  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

    • A simple single-phase material, a random close-packed (volume fraction 67%) ensemble of highly monodisperse bare maghemite (gamma-Fe2O3) nanoparticles, is shown to exhibit ideal superspin-glass behavior (mimicking that of model spin-glasses), namely, an unprecedentedly sharp onset of the absorption component of the ac susceptibility, narrow memory dips in the zero-field-cooled magnetization and a spin-glass characteristic field-dependence of the magnetic susceptibility. This ideal behavior is attributed to the remarkably narrow dispersion in particle size and to the highly dense and spatially homogeneous configuration ensured by the random close-packed arrangement. This material is argued to constitute the closest nanoparticle analogue to a conventional (atomic) magnetic state found to date.

  • Demagnetization effects in dense nanoparticle assemblies

    • We highlight the relevance of demagnetizing-field corrections in the characterization of dense magnetic nanoparticle assemblies. By an analysis that employs in-plane and out-of-plane magnetometry on cylindrical assemblies, we demonstrate the suitability of a simple analytical formula-based correction method. This allows us to identify artifacts of the demagnetizing field in temperature-dependent susceptibility curves (e.g., shoulder peaks in curves from a disordered assembly of essentially bare magnetic nanoparticles). The same analysis approach is shown to be a straightforward procedure for determining the magnetic nanoparticle packing fraction in dense, disordered assemblies.

  • A high-pressure structure in curium linked to magnetism

    • Curium ties at the center of the actinide series and has a half-filled shell. with seven 5f electrons spatially residing inside its radon core. As a function of pressure, curium exhibits five different crystallographic phases up to 100 gigapascals, of which all but one are also found in the preceding element, americium. We describe here a structure in curium, Cm III, with monoctinic symmetry, space group C2/c, found at intermediate pressures (between 37 and 56 gigapascals). Ab initio electronic structure calculations agree with the observed sequence of structures and establish that it is the spin polarization of curium's 5f electrons that stabilizes Cm Ill. The results reveal that curium is one of a few elements that has a lattice structure stabilized by magnetism.

  • Crossover From Individual to Collective Magnetism in Dense Nanoparticle Systems: Local Anisotropy Versus Dipolar Interactions

    • Dense systems of magnetic nanoparticles may exhibit dipolar collective behavior. However, two fundamental questions remain unsolved: i) whether the transition temperature may be affected by the particle anisotropy or it is essentially determined by the intensity of the interparticle dipolar interactions, and ii) what is the minimum ratio of dipole–dipole interaction (Edd) to nanoparticle anisotropy (KefV, anisotropy⋅volume) energies necessary to crossover from individual to collective behavior. A series of particle assemblies with similarly intense dipolar interactions but widely varying anisotropy is studied. The Kef is tuned through different degrees of cobalt-doping in maghemite nanoparticles, resulting in a variation of nearly an order of magnitude. All the bare particle compacts display collective behavior, except the one made with the highest anisotropy particles, which presents “marginal” features. Thus, a threshold of KefV/Edd ≈ 130 to suppress collective behavior is derived, in good agreement with Monte Carlo simulations. This translates into a crossover value of ≈1.7 for the easily accessible parameter TMAX(interacting)/TMAX(non-interacting) (ratio of the peak temperatures of the zero-field-cooled magnetization curves of interacting and dilute particle systems), which is successfully tested against the literature to predict the individual-like/collective behavior of any given interacting particle assembly comprising relatively uniform particles.
