biomarker and Cognitive Profile in Incipient Mixed Dementia.

    • Research has shown that mixed dementia is more common than previously believed but little is known of its early stages.To examine if incipient mixed dementia can be differentiated from incipient Alzheimer's disease (AD) and subcortical ischemic vascular dementia (SVD) using neuropsychological tests, cerebrospinal fluid (CSF) markers, and magnetic resonance imaging markers.We included 493 patients and controls from the Gothenburg MCI study and used the dementia groups for marker selection (CSF total-tau (T-tau), phospho-tau (P-tau), and amyloid-β42 (Aβ42), 11 neuropsychological tests, and 92 regional brain volumes) and to obtain cut-off values which were then applied to the MCI groups.Incipient mixed dementia was best differentiated from incipient AD by the Word fluency F-A-S test and the Trail making test A. CSF T-tau, P-tau, and Aβ42 differentiated incipient mixed dementia from incipient SVD.Incipient mixed dementia is characterized by an AD-like biomarker profile and an SVD-like cognitive profile. Incipient mixed dementia can be separated from incipient AD and incipient SVD using CSF markers and cognitive testing.

  • Size, shape, and density changes of biomass particles during rapid devolatilization

    • Particle properties such as size, shape and density play significant roles on particle flow and flame propagationin pulverized fuel combustion and gasification. A drop tube furnace allows for experiments athigh heating rates similar to those found in large-scale appliances, and was used in this study to carryout experiments on pulverized biomass devolatilization, i.e. detailing the first stage of fuel conversion.The objective of this study was to develop a particle conversion model based on optical informationon particle size and shape transformation. Pine stem wood and wheat straw were milled and sieved tothree narrow size ranges, rapidly heated in a drop tube setup, and solid residues were characterized usingoptical methods. Different shape descriptors were evaluated and a shape descriptor based on particleperimeter was found to give significant information for accurate estimation of particle volume. The opticalconversion model developed was proven useful and showed good agreement with conversion measuredusing a reference method based on chemical analysis of non-volatilized ash forming elements.The particle conversion model presented can be implemented as a non-intrusive method for in-situ monitoringof particle conversion, provided density data has been calibrated.

    • This report is the result of a collaborative project within the Swedish Knowledge Centre for Renewable Transportation Fuels (f3). f3 is a networking organization, which focuses on development of environmentally, economically and socially sustainable renewable fuels, and Provides a broad, scientifically based and trustworthy source of knowledge for industry, governments and public authorities, Carries through system oriented research related to the entire renewable fuels value chain, Acts as national platform stimulating interaction nationally and internationally.f3 partners include Sweden’s most active universities and research institutes within the field, as well as a broad range of industry companies with high relevance. f3 has no political agenda and does not conduct lobbying activities for specific fuels or systems, nor for the f3 partners’ respective areas of interest.The f3 centre is financed jointly by the centre partners, the Swedish Energy Agency and the region of Västra Götaland. f3 also receives funding from Vinnova (Sweden’s innovation agency) as a Swedish advocacy platform towards Horizon 2020.   • A randomly selected population sample undergoing colonoscopy : prevalence of the irritable bowel syndrome and the impact of selection factors

    • ObjectiveTo analyse the epidemiology of irritable bowel syndrome (IBS) symptoms in a random sample of the general population and in a subsample consenting to a colonoscopy, and to what extent this introduces symptom selection bias.Materials and methodsOverall, 3347 randomly selected Swedish adults aged 18-70 years were mailed the validated Abdominal Symptom Questionnaire (ASQ). Responders (n=2293; 68.5%) were contacted by phone, and 745 consented to a colonoscopy. All nonresponders were contacted by phone; 265 were reached and asked seven key ASQ questions. Colonoscopy participants also completed the Rome II Modular Questionnaire.ResultsThe prevalence of IBS on the basis of the mailed ASQ (troublesome abdominal pain and bowel disturbance in the past 3 months) was 26.2% [95% confidence interval (CI): 24.4-28.0] among the ASQ responders and 36.6% (95% CI: 33.2-40.1) among the colonoscopy participants (P<0.001). Nonresponders had a lower prevalence of IBS (15.8%; 95% CI: 11.4-20.3, P<0.001) than ASQ responders. Colonoscopy participants were slightly older than noncolonoscoped participants completing the ASQ (P<0.001), but men and women were equally represented and no significant socioeconomic differences were identified. The prevalence of IBS was 14.8% (95% CI: 12.2-17.5) on the basis of the Rome II Modular Questionnaire in colonoscopy participants and 14.5% (95% CI: 11.9-17.2) when visible inflammatory disease was excluded. Of the colonoscopy participants, 31.9% (95% CI: 28.5-35.3) were symptom free.ConclusionIBS symptoms are common and rarely explained by visible inflammatory disease or cancer. There was a modest selection bias by IBS in participants accepting a screening colonoscopy, but still, one-third were symptom free. Thus, conclusions for the general population can be made from findings in the study cohort.

