  • Family burden and functional assessment in the Swedish CLIPS-study : Do staff and relatives agree on individuals with psychotic disorders' functional status?

    • PURPOSE:In this study, the individuals with psychotic disorders' daily life function was investigated on six dimensions considering their relations to family burden. Functional ratings carried out by relatives and staff were also compared.METHODS:Totally 88 relatives, to as many individuals diagnosed with a psychotic disorder, participated in this study. Relatives were to rate their own perceived burden and their ill relatives' physical functioning, personal care skills, interpersonal relationships, social acceptability, activities, and work skills. The outpatient staff, in all 24 case managers also rated the patients' functional level on the same assessment tool as did the relatives.RESULTS:Most dimensions of patients' everyday functioning were highly correlated to relatives' burden. The two functional dimensions "interpersonal relationships" and "activities" were best at classifying burden when rated by relatives. Also, comparing independent functional ratings by relatives and staff showed that despite great agreements on most dimensions, they differed significantly on "social acceptability" where relatives rated the function to be poorer and "activities" where relatives rated the function to be better.CONCLUSIONS:Relatives, who perceive individuals with a psychotic disorder to be limited in their ability to interpersonal relationships and limited in their ability to activate in everyday life, are more likely to perceive a higher burden. Even though staff and relatives mainly agree on the patients' functional ability, they often differ in ratings regarding patients' social acceptability and ability to activate in everyday life. This should be worth considering in clinical practice as well as in future research.

  • Making in Context : Reconsidering Anders Zorn's Oil Painting Practice

    • Anders Zorn is one of the most well-known Swedish artists of the late nineteenth century. Born 1860 in the Dalecarlian town of Mora, the artist’s works were renowned and sought-after during his lifetime, both at home and abroad. Ranging in motif from bathing nudes, cosmopolitan urban life and rural genre scenes, towards society portraiture, Zorn’s oeuvre – which is further comprised of a variety of different media such as watercolour, oil painting, etching and sculpture – provides a rich material record that serves to demonstrate the intricacies of his artistic process. The aim of the present thesis is to examine the details of Zorn’s materials and techniques, with specific focus on his oil painting practice. What materials did the artist use to construct his paintings? Did they change over time, or where they consistent throughout his career? What was Zorn’s typical approach towards compositional planning and paint application? Also, how does the artist’s practice relate to the techniques of his contemporaries, such as the French Impressionists or the international cohort of artists known as the juste milieu?In order to answer these questions the study draws upon the empirical methods of technical art history, using analytical techniques such as multispectral photography, x-radiography, x-ray fluorescence spectroscopy and paint sample analysis as a means of identifying the various stages of Zorn’s artistic process. In addition to examining the artist’s finished oil paintings, a broad range of both physical and written sources are also consulted, including his remnant studio materials, colourmen’s purchase orders and product catalogues, as well as press clippings of contemporary critics’ reviews. The information provided through these analyses and sources are examined by way of the theoretical concept of materiality, drawing particularly upon the writing of social anthropologists Tim Ingold and Daniel Miller, as well as art historian Michael Yonan. Using this conceptual framework, the study centers on the making of Zorn’s oil paintings, focusing on aspects such as the artist’s training, material conditions, exhibition and market contexts, as well as contemporary aesthetic debates that help explain the details of his practice. The thesis is divided into two parts; the first part examines the details of Zorn’s oil painting practice, and the second focuses on contextualising his technique in relation to his contemporary context. The first part documents specific details such as the artist’s particular choice of supports, grounds and colours. In addition, it discusses his preparatory process, which typically involved the use of pencil drawings and value studies in oil. Finally, the details of Zorn’s paint application are explored, focusing on aspects such as the artist’s use of an ébauche underlayer, his successive build up using shadows and highlights, as well as the particular qualities of his surface facture. Building upon this characterisation of his oil painting practice, part two then goes on to compare Zorn’s paintings with those of other late nineteenth-century painters, starting with a technical juxtaposition with the French Impressionists, followed by an overview of a select number of juste milieu painters, notably Édouard Manet, John Singer Sargent, Giovanni Boldini and Joaquín Sorolla. The remaining sections of part two are devoted to the broader nineteenth-century context, with specific reference to the aesthetic debates surrounding Charles Baudelaire’s concept of modernité (Fr. modernity), as well as Zorn’s particular exhibition and market setting.

  • Qigong : acute affective responses in a group of regular exercisers

    • Qigong is a Chinese mind-body therapy that aims to, through the use of movements, relaxed breathing and relaxation/meditation, create a healthy flow of life force, qi, in the body, and consequently improve health. A growing number of Qigong studies report beneficial effects on health and well-being. However, little attention has fo-cused on the acute affective responses that accompany single sessions of Qigong ex-ercise. The aim of the present thesis was therefore to study affective reactions to Qigong exercise. In Study I, the effects of Qigong exercise on mood and anxiety were compared to a control group. Results showed partial support for the superiority of Qigong exercise compared to controls. In Study II, different lengths of session time were compared, resulting in similar affective benefits for the 30 and 60-minute ses-sions. In Study III, affective responses were also assessed during the session, using mean scores and individual responses. Results showed an increase toward greater Activated and Deactivated Pleasantness during the session, with the greatest changes at the end of the bout. The majority of individuals reported increased Pleasantness during the Qigong session. Expectations of positive outcomes were significantly as-sociated with only few affective responses. Responses to open-ended questions of af-fective experiences displayed affective reactions mostly toward greater Deactivated Pleasantness. This thesis contributes to a greater understanding of the limited area of Qigong-related affective responses. For the exercisers, Qigong is associated with a greater momentary emotional state. However, due to the highly select group of regu-lar Qigong exercisers, generalizing the results outside the sample population is lim-ited. Theories on active mechanisms in the Qigong-affect relationship, and results from studies of affective responses to similar activities, suggest that other groups of people would also benefit affectively from Qigong exercise. Given the many benefits of positive affect, Qigong exercise may also pose great promises for the enhancement of other areas related to health and well-being. This calls for additional studies.

