  • Mode of hand training determines cortical reorganisation: A randomized controlled study in healthy adults

    • Objective: To evaluate two commonly used forms of hand training with respect to influence on dexterity and cortical reorganization. Subjects: Thirty healthy volunteers (mean age 24.2 years). Methods: The subjects were randomized to 25 min of shaping exercises or general activity training of the non-dominant hand. The dexterity and the cortical motor maps (number of excitable positions) of the abductor pollicis brevis muscle were evaluated pre- and post-training by the Purdue Peg Board test and transcranial magnetic stimulation, respectively. Results: After shaping exercises the dexterity increased significantly (p <= 0.005) for both hands, mostly so in the non-dominant hand. The cortical motor map of the abductor pollicis brevis muscle shifted forwardly into the pre-motor area without expanding. After general activity training, no significant improvements in dexterity were found for the non-dominant hand. The cortical motor map of the non-dominant abductor pollicis brevis muscle expanded significantly (p = 0.03) in the posterior (sensory) direction. Conclusion: These results indicate that shaping exercises, but not general activity training, increase dexterity of the trained non-dominant hand in parallel with a shift of location of active transcranial magnetic stimulation positions. Shifts of active cortical areas might be important for the interpretation of brain plasticity in common behavioural tasks.

  • Critical Friends : Health Professionals' Experiences of Collegial Feedback in a Clinical Setting

    • Introduction: A critical friend is a trusted person who asks provocative questions, provides data to be examined through another lens, and offers critique of a person's work as part of collegial feedback. However, empirical evidence presenting the use of collegial feedback to develop health professionals’ competence in clinical settings seems to be scarce. The aim of this study was to explore health professionals’ experiences of observing each other as critical friends in a clinical setting, as part of a continuous professional development initiative. Methods: The study was designed as a qualitative inductive study. Reflective journals written by health professionals (n=57) were analysed using thematic networks. The health professionals represented registered nurses and registered nurses with different specialist education (for example in paediatrics, mental health, intensive care and anaesthesiology), biomedical scientists, occupational therapists, physiotherapists and dental hygienists. Results: Health professionals can successfully use collegial feedback and benefit from critical friendships in clinical settings as it offers ample opportunities for reflection before, during and after the observation. A key finding was that in order to incorporate changes to professional practice, each individual needs to not only to act as a critical friend, but also experience being observed by a critical friend. Discussion: Based on the results of this study, it seems worthwhile to implement and further develop opportunities for health professionals to act as critical friends. We suggest that future research explore not only how professional competence develops over time, but also how it impacts on health related outcomes for patients.

  • Imaging of lipids in human adipose tissue by cluster ion TOF-SIMS

    • Biopsies of human subcutaneous adipose tissue were taken from healthy donors. Samples were high-pressure frozen, freeze-fractured, and freeze dried. Imaging mass spectrometry of samples was performed in a TOF-SIMS mass spectrometer equipped with a bismuth cluster ion source. Blood vessels, the connective tissue, and adipocytes can be seen in TOF-SIMS images. Blood vessels were found labeled by a high content of sodium ions and potassium ions in their lumen and phosphocholine signal in smooth muscle cells of the vessel wall. The connective tissue showed high signal levels of CN(-) fragments, derived from proteins and nucleic acids. Adipocytes showed high signal levels of phosphocholine and cholesterol ubiquitously in their membranes and diacylglycerols in some membrane sites. The central part of adipocytes showed high levels of triacylglycerols and fatty acids. These results are in accordance to those of biochemical studies; however, a precise spatial localization of lipids in adipocytes is demonstrated with MS imaging.

