    • A new method for assessment of dermal exposure to pesticides in subsistence farmers by use of determinants of dermal exposure is described. The method, called the determinants of dermal exposure ranking method (DERM), is a combination of checklists and expert rating assessment. Thus, determinants are listed in a form, which is used to check their presence and to assess them using a simple algorithm based on two factors, the type of transport process (T value) and the area of body surface exposed (A value). In addition, the type of clothing worn during applications is included as a protection factor. We applied the DERM to real pesticide applications, characterizing dermal exposure and comparing DERM estimates with earlier developed semiquantitative visual scores based on fluorescent tracer, the total visual score (TVS) and contaminated body area (CBA). DERM showed a very good level of agreement with both the TVS (r = 0.69, P = 0.000) and the CBA (r = 0.67, P = 0.000). DERM allowed identification of the determinants that had the highest effect on exposure and the farmers with the highest exposure. In conclusion, DERM provided information on the determinants responsible for dermal exposure in a group of subsistence farmers. This can be useful to design monitoring and preventive programs, define priorities for intervention and prioritize and select most adequate measurement strategies. DERM promises to be a low-cost easy-to-use method to assess dermal exposure to pesticides in developing country conditions.

  • Patterns of working and living conditions : a holistic, multivariate approach to occupational health studies

    • The objective of the study was to develop a multivariate approach to occupational health studies that is: capable of identifying groups with similar working conditions; relevant for studies of associations between working and living conditions and health; and an appropriate basis for preventive actions. Data at the individual level were obtained through measurements, observations, interviews and questionnaires, and at the organizational level, through interviews with managers. Cluster analyses were applied with the purpose of identifying groups of individuals with small, within-group differences. Eighty work sites and a sample of employees at each site were strategically selected. The study group comprised 203 men and women, and was characterized by large variation. The final analysis produced eight clusters of individuals, denoted according to their best-defining characteristic, i.e. 'decent', 'boundary-less', 'locked', 'exposed', 'heavy and monotonous', 'changed', 'mobile' and 'restrained'. The clusters differed with regard to 'what' characterized working conditions, 'where' on the labour market they were found, and 'who' clustered in these groups. The holistic approach revealed conditions that were important for health and had higher explanatory power in relation to ill-health than applying socio-economic groupings or the demand-control-support model. It showed how psychosocial, ergonomic-physical and occupational hygiene factors combine and interact to create settings with different risks of ill-health. The chosen strategy is recommended for future occupational health studies and is particularly suitable as guidance for preventive actions relevant to specific clusters of working and living conditions.

  • Long-Term Exposure to Road Traffic Noise and Myocardial Infarction

    • Background: An association has been reported between long-term exposure to road traffic noise and the risk of myocardial infarction (MI), but the evidence is limited and inconclusive. No previous study has simultaneously analyzed the role of exposure to noise and air pollution from road traffic in the risk of MI. Methods: A population-based case-control study on MI was conducted 1992-1994 in Stockholm County. Participants answered a questionnaire and underwent a physical examination. Residential exposure to noise and air pollution from road traffic between 1970 and 1992-1994 was assessed for 3666 participants (1571 cases of MI and 2095 controls), based on residential history combined with information on traffic intensity and distance to nearby roads. Information was also obtained on factors potentially affecting the relationship between noise exposure and MI, such as noise annoyance. Results: The correlation between long-term individual exposure to noise and air pollution from traffic was high (r = 0.6). The adjusted odds ratio for MI associated with long-term road traffic noise exposure of 50 dBA or higher was 1.12 (95% confidence interval = 0.95-1.33). In a subsample, defined by excluding persons with hearing loss or exposure to noise from other sources, the corresponding odds ratio was 1.38 (1.11-1.71), with a positive exposure-response trend. No strong effect modification was apparent by sex or cardiovascular risk factors, including air pollution from road traffic. Conclusions: The results lend some support to the hypothesis that long-term exposure to road traffic noise increases the risk for MI.
