    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

  • Intrafamilial and interfamilial heterogeneity of PINK1-associated Parkinson's disease in Sudan

    • PINK1 is the second most predominant gene associated with autosomal recessive Parkinson's disease. Homo-zygous mutations in this gene are associated with an early onset of symptoms. Bradykinesia, tremors, and rigidity are common features, while dystonia, motor fluctuation, and non-motor symptoms occur in a lower percentage of cases and usually respond well to levodopa. We investigated 14 individuals with parkinsonism and eleven symptom-free siblings from three consanguineous Sudanese families, two of them multigenerational, using a custom gene panel screening 34 genes, 27 risk variants, and 8 candidate genes associated with parkinsonism. We found a known pathogenic nonsense PINK1 variant (NM_032409.3:c.1366C>T; p.(Gln456*)), a novel pathogenic single base duplication (NM_032409.3:c.1597dup; p.(Gln533Profs*29)), and another novel pathogenic insertion (NM_032409.3:c.1448_1449ins[1429_1443; TTGAG]; p.(Arg483Serfs*7)). All variants were homozygous and co -segregated in all affected family members. We also identified intrafamilial and interfamilial phenotypic het-erogeneity associated with PINK1 mutations in these Sudanese cases, possibly reflecting the nature of the Sudanese population that has a large effective population size, which suggests a higher possibility of novel findings in monogenic and polygenic diseases in Sudan.

    • Groundwater in arid and semiarid regions is vital resource for many uses and therefore information about concentrations of uranium isotopes among other chemical parameters are necessary. In the study presented here, distribution of U-238 and U-235 in groundwater of four selected locations in the southern Arabian peninsula, namely at two locations within the United Arab Emirates (UAE) and two locations in Oman are discussed. The analyses of the uranium isotopes were performed using ICP-MS and the results indicated a range of concentrations for U-235 and (238) U at 3-39 ng L-1 (average: 18 ng L-1) and 429-5,293 ng L-1 (average: 2,508 ng L-1) respectively. These uranium concentrations are below the higher permissible WHO limit for drinking water and also comparable to averages found in groundwater from similar aquifers in Florida and Tunisia. Negative correlation between rainfall and uranium concentrations suggests that in lithologically comparable aquifers, climate may influence the concentration of uranium in subtropical to arid regions.

  • Late Holocene pathway of Asian Summer Monsoons imprinted in soils and societal implications

    • The Asian Summer Monsoons (ASM) represent the main source of precipitation in China and East Asia with about one third of the world population and a region of widespread civilizations. Identifying the temporal and spatial patterns (pathways) of these monsoonal events during the Late Holocene to today has been a matter of debate amongst the scientific community. Here we show that the distribution patterns of the cosmogenic isotope Be-10 and oceanic I-127 in the topsoil across China exhibit imprints of the main ASM pathways. Our results indicate the monsoon pathway pattern persisted for several millennia or more and suggest a strong bond between Be-10 and water vapor transport patterns. Our data also reveal a(127)I distribution pattern controlled by the ASM pathways, rather than proximity to the sea or bedrock weathering. The persistent pathway of the ASM during the late Holocene, together with higher than average global soil iodine concentration, may have further strengthened the development of civilizations in this region of the world through reduction of iodine deficiency related diseases.
