  • Supporting Well-Being in Gender-Diverse People : A Tutorial for Implementing Conceptual and Practical Shifts Toward Culturally Responsive, Person-Centered Care in Speech-Language Pathology

    • Purpose: Gender dysphoria is commonly conceptualized as a mental disorder in gender-diverse people who do not identify with the gender assigned to them at birth. Direct support for well-being tends to be delegated to the field of mental health (MH), whereas speech-language pathology (SLP) practice is charged with modifying gender-diverse people's voice and communication in the belief that well-being will improve as a byproduct. However, with the introduction of the minority stress model, gender dysphoria is now understood as the result of sociocultural processes of stigmatization, pathologization, coping, and resilience, and it is to be addressed by all professions providing transgender health services. The purposes of this tutorial are to examine practices in SLP in light of the current conceptualization of gender dysphoria and guide speech-language pathologists in their role in supporting the well-being of gender-diverse people.Method: We reviewed the SLP and MH literature in the topic area to compare the two disciplines' conceptualizations and approaches to professional support for gender-diverse people.Results: We propose a transdisciplinary, person-centered, and culturally responsive approach to SLP practice that directly attends to minority stress, microaggressions, coping skills, and resilience factors.Conclusions: It is not sufficient for speech-language pathologists to delegate support for well-being in gender-diverse people to MH practitioners. Rather, speech-language pathologists need to be proactive in taking responsibility for supporting their clients' well-being based on each individual clinician's knowledge, skills, and capacity to do so. We recommend addressing barriers and facilitators of gender-diverse people's well-being both within SLP as a professional culture and by adapting the clinician's own professional practice.

  • Picturing inclusive places in segregated spaces : a participatory photo project conducted by migrant women in Sweden

    • This article uses participatory photography to explore contradictory processes of inclusion and exclusion in contemporary Sweden. Our aim is to analyse the social relations that shape the kinds of places recently arrived migrant women experience as ‘safe’, as well as their everyday experiences of inclusion and exclusion. The use of photography – wherein the women choose how, when and where to shoot photos – helps us highlight what otherwise would not be immediately evident with regard to the experience of such places. We argue that there are inclusive places in segregated spaces, and that issues of ethnic inclusion and exclusion are linked to ethnic hegemony and other relationships of power. Drawing on theories of relational space in general, and transgressive space in particular, we demonstrate that our informants' daily existence is simultaneously integrated and segregated, included and excluded, and that emancipatory processes that are already under way must be allowed to proceed if the social landscape of integration is to be an open and equal one.

  • Parents’ Experiences of Direct and Indirect Implications of Sleep Quality on the Health of Children with ADHD : A Qualitative Study

    • Sleep problems represent a significant challenge for children with ADHD. However, lack of knowledge about how sleep affects children with ADHD in terms of their health and everyday life prevents the development and implementation of interventions to promote sleep. The aim of this study was to explore parents’ experiences of direct and indirect implications of sleep quality onthe health of children with ADHD. The study used an abductive qualitative design, with Tengland’s two-dimensional theory of health as a deductive analysis framework. Semi-structured interviews were conducted with 21 parents of children aged 6–13 with ADHD and sleep problems. The parents experienced that sleep influenced their children’s abilities to control emotional behaviour relatedto ADHD and to manage everyday life. Sleep also had an impact on the children’s well-being,in relation to both vitality and self-esteem. In conclusion, the results show important direct and indirect implications of sleep quality on the health of children with ADHD. This implies a need forgreater focus on sleep, to target both abilities and well-being in promoting health among childrenwith ADHD. © 2022 by the authors

  • Ruralt flyktingmottagande – En (ny) strategi för mindre kommuners överlevnad?

