  • A Distinctive Adnexal (Usually Paratubal) Neoplasm Often Associated with Peutz-Jeghers Syndrome and Characterized by STK11 Alterations (STK11 Adnexal Tumor) : A Report of 22 Cases

    • We describe 22 examples of a novel, usually paratubal, adnexal tumor associated with Peutz-Jeghers syndrome in nearly 50% of cases that harbored STK11 alterations in all tested (n=21). The patients ranged from 17 to 66 years (median=39 y) and the tumors from 4.5 to 25.5 cm (median=11 cm). Most (n=18) were paratubal, with metastases noted in 11/22 (50%) and recurrences in 12/15 (80%). Morphologically, they were characterized by interanastomosing cords and trabeculae of predominantly epithelioid cells, set in a variably prominent myxoid to focally edematous stroma, that often merged to form tubular, cystic, cribriform, and microacinar formations, reminiscent of salivary gland-type tumors. The tumor cells were uniformly atypical, often with prominent nucleoli and a variable mitotic index (median=9/10 HPFs). The tumors were usually positive to a variable extent for epithelial (CAM5.2, AE1/AE3, cytokeratin 7), sex cord (calretinin, inhibin, WT1), and mesothelial (calretinin, D2-40) markers, as well as hormone receptors. PAX8, SF1, and GATA-3 were rarely positive, while claudin-4, FOXL2, and TTF-1 were consistently negative. All sequenced tumors (n=21) harbored alterations in STK11, often with a loss of heterozygosity event. There were no other recurrently mutated genes. Recurrent copy number alterations included loss of 1p and 11q, and gain of 1q, 15q, and 15p. Despite an extensive morphologic, immunohistochemical, and molecular evaluation, we are unable to determine with certainty the histogenesis of this unique tumor. Wolffian, sex cord stromal, epithelial, and mesothelial origins were considered. We propose the term STK11 adnexal tumor to describe this novel entity and emphasize the importance of genetic counseling in these patients as a significant number of neoplasms occur in association with Peutz-Jeghers syndrome.

    • Currently one of the main challenges in CO2 storage research is the development, testing and validation of accurate and efficient Measuring, Monitoring and Verification (MMV) techniques to be deployed at geological sequestration sites that are cost effective yet help minimize risk. This perspective motivated PETROBRAS, the National Oil Major in Brazil, through its R&D investments portfolio to prioritize research projects that would contribute to decreasing the technological gap in the area. The Company's periodic surveys indicated the lack of infrastructure, as well as expertise in CO2 MMV, as two of the most critical issues at the national level. In order to bridge that gap, initial steps were taken in 2010 for the start-up and development of the first CO2 MMV Field Lab in Brazil, fully sponsored by PETROBRAS, with a long term goal of enabling the ranking of the best, most cost-effective MMV technology alternatives to be deployed at commercial large scale CCGS sites scheduled to be installed in the country. In addition to providing basic infrastructure to carry out the CO2 injection and controlled release experiments, the facility was designed for the simultaneous testing of multiple measuring methodologies. Additional benefits of the initiative are the creation of expertise and the acceleration of the know-how in MMV in Brazil, as well as the development of a deeper and more practical knowledge of CO2 dynamics and impacts in a real world, open air scenario. Under the full support of the PETROBRAS R&D Center (CENPES), through its Climate Change Mitigation Technological Program (PROCLIMA), the Brazilian Pilot CO2 MMV Lab was made possible through a joint 4-year research Project, conceived and carried out by PETROBRAS and local academia in Brazil, in close cooperation with international experts. An overview of the Project and the multiple research areas encompassed will be presented, together with the preliminary results of the first CO2 injection campaign, which took place in 2013.   • Nonadiabaticity of cavity-free neutral nitrogen lasing

    • We report on a theoretical study of cavity-free lasing of neutral nitrogen molecules in femtosecond laser filaments with a nonadiabatic Maxwell-Bloch model, compared with recent pump-seed experiments. The nonadiabaticity of the lasing process is revealed and it is found that electron-neutral collisions dominate the dipole dephasing rate. Moreover, we show that the asymmetry between forward and backward lasing not only depends on the different amplification lengths but also on the temporal dynamics of electron-neutral collisions. The comparison of the nonadiabatic model with simulations based on the adiabatic approximation (such as radiative transfer equations) explicitly sets a bound on the validity of the latter model for cavity-free nitrogen lasing phenomenon, which holds a unique potential in optical remote sensing applications.

  • The patient perspective : Quality of life in advanced heart failure with frequent hospitalisations

    • End of life is an unfortunate but inevitable phase of the heart failure patients' journey. It is often preceded by a stage in the progression of heart failure defined as advanced heart failure, and characterised by poor quality of life and frequent hospitalisations. In clinical practice, the efficacy of treatments for advanced heart failure is often assessed by parameters such as clinical status, haemodynamics, neurohormonal status, and echo/MRI indices. From the patients' perspective, however, quality-of-life-related parameters, such as functional capacity, exercise performance, psychological status, and frequency of re-hospitalisations, are more significant. The effects of therapies and interventions on these parameters are, however, underrepresented in clinical trials targeted to assess advanced heart failure treatment efficacy, and data are overall scarce. This is possibly due to a non-universal definition of the quality-of-life-related endpoints, and to the difficult standardisation of the data collection. These uncertainties also lead to difficulties in handling trade-off decisions between quality of life and survival by patients, families and healthcare providers. A panel of 34 experts in the field of cardiology and intensive cardiac care from 21 countries around the world convened for reviewing the existing data on quality-of-life in patients with advanced heart failure, discussing and reaching a consensus on the validity and significance of quality-of-life assessment methods. Gaps in routine care and research, which should be addressed, were identified. Finally, published data on the effects of current i.v. vasoactive therapies such as inotropes, inodilators, and vasodilators on quality-of-life in advanced heart failure patients were analysed.

  • Assessing urban water demand-side management policies before their implementation : An agent-based model approach

    • In the context of climate change and increasing water scarcity, adopting water demand-side management (DSM) policies has become necessary. This study advocates for utilizing agent-based modelling (ABM) as a robust simulation tool to assess the impact of nonprice (nudges) and price (changes in increasing block tariff) measures on urban water use. Overcoming challenges posed by insufficient high-quality data, the research integrates four sociocognitive profiles and diverse household income levels to reflect the variability in DSM policy effectiveness based on socioeconomic characteristics. Through 125 simulated scenarios combining various increasing block tariffs with nonpricing measures, the study reveals an average monthly demand reduction ranging from 8.1% to 15.6%. Significantly, nonprice measures prove more effective in curbing water use than pricing measures, attributed to the prioritization of environmental concerns in conservation efforts. Higher-income households exhibit less-pronounced reductions in water consumption. Emphasizing the reliability of ABM for ex ante evaluations of DSM policies, this research underscores the importance of a balanced approach, incorporating both nonprice and price-based measures, to effectively address water scarcity challenges.
