    • Objectives For the last decade a host of different projects have been launched to allow persons who are concerned about their hearing status to quickly and at a low cost test their hearing ability. Most often, this is carried out without collecting complementary information that could be correlated with hearing impairment. In this two-part study we first, present the development and validation of a novel Internet-based hearing test, and second, report on the associations between this test and phonological representation, quality of life and self-reported hearing difficulties.Design Cross-sectional study.Setting An opportunity sample of participants was recruited at the Stockholm central station for the first study. All parts of the second study were conducted via the Internet, with testing and self-report forms adapted for online use.Participants The first part of the study was carried out in direct contact with the participants, and participants from the second study were recruited by means of advertisements in newspapers and on webpages. The only exclusion criterion was that participants had to be over 18 years old. Most participants were between 60 and 69 years old. There were almost an equal number of men and women (total n=316).Outcome measures 48 participants failed the Internet-based hearing screening test. The group failing the test reported more problems on the Amsterdam Inventory of Auditory Disability. In addition, they were found to have diminished phonological representational skills. However, no difference in quality of life was found.Conclusions Almost one in five participants was in need of contacting their local hearing clinic. This group had more complaints regarding tinnitus and hyperacusis, rated their own hearing as worse than those who passed, and had a poorer capability of generating accurate phonological representations. This study suggests that it is feasible to screen for hearing status online, and obtain valid data.

  • Online administration of a speech in noise test and its relationship to cognition, hearing problems and mental health

    • Hearing loss is common, but often both undetected and untreated. In this two-part study we evaluated an online hearing test and used this test to explore potential links between hearing status, cognitive abilities, psychological distress as well as quality of life.Out of a total of 1370 online recruited participants who completed the procedure, 16.2% failed the online hearing test. Hearing difficulties were more prevalent among the older participants. Poor self-rated hearing ability, as measured by the Amsterdam Inventory of Auditory Handicap, increased the odds ratio for failing the hearing test (OR 2.34, 95 % CI 1.74-3.15). The same was true for scoring above the cut-off score of 11 on the anxiety subscale on the Hospital Anxiety and Depression Scale (OR 2.55, 95 % CI 1.22-5.33). On the other hand, good performance on the cognitive tasks lowered the risk for a failed hearing test.We conclude that online hearing tests may have the potential to reduce the time lag between noticing hearing difficulties and beginning a process to address the problem. Moreover, online data collection facilitate large scale investigations on the links between hearing, cognition and perceived communication and mental health problems.

  • Bridging the Gap Between Hearing Screening and Successful Rehabilitation : Research Protocol of a Randomized Controlled Trial of Motivational Interviewing via Internet

    • Purpose: Studies point to low help-seeking after a failed hearing screening. This research forum article presents the research protocol for a randomized controlled trial of motivational interviewing via the Internet to promote help-seeking in people who have failed an online hearing screening. Method: Adults who fail a Swedish online hearing screening, including a speech-in-noise recognition test, will be randomized to either an intervention group (participating in motivational interviewing) or an active control group (reading a book on history of hearing aids). Both of the conditions will be delivered via the Internet. The primary outcome is experience with seeking health care and using hearing aids 9 months after the intervention. Secondary outcomes are changes in before and after measures of self-reported hearing difficulties, anxiety, depression, and quality of life. Stages of change and self-efficacy in hearing help-seeking are measured immediately after intervention and at a 9-month follow-up for the purpose of mediation analysis. Results: The results of this randomized controlled trial may help bridge the gap between hearing screening and successful hearing rehabilitation. Conclusion: Although no large instantaneous benefits are expected, a slow change toward healthy behaviors—seeking health care and using hearing aids—would shed light on how to use the Internet to assist people with hearing impairment.

  • International Outcome Inventory for Hearing Aids : Data From a Large Swedish Quality Register Database

    • Purpose: The purpose of this study was to analyze a database of completed International Outcome Inventory for Hearing Aids (IOI-HA) questionnaires obtained from over 100,000 clients fitted with new hearing aids in Sweden during the period of 2012-2016. Mean IOI-HA total scores were correlated with degree of hearing loss, unilateral versus bilateral fitting, first-time versus return clients, gender, and variation among dispensing clinics. The correlations with expectations, service quality, and technical functioning of the hearing aids were also analyzed. Method: Questionnaires containing the 7 IOI-HA items as well as questions concerning some additional issues were mailed to clients 3-6 months after fitting of new hearing aids. The questionnaires were returned to and analyzed by an independent research institute. Results: More than 100 dispensing clinics nationwide take part in this project. A response rate of 52.6% resulted in 106,631 data sets after excluding incomplete questionnaires. Forty-six percent of the responders were women, and 54% were men. The largest difference in mean score (0.66) was found for the IOI-HA item "use" between return clients and first-time users. Women reported significantly higher (better) scores for the item "impact on others" compared with men. The bilaterally fitted subgroup reported significantly higher scores for all 7 items compared with the unilaterally fitted subgroup. Experienced users produced higher scores on benefit and satisfaction items, whereas first-time users gave higher scores for residual problems. No correlation was found between mean IOI-HA total score and average hearing threshold level (pure-tone average [ PTA]). Mean IOI-HA total scores were found to correlate significantly with perceived service quality of the dispensing center and with the technical functionality of the hearing aids. Conclusions: When comparing mean IOI-HA total scores from different studies or between groups, differences with regard to hearing aid experience, gender, and unilateral versus bilateral fitting have to be considered. No correlation was found between mean IOI-HA total score and degree of hearing loss in terms of PTA. Thus, PTA is not a reliable predictor of benefit and satisfaction of hearing aid provision as represented by the IOI-HA items. Identification of a specific lower fence in PTA for hearing aid candidacy is therefore to be avoided. Large differences were found in mean IOI-HA total scores related to different dispensing centers.