  • Validity and reliability of the medial temporal lobe atrophy scale in a memory clinic population

    • Background Visual rating of medial temporal lobe atrophy (MTA) is often performed in conjunction with dementia workup. Most prior studies involved patients with known or probable Alzheimer's disease (AD). This study investigated the validity and reliability of MTA in a memory clinic population. Methods MTA was rated in 752 MRI examinations, of which 105 were performed in cognitively healthy participants (CH), 184 in participants with subjective cognitive impairment, 249 in subjects with mild cognitive impairment, and 214 in patients with dementia, including AD, subcortical vascular dementia and mixed dementia. Hippocampal volumes, measured manually or using FreeSurfer, were available in the majority of cases. Intra- and interrater reliability was tested using Cohen's weighted kappa. Correlation between MTA and quantitative hippocampal measurements was ascertained with Spearman's rank correlation coefficient. Moreover, diagnostic ability of MTA was assessed with receiver operating characteristic (ROC) analysis and suitable, age-dependent MTA thresholds were determined. Results Rater agreement was moderate to substantial. MTA correlation with quantitative volumetric methods ranged from -0.20 (p< 0.05) to -0.68 (p < 0.001) depending on the quantitative method used. Both MTA and FreeSurfer are able to distinguish dementia subgroups from CH. Suggested age-dependent MTA thresholds are 1 for the age group below 75 years and 1.5 for the age group 75 years and older. Conclusions MTA can be considered a valid marker of medial temporal lobe atrophy and may thus be valuable in the assessment of patients with cognitive impairment, even in a heterogeneous patient population.

  • How individuals with the irritable bowel syndrome describe their own symptoms before formal diagnosis

    • Aim: To investigate how individuals fulfilling the Rome II criteria for irritable bowel syndrome (IBS) spontaneously described their symptoms. Method: From a general population, 1,244 randomly sampled adults were asked to describe their gastrointestinal symptoms (if any) verbally, in their own words, at a semi-structured interview. Their own descriptions were sorted into five symptom clusters. The participants independently completed a written questionnaire (the Rome II Modular Questionnaire (RMIIMQ)). Results: A total of 601 participants reported at least one gastrointestinal symptom, and 128 had IBS according to the RMIIMQ. After exclusion of organic causes, previously diagnosed IBS, or additional gastrointestinal diagnosis, 81 participants with IBS according to RMIIMQ remained. Five participants (6%) described symptoms included in the full definition of IBS, but none fulfilled the Rome II criteria completely. Abdominal pain or other IBS-related symptoms were reported by 64 (79%), and 12 (15%) did not report any IBS-like symptom. Conclusion: Previously undiagnosed individuals, who fulfil criteria for Rome II-IBS, often express their complaints in words that do not fit into the current diagnostic criteria.

  • Forecasting of Icing Related Wind Energy Production Losses : Probabilistic and Machine Learning Approaches

    • Icing on wind turbine blades causes significant production losses for wind energy in cold climate. Next-day forecasts of these production losses are crucial for the power balance in the electrical grid and for the trading process, but they are uncertain due to lack of understanding of, and simplifications, in the modelling chain. In the present work, uncertainties in the modelling chain for icing related production losses are addressed with the aim to increase the utility of next-day production loss forecasts. Probabilistic and machine learning methods are applied both to improve the forecast skill and to estimate reliable forecast uncertainties. The different methods enable uncertainties in different parts of the chain to be addressed. A Numerical Weather Prediction (NWP) ensemble captures uncertainties in the initial conditions of the forecasts while a neighbourhood method describes uncertainties in the spatial representation of the NWP forecast at the exact locations of the wind parks. An icing model ensemble is generated in order to address uncertainties in the icing model parameters. Finally, machine learning approaches are employed to both deterministically and probabilistically address uncertainties in the modelling chain. Production data from wind parks in Sweden were used to evaluate all methods. The physically based probabilistic methods; the NWP ensemble, the neighbourhood method and the icing model ensemble, increase the forecast skill and provide valuable uncertainty estimations. The largest forecast improvement is obtained when the different probabilistic approaches are combined. On the other hand, machine learning approaches for icing related production losses demonstrate large potential. The probabilistic machine learning method employed generally outperforms every other single probabilistic method mentioned above. By applying the different methods of uncertainty quantification, the utility of icing related production loss forecast in the trading process is improved since related costs can be reduced and usage of the produced power can be optimised. These methods can also be beneficial when planning for site maintenance and for the use of de-icing systems, since icing on the wind turbines are directly or indirectly forecasted. Thus, the improved representations of uncertainties in the modelling chain contributes to an enhanced usage of wind power in cold climates.

    • The problem of icing on wind turbines in cold climates is addressed using probabilistic forecasting to improve next-day forecasts of icing and related production losses. A case study of probabilistic forecasts was generated for a 2-week period. Uncertainties in initial and boundary conditions are represented with an ensemble forecasting system, while uncertainties in the spatial representation are included with a neighbourhood method. Using probabilistic forecasting instead of one single forecast was shown to improve the forecast skill of the ice-related production loss forecasts and hence the icing forecasts. The spread of the multiple forecasts can be used as an estimate of the forecast uncertainty and of the likelihood for icing and severe production losses. Best results, both in terms of forecast skill and forecasted uncertainty, were achieved using both the ensemble forecast and the neighbourhood method combined. This demonstrates that the application of probabilistic forecasting for wind power in cold climates can be valuable when planning next-day energy production, in the usage of de-icing systems and for site safety.
