  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

    • The histopathological lesions, minimal change disease (MCD) and focal segmental glomerulosclerosis (FSGS) are entities without immune complex deposits which can cause podocyte injury, thus are frequently grouped under the umbrella of podocytopathies. Whether MCD and FSGS may represent a spectrum of the same disease remains a matter of conjecture. Both frequently require repeated high-dose glucocorticoid therapy with alternative immunosuppressive treatments reserved for relapsing or resistant cases and response rates are variable. There is an unmet need to identify patients who should receive immunosuppressive therapies as opposed to those who would benefit from supportive strategies. Therapeutic trials focusing on MCD are scarce, and the evidence used for the 2021 Kidney Disease: Improving Global Outcomes (KDIGO) guideline for the management of glomerular diseases largely stems from observational and pediatric trials. In FSGS, the differentiation between primary forms and those with underlying genetic variants or secondary forms further complicates trial design. This article provides a perspective of the Immunonephrology Working Group (IWG) of the European Renal Association (ERA) and discusses the KDIGO 2021 Clinical Practice Guideline for the Management of Glomerular Diseases focusing on the management of MCD and primary forms of FSGS in the context of recently published evidence, with a special emphasis on the role of rituximab, cyclophosphamide, supportive treatment options and ongoing clinical trials in the field. 10.1093/ndt/gfae025 Video Watch the video of this contribution at https://academic.oup.com/ndt/pages/author_videos gfae025Media1 6346336829112

  • Deep-water methane hydrates in the Arctic Ocean: Re-assessing the significance of a shallow BSR on the Lomonosov Ridge

    • Recently published multi-channel seismic data from the Lomonosov Ridge image a reversed polarity bottom-simulating reflector (BSR) tentatively attributed to the presence of deepwater marine hydrates and recognized throughout a survey area exceeding 100,000 km2. In addition to the importance of these findings for estimating Arctic hydrate reserves, if shown to correspond to the base of the hydrate stability zone, this seismic marker could provide a means for expanding spatial cover of heat flow data in deepwater settings of the Amerasian Basin, where little is known about the tectonic origin and nature of plate boundaries. As an initial test on the validity of this assumption, we develop a petrophysical model using sediments collected from circumpolar regions of the Lomonosov Ridge to derive an estimate of surface heat flow patterns from the BSR. The results show that the BSR inferred geothermal gradient and surface heat flow are exceedingly high when compared to published regional measurements. Although potential errors in the analysis may explain some of this discrepancy, the observation that the BSR remains at a constant sub-bottom depth despite large variations in water depths (> 2400 m) and relative sedimentation rates provides additional evidence that it cannot mark the base of the hydrate stability zone. A further understanding of its origin requires a more detailed investigation of the existing seismic data and highlights the need for renewed collection of heat flow data from the Arctic Ocean.

    • With the Arctic Coring Expedition (ACEX) (the first Mission Specific Platform expedition within the Integrated Ocean Drilling Program) Expedition 302 in 2004, a new era in Arctic research began. For the first time, a scientific drilling expedition in the permanently ice-covered Arctic Ocean was carried out, penetrating 428 m of Quaternary, Neogene, Paleogene, and Campanian sediment on the crest of Lomonosov Ridge close to the North Pole between 87 and 88°N. By studying the unique ACEX sequence, a large number of scientific discoveries that describe previously unknown Arctic paleoenvironments were obtained during the last decade. Key results include subtropical warm conditions during the Paleocene-Eocene Thermal Maximum and the early-mid Eocene, an episodic freshening of Arctic surface waters in the Eocene, black shales and euxinic conditions in the Eocene Arctic Ocean, and an early onset of Arctic sea ice (Northern Hemisphere glaciation) in the middle Eocene. While these results from ACEX were unprecedented, key questions related to the climate history of the Arctic Ocean on its course from Greenhouse to Icehouse conditions during early Cenozoic times remain unanswered, in part because of poor core recovery, and in part because of the possible presence of a major mid-Cenozoic hiatus within the ACEX record. Furthermore, the ACEX sites remain the one and only drill holes in the entire central Arctic Ocean to date. In order to decipher the paleoclimatic and tectonic history of this unique and sensitive but still not well-known region on Earth, future scientific Arctic drilling is certainly needed.