  • Targeting tumor cells based on Phosphodiesterase 3A expression

    • We and others have previously reported a correlation between high phosphodiesterase 3 A (PDE3A) expression and selective sensitivity to phosphodiesterase (PDE) inhibitors. This indicates that PDE3A could serve both as a drug target and a biomarker of sensitivity to PDE3 inhibition. In this report, we explored publicly available mRNA gene expression data to identify cell lines with different PDE3A expression. Cell lines with high PDE3A expression showed marked in vitro sensitivity to PDE inhibitors zardaverine and quazinone, when compared with those having low PDE3A expression. Immunofluorescence and immunohistochemical stainings were in agreement with PDE3A mRNA expression, providing suitable alternatives for biomarker analysis of clinical tissue specimens. Moreover, we here demonstrate that tumor cells from patients with ovarian carcinoma show great variability in PDE3A protein expression and that level of PDE3A expression is correlated with sensitivity to PDE inhibition. Finally, we demonstrate that PDE3A is highly expressed in subsets of patient tumor cell samples from different solid cancer diagnoses and expressed at exceptional levels in gastrointestinal stromal tumor (GIST) specimens. Importantly, vulnerability to PDE3 inhibitors has recently been associated with co-expression of PDE3A and Schlafen family member 12 (SLFN12). We here demonstrate that high expression of PDE3A in clinical specimens, at least on the mRNA level, seems to be frequently associated with high SLFIV12 expression. In conclusion, PDE3A seems to be both a promising biomarker and drug target for individualized drug treatment of various cancers.

  • Sorafenib and nitazoxanide disrupt mitochondrial function and inhibit regrowth capacity in three-dimensional models of hepatocellular and colorectal carcinoma

    • Quiescent cancer cells in malignant tumors can withstand cell-cycle active treatment and cause cancer spread and recurrence. Three-dimensional (3D) cancer cell models have led to the identification of oxidative phosphorylation (OXPHOS) as a context-dependent vulnerability. The limited treatment options for advanced hepatocellular carcinoma (HCC) and colorectal carcinoma (CRC) metastatic to the liver include the multikinase inhibitors sorafenib and regorafenib. Off-target effects of sorafenib and regorafenib are related to OXPHOS inhibition; however the importance of this feature to the effect on tumor cells has not been investigated in 3D models. We began by assessing global transcriptional responses in monolayer cell cultures, then moved on to multicellular tumor spheroids (MCTS) and tumoroids generated from a CRC patient. Cells were treated with chemotherapeutics, kinase inhibitors, and the OXPHOS inhibitors. Cells grown in 3D cultures were sensitive to the OXPHOS inhibitor nitazoxanide, sorafenib, and regorafenib and resistant to other multikinase inhibitors and chemotherapeutic drugs. Furthermore, nitazoxanide and sorafenib reduced viability, regrowth potential and inhibited mitochondrial membrane potential in an additive manner at clinically relevant concentrations. This study demonstrates that the OXPHOS inhibition caused by sorafenib and regorafenib parallels 3D activity and can be further investigated for new combination strategies.

  • Preceptors' conceptions of a peer learning model : A phenomenographic study

    • Background: During the past few years nurses precepting undergraduate nursing students have been put under greater pressure because of increased number of students admitted to the universities combinedwith a shortage of clinical placements. One solution is the preceptor model peer learning where two students are tutored by the same preceptor simultaneously. Objectives: The aim of this study was to describe the variation of registered nurses' conceptions of preceptorship in a peer learning model for undergraduate nursing students. Design: The study used a qualitative descriptive design and a phenomenographic approach. Settings: The interviews took place at somatic and psychiatric units at two different hospitals in southern Sweden. Participants: Twelve informants participated who had worked as registered nurses between 1–17 years and acted as peer learning preceptors between 2 month and 6 years. Methods: Each nurse was interviewed individually using a semi structured interview guide. Follow up questions were used to make the informants develop and deepen their answers. Results: Four different descriptive categories emerged in the study: 1) Preceptorship in peer learning generates development and new perspectives 2) Preceptorship in peer learning enables student reflection and independence 3) Preceptorship in peer learning engenders insufficiency and stress 4) Preceptorship in peer learning requires education and support. Conclusions: The result of this study showed that preceptors conceived that peer learning enabled them to take a step backwhich gave thema new role and perspectives. The consequencewas that the students could be more independent which saved time for some of the preceptors. However, some preceptors perceived insufficiency and stress while working with two students. It is also important to educate both students and preceptors to optimise the use of peer learning.

