  • Soluble tumor necrosis factor receptor 1 is associated with glomerular filtration rate progression and incidence of chronic kidney disease in two community-based cohorts of elderly individuals

    • Objective: We aimed to explore and validate the longitudinal associations between soluble tumor necrosis factor receptor 1 (sTNFR1), glomerular filtration rate (GFR) progression, and chronic kidney disease (CKD) incidence in two independent community-based cohorts of elderly individuals with prespecified subgroup analyses in individuals without prevalent diabetes.Research design and methods: Two community-based cohorts of elderly individuals were used with 5-year follow-up data on estimated GFR: the Uppsala Longitudinal Study of Adult Men (ULSAM; n = 437 men; mean age: 78 years) and the Prospective Investigation of the Vasculature in Uppsala Seniors (PIVUS; n = 703; mean age: 70 years; 51% women). GFR categories were defined as >= 60, 30-60, and <30 ml/min/1.73 m(2).Results: In longitudinal multivariable logistic regression models adjusted for inflammatory markers and established cardiovascular risk factors, higher serum sTNFR1 was significantly associated with an increased risk to progress to a lower GFR category in both ULSAM and PIVUS [odds ratio (OR) per standard deviation (SD) increase 1.28 (95% CI 1.03-1.60) and OR 1.56 (95% CI 1.30-1.87), respectively]. Also, in subgroup analyses in individuals with a GFR >= 60 ml/min/1.73 m(2) at baseline, higher sTNFRs were associated with incident CKD after 5 years in both cohorts [ULSAM: OR per SD increase 1.49 (95% CI 1.16-1.9) and PIVUS: OR 1.84 (95% CI 1.50-2.26)]. Associations were similar in individuals without diabetes.Conclusions: Higher circulating sTNFR1 independently predicts the progression to a worse GFR category and CKD incidence in elderly individuals even in the absence of diabetes. Further studies are warranted to investigate the underlying mechanisms, and to evaluate the clinical relevance of our findings.

    • Spectra from the Rydberg series 1s-np for the principal quantum numbers n greater than or equal to 8 of Cl16+ are observed on the JET tokamak. Individual lines up to n = 15 are resolved. The line intensity of n = 10 is enhanced, relative to the underlying 1/n(3) scaling, due to charge exchange collisions with neutral deuterium in the ground state. The observations are compared with predictions based on available partial charge exchange cross section data. There is generally good agreement, however, some details of the predicted n-shell distribution are different from those observed in the experiment.

    • The reactions in the hot spot area and in droplets during top lance blowing in a laboratory induction furnace were investigated. The experiments were performed in alumina crucible, with an inner diameter of 150mm and a height of 300mm. In hot spot sampling, the lance was lowered with a flow of inert gas through it to detect where the hot spot was going to be situated. In metal droplet sampling, the droplets were captured with an alumina cup that had a quartz tube as grip. A difference in size, appearance and shape were noted between droplets from melts with and without silicon addition.

  • A Struggle for the Realm : Late-medieval Swedish rhyme chronicles as ideological expressions

    • The period from the mid-fifteenth century to the early sixteenth century was characterised by recurrent political turmoil and conflict in Scandinavia in general, and in Sweden in particular. Political power was contested, not least because of the disputed conditions on which the Swedish realm should be governed as part of the Scandinavian union monarchy. Contenders for power based their claims on support from various factions of the ecclesiastical and secular aristocracy. Other groups within the realm played a crucial role as allies, supporters or armed levies.In A Struggle for the Realm, Margaretha Nordquist examines the late-medieval Swedish rhyme chronicles Sturekrönikan (c. 1496–1497) and Cronica Swecie (c. 1520–1523) as ideological responses to contemporary conflicts of power. The rhyme chronicles describe some of the key events and actors in the period, in ways that both differ and overlap. As partisan narratives written in support of particular political factions, the rhyme chronicles can be seen as verbal contributions in a struggle for the realm. Nordquist's analysis shows that the realm functioned as a fundamental ideological construct in the chronicles. The realm and the Swedish community were key mobilising elements in the efforts to legitimate and consolidate certain positions of power, but the precise meaning and implications of the Swedish realm and the Swedish community were not fixed. The community of Swedes is primarily envisioned as a political and social entity, although the common, ancient origin of the people is an important element in Cronica Swecie. The concept of regnal ideology is used to describe the creation of ideological meaning in the rhyme chronicles. While the realm possesses substantial ideological validity in both chronicles, they differ in the particular configuration of power they advocate as a basis for the government of the realm.

  • Genetic links between the acute-phase response and arthritis development in rats.

    • OBJECTIVE: The acute-phase inflammatory response is closely correlated with the development of rheumatoid arthritis, but the pathophysiologic role of its specific components is largely unknown. We investigated the genetic control of the acute-phase protein response in pristane-induced arthritis (PIA), which is a chronic erosive arthritis model in rats. METHODS: Plasma levels of the acute-phase proteins interleukin-6 (IL-6), alpha1-acid glycoprotein (orosomucoid), fibrinogen, and alpha1-inhibitor3 were quantified in 3 strains of rats during the development and progression of disease: DA and LEW.1F, which are susceptible to arthritis, and E3, which is resistant. Genetic linkage analysis was performed on an F2 intercross between E3 and DA to determine the genetic control of the acute-phase response in arthritis. Elevated levels of alpha1-acid glycoprotein were associated with acute inflammation, whereas levels of IL-6 were increased during the entire course of the disease. RESULTS: Using these acute-phase markers as quantitative traits in linkage analysis revealed a colocalization of loci controlling the acute-phase response and regions previously shown to control the development of arthritis in chromosomes 10, 12, and 14. In addition, 2 loci that were not associated with arthritis were found to regulate serum levels of the acute-phase protein Apr1 (acute-phase response 1) at the telomeric end of chromosome 12 and Apr2 on chromosome 5. CONCLUSION: The PIA model in rats is a useful tool for understanding some of the pathways leading to chronic erosive arthritis. The analysis of acute-phase proteins in PIA and its application as quantitative traits for studying the genetics of arthritis will promote the understanding of the genetic regulation of the acute-phase response.

  • Increased mortality after fracture of the surgical neck of the humerus: a case-control study of 253 patients with a 12-year follow-up.

    • Background: Several studies have shown a higher mortality rate in patients with osteoporosis-related fractures of the hip and vertebrae. Method: In 1999, we did a long-term follow-up case-control mortality study of 253 patients, mean age 72 years, who had sustained a fracture of the surgical neck of the humerus in 1987. Results: We found a higher mortality in fracture patients giving at end point a cumulative survival difference of 16%. The median survival time was 8.9 years in patients and 12 years in controls (pÊ= 0.005). The mortality rate was higher in men during the first 3 years after fracture and fewer than half of the male patients survived this period. The median survival time was 6.5 years in male patients and 12 years in their male controls (pÊ= 0.02). The mortality was only slightly higher in women (pÊ= 0.06). Interpretation: Cardiovascular disease and malignancy were the commonest causes of death in both groups. We could not explain the higher mortality rate in patients with a fracture of the surgical neck of the humerus.
