Detta är några av de frågor som ställs i artiklarna där svaren kanske kan överraska.Andra artiklar handlar om hur man stimulerar elever till att läsa och skriva, problematiken när det gäller att undervisa i svenska som andraspråk och varför elever väljer - eller inte väljer - att studera något annat främmande språk än engelska.     • Digitalisation is rapidly changing the conditions for providing healthcare. New digital products and services are continuously integrated into the operations although not without frequent hurdles and difficulties. In addition, the ehealth market for public healthcare is dominated by fairly large companies while smaller, entrepreneurial companies have difficulties accessing the public health market. The purpose for the project reported in this paper is to examine how the competitiveness of ehealth entrepreneurs can be increased by gaining access to public healthcare. The aim is to develop a model describing the necessary conditions for a common development-process involving organisations in healthcare and small and medium size companies (SMEs), which can serve as a support in iterative improvement activities. The main methodology used in the project has been interactive workshops led by a professional moderator. Ten workshops involving representatives from healthcare, elderly care, SMEs and academia as well as politicians have been carried out. The findings are summarised in the model which gradually has emerged. The model has four vital cornerstones: understanding of needs, technical openness, procurement process and collaboration forum. Understanding of needs is a crucial aspect both from the perspective of healthcare and from SMEs. Currently, dialogue is lacking and SMEs do not in enough detail understand the processes taking place in healthcare and thus which the problems are. On the other hand, the healthcare representatives often do not have any overview of which suppliers and products exist and in what way they can aid in their operations. This leads to the procurement process often being ad hoc at the same time as it is limited by public procurement rules which frequently are too rigidly followed. Consequently, competence development of both healthcare organisations and SMEs is key to success. This development process will be highly facilitated by a collaboration forum – an arena in which healthcare representatives and SMEs can meet and learn from each other. The model, with its cornerstones, constitutes the main finding of the project.

  • Challenging conventional karyotyping by next-generation karyotyping in 281 intensively treated patients with AML

    • Although copy number alterations (CNAs) and translocations constitute the backbone of the diagnosis and prognostication of acute myeloid leukemia (AML), techniques used for their assessment in routine diagnostics have not been reconsidered for decades. We used a combination of 2 next-generation sequencing-based techniques to challenge the currently recommended conventional cytogenetic analysis (CCA), comparing the approaches in a series of 281 intensively treated patients with AML. Shallow whole-genome sequencing (sWGS) outperformed CCA in detecting European Leukemia Net (ELN)-defining CNAs and showed that CCA overestimated monosomies and suboptimally reported karyotype complexity. Still, the concordance between CCA and sWGS for all ELN CNA-related criteria was 94%. Moreover, using in silico dilution, we showed that 1 million reads per patient would be enough to accurately assess ELN-defining CNAs. Total genomic loss, defined as a total loss 200 Mb by sWGS, was found to be a better marker for genetic complexity and poor prognosis compared with the CCA-based definition of complex karyotype. For fusion detection, the concordance between CCA and whole-transcriptome sequencing (WTS) was 99%. WTS had better sensitivity in identifying inv(16) and KMT2A rearrangements while showing limitations in detecting lowly expressed PML-RARA fusions. Ligation-dependent reverse transcription polymerase chain reaction was used for validation and was shown to be a fast and reliable method for fusion detection. We conclude that a next-generation sequencing-based approach can replace conventional CCA for karyotyping, provided that efforts are made to cover lowly expressed fusion transcripts.

  • Vehicle Driver Monitoring : sleepiness and cognitive load

    • To prevent road crashes, it is important to understand driver related contributing factors. The overall aim of the Vehicle Driver Monitoring project was to advance the understanding of two such factors; sleepiness and cognitive distraction. The project aimed at finding methods to measure the two states, with focus on physiological measures, and to study their effect on driver behaviour. The data collection was done in several laboratory and driving simulator experiments. Much new knowledge and insights were gained in the project. Significant effects of cognitive load as well as of sleepiness were found in several physiological measures. The results also showed that context, including individual and environmental factors, has a great impact on driver behaviours, measures and driver experiences.

