  • Carotid artery stenting compared with endarterectomy in patients with symptomatic carotid stenosis (International Carotid Stenting Study): an interim analysis of a randomised controlled trial

    • Background Stents are an alternative treatment to carotid endarterectomy for symptomatic carotid stenosis, but previous trials have not established equivalent safety and efficacy. We compared the safety of carotid artery stenting with that of carotid endarterectomy. Methods The International Carotid Stenting Study (ICSS) is a multicentre, international, randomised controlled trial with blinded adjudication of outcomes. Patients with recently symptomatic carotid artery stenosis were randomly assigned in a 1:1 ratio to receive carotid artery stenting or carotid endarterectomy. Randomisation was by telephone call or fax to a central computerised service and was stratified by centre with minimisation for sex, age, contralateral occlusion, and side of the randomised artery. Patients and investigators were not masked to treatment assignment. Patients were followed up by independent clinicians not directly involved in delivering the randomised treatment. The primary outcome measure of the trial is the 3-year rate of fatal or disabling stroke in any territory, which has not been analysed yet. The main outcome measure for the interim safety analysis was the 120-day rate of stroke, death, or procedural myocardial infarction. Analysis was by intention to treat (ITT). This study is registered, number ISRCTN25337470. Findings The trial enrolled 1713 patients (stenting group, n=855; endarterectomy group, n=858). Two patients in the stenting group and one in the endarterectomy group withdrew immediately after randomisation, and were not included in the ITT analysis. Between randomisation and 120 days, there were 34 (Kaplan-Meier estimate 4.0%) events of disabling stroke or death in the stenting group compared with 27 (3.2%) events in the endarterectomy group (hazard ratio [HR] 1.28, 95% CI 0.77-2.11). The incidence of stroke, death, or procedural myocardial infarction was 8.5% in the stenting group compared with 5.2% in the endarterectomy group (72 vs 44 events; HR 1.69, 1.16-2.45, p=0.006), Risks of any stroke (65 vs 35 events; HR 1.92, 1.27-2.89) and all-cause death (19 vs seven events; HR 2.76, 1.16-6.56) were higher in the stenting group than in the endarterectomy group. Three procedural myocardial infarctions were recorded in the stenting group, all of which were fatal, compared with four, all non-fatal, in the endarterectomy group. There was one event of cranial nerve palsy in the stenting group compared with 45 in the endarterectomy group. There were also fewer haematomas of any severity in the stenting group than in the endarterectomy group (31 vs 50 events; p=0.0197). Interpretation Completion of long-term follow-up is needed to establish the efficacy of carotid artery stenting compared with endarterectomy. In the meantime, carotid endarterectomy should remain the treatment of choice for patients suitable for surgery.

    • High-resolution far-infrared and sub-millimetre spectroscopy of water lines is an important tool to understand the physical and chemical properties of cometary atmospheres. We present observations of several rotational ortho- and para-water transitions in comet C/2008 Q3 (Garradd) performed with HIFI on Herschel. These observations have provided the first detection of the 2(12)-1(01) (1669 GHz) ortho and 1(11)-0(00) (1113 GHz) para transitions of water in a cometary spectrum. In addition, the ground-state transition 1(10)-1(01) at 557 GHz is detected and mapped. By detecting several water lines quasi-simultaneously and mapping their emission we can constrain the excitation parameters in the coma. Synthetic line profiles are computed using excitation models which include excitation by collisions, solar infrared radiation, and radiation trapping. We obtain the gas kinetic temperature, constrain the electron density profile, and estimate the coma expansion velocity by analyzing the map and line shapes. We derive water production rates of 1.7-2.8 x 10(28) s(-1) over the range r(h) = 1.83-1.85 AU.

    • An x-ray streak camera system uses a laser triggered photo-conductive switch to synchronize the high voltage ramp applied to its sweep plates to the photo-excitation of a sample. This technique allows the jitter. between successive sweeps to be kept below 100 fs compared to over 1 ps:using classical methods. By accumulating a stable pulsed x-ray signal over many sweeps, a very high dynamic range can be achieved whilst maintaining a sub-picosecond time resolution. The ultimate time resolution is limited by the dispersion of electrons in the streak tube. Whilst triggering with a laser at 900 Hz we have achieved a time resolution (FWHM) of 640 fs at a detection wavelength of 267 nm for a 60 s accumulation period, corresponding to over 50 000 sweeps.

    • Coherent folded acoustic phonons in a multilayered GaSb/InAs epitaxial heterostructure were generated by femtosecond laser pulses and studied by means of ultrafast x-ray diffraction. Coherent phonons excited simultaneously in the fundamental acoustic branch and the first back-folded branch were detected. This represents the first clear evidence for phonon branch folding based directly on the atomic motion to which x-ray diffraction is sensitive. From a comparison of the measured phonon-modulated x-ray reflectivity with simulations, evidence was found for a reduction of the laser penetration depth. This reduction can be explained by the self-modulation of the refractive index due to photogenerated free carriers.

    • Atomic processes like e.g. molecular vibrations, chemical reactions or phase transitions happen on picosecond down to femtosecond time scales. Novel pulsed X-ray sources or alternatively ultrafast X-ray detectors allow the investigation of these processes in real time. A powerful tool for the investigation of the dynamics in crystalline materials is time resolved X-ray diffraction (TRXD). As an example the authors present the measurement of "phonon branch folding" in a GaSb/InAs superlattice by means of TRXD. In the second part we look forward to the near future of TRXD. X-ray scattering from coherent acoustic and optical phonons has recently become describable within the framework of dynamical diffraction theory. This theory provides the means for the detailed modeling of how various lattice dynamical processes manifest themselves in the diffracted X-ray signal. Simulations are presented showing the effects of coherent acoustic and optical phonons on the rocking curve of quartz (010)