  • Activity and participation long term after stroke

    • Background and aim: As an increasing number of people are living and ageing with the consequences ofstroke, the rehabilitation and support provided for this group needs to have a long-term perspective. Moreover,to prevent long-term activity limitations and participation restrictions after stroke, increased knowledge is neededregarding how engagement in different types of activities develops beyond the first years of recovery andadaptation. As such engagement is complex and multifaceted it needs to be studied from different perspectivesand using well-defined methods of assessment.Thus, the overarching aim of this thesis was to contribute to animproved understanding of how activity and participation develops up to 15 years after stroke and of factors thatmay influence this process.Participants and methods: The first two sub-studies of the thesis are based on longitudinal follow-up data froma sample of 145 stroke survivors consecutively included in the Lund Stroke Register during a one-year periodand followed up after ten years with high retention rate. The second and third study builds on qualitative datafrom a sub-sample of the original participants, consisting of ten stroke survivors and four of their familiymembers, all interviewed 15 years after stroke.Methods include an ICF-oriented conceptual analyzis of the Barthel Index (BI) and the modified FrenchayActivities Index (mFAI), and descriptive analysis of activity performance ten years after stroke as assesd bythese two instruments. In addition, long-term predictors of the frequency of performing social and lesiureactivities ten years after stroke were identified through multiple regression analysis using follow-up data from 16months after the stroke. Finally, the subjective experiences of social and lesiure participation long-term afterstroke were explored using a grounded theory approch, focusing on personal and environmental conditions andindividual strategies.Main results and conclusions: The results show that the BI and the mFAI together cover 69 % of the ‘activitiesand participation’ component of the ICF core set for stroke and largely focus on different types of activities.Although most of the ten-year survivors were independent in self-care and basic mobility, performance of othertypes of activies varied substantially. In total, 23 % were rated as inactive, whereas the majority reported a highor moderate activity level ten years after stroke. The regression analyses resulted in a total of five significantpredictors. A wide social network, ability to walk a few hundred meters, and driving a car at 16 months after thestroke was related to a higher frequency of social and leisure activities after ten years, whereas an age >75years was related to a lower activity. The qualitative findings revealed that personal characteristics, access toactivities, social networks, motivation, and perceived capacity are central to engagement in social and leisureactivities after stroke. Furthermore, the strategies used to engage in such activities are based on individualgoals and contexts, and result from balancing different priorities in life and evaluating costs and rewards ofengagement. In conclusion, rehabilitation and support can be needed at different stages after stroke to meet thechanging needs of long-term stroke survivors, and should focus on supporting stroke survivors and their familiesto understand and handle the long-term consequences of stroke in all areas of life, including social and leisuredomains.

  • FACTORS ASSOCIATED WITH FATIGUE AMONG PEOPLE WHO HAVE RETURNED TO WORK AFTER STROKE : AN EXPLORATORY STUDY

    • Objective: To explore the associations between fatigue impact and (a) personal and stroke-related characteristics, (b) functional impairments and (c) work-related factors among individuals who have returned to work after stroke. Design: A cross-sectional exploratory study. Subjects: 87 working stroke survivors. Methods: This study comprises data from a pos-tal survey targeting work ability and perceived stroke-related consequences 1 year after stroke. Fatigue was evaluated using the Fatigue Severity Scale (FSS). Factors associated with having fatigue (FSS total score ≥ 4) were identified using univari-able and multivariable logistic regression analyses. Three domain-specific multivariable models and 1 final combined model were created. Results: Fatigue was reported by 43% of the parti-cipants. Several factors representing all the investi-gated domains were associated with fatigue. In the final combined regression model, self-perceived low cognitive functioning, low decision control at work and high quantitative job demands had the strongest independent effects on the odds of having fatigue. Conclusion: Among people who were working 1 year after stroke, fatigue was associated with both personal and stroke-related characteristics as well as functional impairments and work-related factors. This highlights the complex nature of post-stroke fatigue. Fatigue management interventions should have a comprehensive approach and also consider the work environment.

    • Objective: To assess the occurrence of self-reported fatigue among men and women who have returned to work after stroke, and the association between 2 fatigue rating scales. Design: A cross-sectional study. Subjects: A total of 91 adults (58 men/33 women, mean age 53 years) with mild to moderate disability. Methods: Questionnaires were posted to participants approximately one year after stroke. Fatigue was assessed with the Fatigue Severity Scale (FSS) and the Mental Fatigue Scale (MFS). Results: In total, 58% of the women and 33% of the men reported fatigue on the FSS (total score ≥ 4), and 46% of the women and 28% of the men reported mental fatigue on the MFS (total score ≥ 10.5). Being easily fatigued, decreased motivation, mental fatigability and sensitivity to stress were the most reported problems. FSS and MFS were moderately associated (rho 0.517–0.732). Conclusion: Fatigue is common among persons who have returned to work after stroke, and interferes with daily life. The long-term consequences of fatigue should be addressed after stroke, especially in women. The FSS and the MFS can be used in combination, as they provide information on different aspects of fatigue.