    • A 0.5-13 GHz cryogenic MMIC low-noise amplifier (LNA) was designed and fabricated using a 130 nm InP HEMT process. A packaged LNA has been measured at both 300 K and 15 K. At 300 K the measured minimum noise temperature was 48 K at 7 GHz. At 15 K the measured minimum noise temperature was 3 K at 7 GHz and below 7 K within the entire 0.5-13 GHz band. The gain was between 34 dB and 40 dB at 300 K and between 38 dB and 44 dB at 4 K.

    • We present in this letter an InGaAs/InAlAs/InP high-electron-mobility transistor (InP HEMT) with record noise temperature at very low dc power dissipation. By minimizing parasitic contact and sheet resistances and the gate current, a 130-nm-gate-length InP HEMT was optimized for cryogenic low-noise operation. When integrated in a 4- to 8-GHz three-stage hybrid low-noise amplifier operating at 10 K, a noise temperature of 1.2 K +/- 1.3 K at 5.2 GHz was measured. The gain of the amplifier across the entire band was 44 dB, consuming only 4.2 mW of dc power. The extracted minimum noise temperature of the InP HEMT was 1 K at 6 GHz.

    • Severe infection, including sepsis, is an increasing clinical problem that causes prolonged morbidity and substantial mortality. At present, antibiotics are essentially the only pharmacological treatment for sepsis. The incidence of resistance to antibiotics is increasing and it is therefore critical to find new therapies for sepsis. Staphylococcus aureus (S. aureus) is a major cause of septic mortality. Neutrophils play an important role in the defense against bacterial infections. We have shown that a diet with high levels of dietary saturated fatty acids decreases survival in septic mice, but the mechanisms behind remain elusive. The aim of the present study was to investigate how the differences in dietary fat composition affect survival and bacterial load after experimental septic infection and neutrophil function in uninfected mice. We found that, after S. aureus infection, mice fed polyunsaturated high fat diet (HFD/P) for 8 weeks had increased survival and decreased bacterial load during sepsis compared with mice fed saturated high fat diet (HFD/S), and similar to that of mice fed low fat diet (LFD). Uninfected mice fed HFD/P had increased frequency of neutrophils in bone marrow compared with mice fed HFD/S. In addition, mice fed HFD/P had a higher frequency of neutrophils recruited to the site of inflammation in response to peritoneal injection of thioglycollate compared with HFD/S. Differences between the proportion of dietary protein and carbohydrate did not affect septic survival at all. In conclusion, polyunsaturated dietary fat increased both survival and efficiency of bacterial clearance during septic S. aureus infection. Moreover, this diet increased the frequency and chemotaxis of neutrophils, key components of the immune response to S. aureus infections.

    • Influenza causes seasonal epidemics of respiratory infection in all parts of the world. Manifestations of influenza range from mild upper to severe lower respiratory tract infection. Medical risk groups are defined by factors predisposing for development of severe disease and are recommended annual vaccination as a protective measure. The previous Swedish treatment guidelines for influenza were issued in 2011, and a review of current evidence was deemed relevant. An important reason to revisit the guidelines is the recent approval of a novel drug for influenza treatment, baloxavir. Updated Swedish evidence-based guidelines created by a group of experts from various research areas, for the management of influenza are presented here. The work has been made in collaboration with the Public Health Agency of Sweden and the Swedish Reference Group for AntiViral therapy (RAV). The updated guidelines include guidelines for diagnostics, treatment and prophylaxis in special groups, including management of pregnant women and children with influenza. A new section about infection control has been added. Pharmacological treatment is covered in detail with regards to indication and dosage. Additionally, drug resistance and environmental aspects are discussed.