    • Theories of emotion postulate that emotional input is processed independently from perceptual awareness.Although visual masking has a long tradition in studying whether emotional pictures are processedbelow a supposed threshold of perceptual awareness (subliminal perception), a consensus has yet to bereached. This article reviews current concerns in the use of visual masking. These include a reliablepresentation method, the role of masking pictures, common definitions of awareness and their problems,current models of awareness, and neural mechanisms. A useful strategy may be the study of dose–response relationships between awareness and emotion processing that avoids a dichotomous view ofawareness and allows conclusions about the relative independence of emotional processing fromawareness.

    • What is the recommended assessment and management of those with polycystic ovary syndrome (PCOS), based on the best available evidence, clinical expertise, and consumer preference?International evidence-based guidelines address prioritized questions and outcomes and include 254 recommendations and practice points, to promote consistent, evidence-based care and improve the experience and health outcomes in PCOS.The 2018 International PCOS Guideline was independently evaluated as high quality and integrated multidisciplinary and consumer perspectives from six continents; it is now used in 196 countries and is widely cited. It was based on best available, but generally very low to low quality, evidence. It applied robust methodological processes and addressed shared priorities. The guideline transitioned from consensus based to evidence-based diagnostic criteria and enhanced accuracy of diagnosis, whilst promoting consistency of care. However, diagnosis is still delayed, the needs of those with PCOS are not being adequately met, evidence quality was low and evidence-practice gaps persist.The 2023 International Evidence-based Guideline update reengaged the 2018 network across professional societies and consumer organizations with multidisciplinary experts and women with PCOS directly involved at all stages. Extensive evidence synthesis was completed. Appraisal of Guidelines for Research and Evaluation-II (AGREEII)-compliant processes were followed. The Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) framework was applied across evidence quality, feasibility, acceptability, cost, implementation and ultimately recommendation strength and diversity and inclusion were considered throughout.This summary should be read in conjunction with the full Guideline for detailed participants and methods. Governance included a six-continent international advisory and management committee, five guideline development groups, and paediatric, consumer, and translation committees. Extensive consumer engagement and guideline experts informed the update scope and priorities. Engaged international society-nominated panels included paediatrics, endocrinology, gynaecology, primary care, reproductive endocrinology, obstetrics, psychiatry, psychology, dietetics, exercise physiology, obesity care, public health and other experts, alongside consumers, project management, evidence synthesis, statisticians and translation experts. Thirty-nine professional and consumer organizations covering 71 countries engaged in the process. Twenty meetings and five face-to-face forums over 12 months addressed 58 prioritized clinical questions involving 52 systematic and 3 narrative reviews. Evidence-based recommendations were developed and approved via consensus across five guideline panels, modified based on international feedback and peer review, independently reviewed for methodological rigour, and approved by the Australian Government National Health and Medical Research Council (NHMRC).The evidence in the assessment and management of PCOS has generally improved in the past five years, but remains of low to moderate quality. The technical evidence report and analyses (∼6000 pages) underpins 77 evidence-based and 54 consensus recommendations, with 123 practice points. Key updates include: i) further refinement of individual diagnostic criteria, a simplified diagnostic algorithm and inclusion of anti-Müllerian hormone (AMH) levels as an alternative to ultrasound in adults only; ii) strengthening recognition of broader features of PCOS including metabolic risk factors, cardiovascular disease, sleep apnea, very high prevalence of psychological features, and high risk status for adverse outcomes during pregnancy; iii) emphasizing the poorly recognized, diverse burden of disease and the need for greater healthcare professional education, evidence-based patient information, improved models of care and shared decision making to improve patient experience, alongside greater research; iv) maintained emphasis on healthy lifestyle, emotional wellbeing and quality of life, with awareness and consideration of weight stigma; and v) emphasizing evidence-based medical therapy and cheaper and safer fertility management.Overall, recommendations are strengthened and evidence is improved, but remain generally low to moderate quality. Significantly greater research is now needed in this neglected, yet common condition. Regional health system variation was considered and acknowledged, with a further process for guideline and translation resource adaptation provided.The 2023 International Guideline for the Assessment and Management of PCOS provides clinicians and patients with clear advice on best practice, based on the best available evidence, expert multidisciplinary input and consumer preferences. The guideline was peer reviewed by special interest groups across our 39 partner and collaborating organizations, was independently methodologically assessed against AGREEII criteria and was approved by all members of the guideline development groups and by the NHMRC.