    • Syftet med denna rapport är att belysa förutsättningarna, utmaningarna och möjligheterna för flyktingmottagande och integrationsarbete på mindre orter, med särskilt fokus på glesbygdskommuner i norra Sverige. Tills nyligen har Sverige haft en relativt generös migrationspolitik och de svenska kommunerna har haft en nyckelroll i mottagandet och integrationen av flyktingar. Många små, glesbefolkade landsbygdskommuner har tagit emot en hög andel individer i förhållande till sin befolkningsstorlek. Flyktingmottagande har setts som en möjlighet att utveckla kommunen. Många av de små kommuner i norra Sverige som står inför en minskande befolkning har sett mottagandet som en möjlighet till sysselsättning, genererad av de uppgifter som en tillströmning av flyktingar innebär, men också som en grund för att utveckla den kommunala verksamheten. Kommunernas flyktingmottagande är dock beroende av politiken på nationell nivå och förändringar i denna politik som påverkar antalet flyktingar som kommunerna kan ta emot. När politikerna beslutar att strama åt flyktingpolitiken och stänga ned verksamheter som kommit att utgöra ett hopp inför framtiden, utvecklas en känsla av maktlöshet. Besluten om antal och kvoter fattas någon annanstans, och detta bidrar till en process där små glesbygdskommuner hamnar ännu längre bort från maktens centrum. Att ta emot flyktingar har för dessa kommuner, åtminstone inledningsvis, varit en strategi som gett hopp för framtiden, inte bara för de flyktingar som tas emot i dessa små samhällen, utan också för de små kommunerna själva. Den lilla orten blir således perifer genom att den marginaliseras i såväl politiska som ekonomiska processer och alltså inte (enbart) på grund av sin rumsliga position.Rapporten är organiserad utifrån forskningsprojektets tre delstudier. Delstudierna baseras på olika material och besvarar olika frågor. Den första handlar om hur integration som fenomen beskrivs i ansökningar om projektmedel från Länsstyrelserna och vilka problem som kommunerna beskriver som särskilt centrala att adressera. Den andra delstudien fokuserar på de sätt på vilka flyktingmottagande framställs i lokala och regionala nyhetsmedier i Västerbotten och Västernorrland. Den tredje och sista delstudien fokuserar på erfarenheter och vardagliga arbetsvillkor för flyktingmottagare och integrationsarbetare i två mindre orter. I en sammantagen analys av resultatet från de tre delstudierna identifieras fyra mer övergripande berättelser varigenom flyktingmottagande i glesbygd skildras 1) engagemang och frustration, 2) förutsättningarna för mottagandet och organiseringens betydelse, 3) integration och svenskhet och 4) periferiseringsprocesser.

  • Cost-effectiveness of occupational therapy return-to-work interventions for people with mental health disorders : a systematic review

    • Background: Return-to-work (RTW) resources for persons with mental health disorders are limited and costs are typically shared by several stakeholders in society. Occupational therapists (OT) provide RTW interventions for this target group, however, increased knowledge of health, and employment effects, as well as costs are needed to better inform decision makers in their prioritisations.Aims/objectives: To identify and summarise evidence of cost-effectiveness of RTW interventions for persons with mental health disorders which OTs provide.Materials and methods: A systematic search was applied and resulted in 358 articles. After screening, nine articles met inclusion criteria and were reviewed. Quality assessment was conducted using the economic evaluation tool by Joanna Briggs Institute.Results: Supported employment, Individual Placement and Support was cost-effective in several contexts while three studies showed larger effects and higher costs. An OT intervention added to treatment for major depression was indicated to be cost-beneficial and an advanced supported employment was cost-saving. The methodological quality varied considerably between studies.Conclusions and significance: The results of the included studies are promising, however, to further strengthen the economic perspective in OT RTW interventions, the need for conducting more and methodologically robust economic evaluations is crucial in future studies.