    • The Hearing at Home (HaH) project focuses on the needs of hearing-impaired people in home environments. The project is researching and developing an innovative media-center solution for hearing support, with several integrated features that support perception of speech and audio, such as individual loudness amplification, noise reduction, audio classification and event detection, and the possibility to display an animated talking head providing real-time speechreading support. In this paper we provide a brief project overview and then describe some recent results related to the audio classifier and the talking head. As the talking head expects clean speech input, an audio classifier has been developed for the task of classifying audio signals as clean speech, speech in noise or other. The mean accuracy of the classifier was 82%. The talking head (based on technology from the SynFace project) has been adapted for German, and a small speech-in-noise intelligibility experiment was conducted where sentence recognition rates increased from 3% to 17% when the talking head was present.

    • Speech and sounds are important sources of information in our everyday lives for communication with our environment, be it interacting with fellow humans or directing our attention to technical devices with sound signals. For hearing impaired persons this acoustic information must be supplemented or even replaced by cues using other senses. We believe that the most natural modality to use is the visual, since speech is fundamentally audiovisual and these two modalities are complementary. We are hence exploring how different visualization methods for speech and audio signals may support hearing impaired persons. The goal in this line of research is to allow the growing number of hearing impaired persons, children as well as the middle-aged and elderly, equal participation in communication. A number of visualization techniques are proposed and exemplified with applications for hearing impaired persons.

  • Antibodies to SARS-CoV-2 and risk of past or future sick leave

    • The extent that antibodies to SARS-CoV-2 may protect against future virus-associated disease is unknown. We invited all employees (n=15,300) at work at the Karolinska University Hospital, Stockholm, Sweden to participate in a study examining SARS-Cov-2 antibodies in relation to registered sick leave. For consenting 12,928 healthy hospital employees antibodies to SARS-CoV-2 could be determined and compared to participant sick leave records. Subjects with viral serum antibodies were not at excess risk for future sick leave (adjusted odds ratio (OR) controlling for age and sex: 0.85 [95% confidence interval (CI) (0.85 (0.43-1.68)]. By contrast, subjects with antibodies had an excess risk for sick leave in the weeks prior to testing [adjusted OR in multivariate analysis: 3.34 (2.98-3.74)]. Thus, presence of viral antibodies marks past disease and protection against excess risk of future disease. Knowledge of whether exposed subjects have had disease in the past or are at risk for future disease is essential for planning of control measures.Trial registration: First registered on 02/06/20, ClinicalTrials.gov NCT04411576.

    • Nationellt kompetenscentrum anhöriga (Nka), som är en del av Linnéuniversitetet, är en av aktörerna i det nationella utvecklingsuppdraget kring barn som anhöriga, som Socialstyrelsen är huvudman för (www.anhoriga.se). Nka arbetar som ett expertstöd för praktiker, beslutsfattare och andra aktörer inom området barn som anhöriga. Nka har ingått som ett av partnerländerna inom det Erasmus+ (KA2 Youth) finansierade projektet Care2Work tillsammans med the IARS International Institute (Storbritannien, projektkoordinator), the Family and ChildrenCare Centre (KMOP, Grekland) och Anziani e Non Solo (Italien). Projektet Care2Work handlar om unga omsorgsgivare med minoritets- och utländsk bakgrund inom Europa, och de hinder och möjligheter de kan uppleva inför framtida utbildning, träning och arbete (www.care2work.org). Under projekttiden har vi undersökt hur ett utbildningsstöd för unga omsorgsgivare (15-29 år) med minoritets- och utländsk bakgrund skulle kunna utformas. Utöver det har en kartläggning av befintliga verksamheter och utbildnings- eller stödfunktioner som direkt eller indirekt riktar sig till målgruppen genomförts.Den här rapporten är en sammanställning av de lärdomar och reflektioner som vuxit fram under projekttiden. Vi har inte funnit någon forskning i Sverige som direkt belyser den här gruppen utan har istället använt oss av forskning där målgruppen indirekt är berörd tillsammans med de studier vi själva genomfört. Det här gör det än viktigare att lärdomar från projektet används som material i utformandet av stöd till unga omsorgsgivare samt vägledning i de kunskapsluckor som finns inom området. Under projekttiden har de deltagande länderna bidragit i skapandet av en interaktiv webbplattform som innehåller utbildningsverktyg, hjälpmedel, bloggar, projektuppdateringar, events och nyheter. Webbplatsen (www.care2work.org) är speciellt designad för en bred målgrupp som inkluderar unga omsorgsgivare, ungdomsledare, forskare, akademiker, beslutsfattare och arbetsgivare. Den nya webbplattformen syftar till att öka medvetenheten om behoven hos unga omsorgsgivare och skapar en mötesplats för kreativa debatter bland olika berörda parter. Webbsidan erbjuder även en rad innovativa utbildningsverktyg, som riktar sig till både unga omsorgsgivare och professionella, och kan användas för att höja och framhäva unga omsorgsgivares kompetens och kunskap som de förvärvat genom anhörigomsorg och därmed ökat deras utbildningsmöjlighet och anställningsbarhet.