    • In this case study the benefit of using analysis of conversational interaction in clinical assessment of communication is explored. A diagnostic test revealed a severe global aphasia in a man. However, according to a questionnaire filled out by the wife, her husband had some functional communication. The description of the consequences of aphasia in this man was further modified and enriched by analyses of conversational interaction between the man and his spouse. The Measure of Participation in Conversation and the Measure of Skill in Supported Conversation show that the couple has strategies that facilitate communication. Activity-based Communication Analysis provides information about how the strategies are used and about the influence of different factors brought into the interaction by the situation and the individuals involved. Thus, although time consuming, analysis of natural conversations provide valid information that is of importance in assessment of communication and implementation of intervention that can have an impact on everyday life in persons with aphasia and their conversation partners.

  • Predictors of preparedness for recovery following colorectal cancer surgery: a latent class trajectory analysis.

    • With an interest in providing knowledge for person-centred care, our overall goal is to contribute a greater understanding of diversity among patients in terms of their preparedness before and up to six months after colorectal cancer surgery. Our aim was to describe and provide a tentative explanation for differences in preparedness trajectory profiles.The study was explorative and used prospective longitudinal data from a previously published intervention study evaluating person-centred information and communication. The project was conducted at three hospitals in Sweden. Patient-reported outcomes measures, including the Longitudinal Preparedness for Colorectal Cancer Surgery Questionnaire, were collected before surgery, at discharge, and four to six weeks, three months, and six months after surgery. Clinical data were retrospectively obtained from patients' medical records. We used latent class growth models (LCGMs) to identify latent classes that distinguish subgroups of patients who represent different preparedness trajectory profiles. To determine the most plausible number of latent classes, we considered statistical information about model fit and clinical practice relevance. We used multivariable regression models to identify variables that explain the latent classes.The sample (N=488) comprised people with a mean age of 68years (SD = 11) of which 44% were women. Regarding diagnoses, 60% had colon cancer and 40% rectal cancer. The LCGMs identified six latent classes with different preparedness for surgery and recovery trajectories. The latent classes were predominantly explained by differences in age, sex, physical classification based on comorbidities, treatment hospital, global health status, distress, and sense of coherence (comprehensibility and meaningfulness).Contrary to the received view that emphasizes standardized care practices, our results point to the need for adding person-centred and tailored approaches that consider individual differences in how patients are prepared before and during the recovery period related to colorectal cancer surgery.

  • Validity of Three Survey Questions for Self-Assessed Sedentary Time

    • Time spent in sedentary behavior (SB) has increased during the last decades. Accurate assessments are of importance when studying health consequences of SB. This study aimed to assess concurrent validity between three different questions for self-reported sitting and thigh worn accelerometer data. In total, 86 participants wore the ActivPAL accelerometer during three separate weeks, assessing sitting time with different questions each week. The questions used were Katzmarzyk, GIH stationary single-item question (SED-GIH), and a modified version of the singleitem from IPAQ short form. In total 64, 57, and 55 participants provided valid accelerometer and questionnaire data at each time-point, respectively, and were included for analysis. Spearman and Pearson correlation was used to assess the validity. The three questions, Katzmarzyk, SED-GIH, and a modified question from IPAQ all showed a weak non-significant correlation to ActivPAL with r-values of 0.26, 0.25, and 0.19 respectively. For Katzmarzyk and SED-GIH, 50% and 37% reported correctly, respectively. For the modified IPAQ, 53% over-reported and 47% under-reported their sitting time. In line with previous research, our study shows poor validity for self-reported sitting-time. For future research, the use of sensor-based data on SB are of high importance.