  • Potential sources of contamination on textiles and hard surfaces identified as high-touch sites near the patient environment

    • The hospital environment represents an important mediator for the transmission of healthcare-associated infections through direct and indirect hand contact with hard surfaces and textiles. In this study, bacteria on high-touch sites, including textiles and hard surfaces in two care wards in Sweden, were identified using microbiological culture methods and 16S rDNA sequencing. During a cross-sectional study, 176 high-touch hard surfaces and textiles were identified and further analysed using microbiological culture for quantification of total aerobic bacteria, Staphylococcus aureus, Clostridium difficile and Enterobacteriacae. The bacterial population structures were further analysed in 26 samples using 16S rDNA sequencing. The study showed a higher frequency of unique direct hand-textile contacts (36 per hour), compared to hard surfaces (2.2 per hour). Hard surfaces met the recommended standard of ≤ 5 CFU/cm2 for aerobic bacteria and ≤ 1 CFU/cm2 for S. aureus (53% and 35%, respectively) to a higher extent compared to textiles (19% and 30%, respectively) (P = 0.0488). The number of bacterial genera was higher on textiles than on the hard surfaces. Staphylococcus (30.4%) and Corynebacterium (10.9%) were the most representative genera for textiles and Streptococcus (13.3%) for hard surfaces. The fact that a big percentage of the textiles did not fulfil the criteria for cleanliness, combined with the higher bacterial diversity, compared to hard surfaces, are indicators that textiles were bacterial reservoirs and potential risk vectors for bacterial transmission. However, since most of the bacteria found in the study belonged to the normal flora, it was not possible to draw conclusions of textiles and hard surfaces as sources of healthcare associated infections. 2023 Nygren et al.

  • Age determination of plutonium using inductively coupled plasma mass spectrometry

    • The age of plutonium is defined as the time since the last separation of the plutonium isotopes from their daughter nuclides. In this paper, a method for age determination based on analysis of 241Pu/241Am and 240Pu/236Pu using ICP-SFMS is described. Separation of Pu and Am was performed using a solid phase extraction procedure including UTEVA, TEVA, TRU and Ln-resins. The procedure provided separation factors adequate for this purpose. Age determinations were performed on two plutonium reference solutions from the Institute for Reference Materials and Measurements, IRMM081 (239Pu) and IRMM083 (240Pu), on sediment from the Marshall Islands (reference material IAEA367) and on soil from the Trinity test site (Trinitite). The measured ages based on the 241Am/241Pu ratio corresponded well with the time since the last parent-daughter separations of all the materials. The ages derived from the 236U/240Pu ratio were in agreement for the IRMM materials, but for IAEA367 the determination of 236U was interfered by tailing from 238U, and for Trinitite the determined age was biased due to formation of 236U in the detonation of the "Gadget".

  • Determination of actinides using ICP-SFMS

    • Interest in the determination of the actinides using ICP-MS has steadily increased with the development of systems capable of more sensitive and precise measurements. However, the analysis of less abundant actinides such as Pu and Am is not straightforward due to the need for chemical separation of these elements prior to determination. In many applications of mass- spectrometric actinide analysis, isotope ratio measurements are important, either for the analysis of the isotopic composition of, e.g., U or Pu in the sample, or for quantitative determinations using isotope dilution mass spectrometry. In order to achieve high precision and accuracy in an isotope ratio measurement, corrections for instrumentally induced systematic errors, e.g., due to dead-time and mass bias, need to be considered. In this thesis, different aspects of actinide analysis using ICP-SFMS have been addressed. In Papers I and III, separation procedures based on solid phase extraction for Pu, Am and U were developed and evaluated with respect to chemical yield and separation from elements causing spectral interferences. Applications of the analytical procedures developed comprised measurement of the 240Pu/239Pu ratio in environmental reference materials (Paper I), and age determination of Pu based on the 241Pu/241Am and 240Pu/236U ratios (Paper III). In the application of different separation procedures for Pu (Paper I), previously unidentified spectral interferences were discovered. In Paper II, these interferences were identified as lanthanide phosphate ions and the composition and formation of these species with respect to different instrumental parameters were further examined. Due to the importance of precise and accurate isotope ratio determination, a thorough investigation of the instrumental dead time of an ICP-SFMS system was performed. The dead time was evaluated via both isotope ratio and electronic measurements of the output from the detector amplifier. It was found that the overall uncertainty in ratio measurements can be significantly reduced by using an electronically determined dead time due to the inherently higher precision compared to traditional isotope ratio based approaches. However, this requires that an correct artificial dead-time is implemented in the pulse counting system (Paper IV).
