    • Background. An abnormally high [above mean + 2 standard deviations (SD)] umbilical artery (UA) pulsatility index (PI) indicates impaired fetal outcome, whereas the impact of an "abnormally" low (below mean -2 SD) PI is unknown. Methods. Perinatal outcome was compared between cases with a UA PI less than mean -2 SD (group A: high-risk cases selected from a database, n = 330; group B: unselected cases, n = 39) and unselected controls (group C) with a PI within mean ± 2 SD (n = 863) at Doppler velocimetry. Groups B and C were retrieved from a population-based sample. The unpaired t-test, Mann-Whitney U-test, chi2-test and Fisher's exact probability test were used for statistical comparisons with a two-tailed p < 0.05 being significant. Results. No significant differences were found between group A vs. group C and group B vs. group C regarding perinatal mortality, Apgar scores at 1, 5 or 10 min, or arterial or venous cord blood pH. Postterm pregnancy in group A carried no additional risk. For obvious reasons, operative delivery and neonatal intensive care were more common in group A than in group C, but no such differences were found between groups B and C. The mean birthweight was 3.7% higher in group B than in group C (p = 0.049). Conclusions. Deeming a UA PI below the lower reference limit as "abnormally" low is a statistical definition that was not reflected by a biological imperfection. Instead, a low UA PI promoted fetal growth.

  • Hemithyroidectomy, does the indication influence the outcome?

    • PurposeHemithyroidectomies are mainly performed for two indications, either therapeutically to relieve compression symptoms or diagnostically for suspicious nodule(s). In case of the latter, one could consider the approach to be rather extensive since the majority of patients have no symptoms and will have benign disease. The aim of this study is to investigate the complication rates of diagnostic hemithyroidectomy and to compare it with the complication rates of compressive symptoms hemithyroidectomy.MethodsData from patients who had undergone hemithyroidectomy either for compression symptoms or for excluding malignancy were extracted from a well-established Scandinavian quality register (SQRTPA). The following complications were analyzed: bleedings, wound infections, and paresis of the recurrent laryngeal nerve (RLN). Risk factors for these complications were examined by univariable and multivariable logistic regression.ResultsA total of 9677 patients were included, 3871 (40%) underwent surgery to exclude malignancy and 5806 (60%) due to compression symptoms. In the multivariable analysis, the totally excised thyroid weight was an independent risk factor for bleeding. Permanent (6-12 months after the operation) RLN paresis were less common in the excluding malignancy group (p = 0.03).ConclusionA range of factors interfere and contribute to bleeding, wound infections, and RLN paresis after hemithyroidectomy. In this observational study based on a Scandinavian quality register, the indication "excluding malignancy" for hemithyroidectomy is associated with less permanent RLN paresis than the indication "compression symptoms." Thus, patients undergoing diagnostic hemithyroidectomy can be reassured that this procedure is a safe surgical procedure and does not entail an unjustified risk.

  • To read what not is written. Report from a learning study with inferences as a learning object in an upper secondary school.

    • An important component in reading comprehension is the ability to make inferences, what sometimes also is described as reading between the lines. The ability can be said to be three-folded, as inferences can be made from an individuals' experiences and thoughts, from the text itself and from other texts. On an upper-secondary school a group of teachers (n= 16) teaching in Swedish found through screening that their students (16 years old) were comfortable with making inferences using their own experiences and thoughts, but less skilled in making inferences by using clues in the text. The teachers chose as a learning-object the ability to localize clues in the given information and make inferences in the text to be able to read between lines. The aim was to develop the students awareness of that everything in a text not is explicitly written out and to discern values in words. Students should by finding the clues and reading between and outside the lines be able to perceive the main message in the text clearer. The group of teachers conducted a learning study-cycle with three lessons using contrast as the main pattern of variation. Each lesson was jointly planned and then performed for a new group. This presentation is a report from the learning study.

  • "Being a girl has become a public health problem!" : Feminist organizations both resisting and reproducing postfeminist mental health discourses

    • An apparent decline in youth mental health in the Nordic countries has received a lot of attention over the last decade, and young women have been described as a particularly vulnerable group. To combat the problem, political debate has centred on solutions at the individual level, despite significant evidence of the associations between structural factors and adverse mental health. In this article, I explore how two feminist support organizations working for young women in Sweden (tjejjourer) frame and address girls' mental health as a problem of feminist concern. Interviews with staff, organizational documents, websites, social media, and other communications were analysed following Snow and Benford's theory on collective action frames, to identify the organizations' problem diagnoses, solutions, and motivational frames for action. The results show that the organizations both reiterate and resist a postfeminist discourse on mental health. There is notable friction between structural problem diagnoses, based on intersectional analyses of power, and political demands and support oriented towards the individual. In conclusion, I argue that this friction may be a result of the organizations' adoption of discourses on risk, vulnerability, and mental health itself, but also that the complex relationship between structural critique and individual care is indispensable for feminist organizing.

  • Sisterhood at a distance : doing feminist support work online

    • The aim of this study is to analyze the characteristics of feminist peer support in the context of online chat counseling. Based on 15 interviews with female lay supporters associated with a branch of the Swedish women's shelter movement targeting young women, we explore how the digital setting—characterized by distance and anonymity—affects the meaning and doing of feminist support. Our results show that core principles of feminist support—striving for equality and trust, the crafting of safe spaces, and sharing experiences—are all renegotiated and/or accentuated by the digital setting. The chat is experienced as enabling a more equal relationship and a high level of safety. The meaning of safety has largely shifted, however, from being associated with a feminist community to safety associated with solitude and distance. We further show a tension in the respondents’ understanding of shared experiences, stressing both the importance of situated knowledges and the value of not knowing who is seeking or offering support. By combining research and material on feminist support and online youth counseling, the article offers novel perspectives on feminist counseling and social work, the power dimensions of online counseling, and the virtual space as an arena for feminist activism.

  • Ecosystem change and stability over multiple decades in the Swedish subarctic : complex processes and multiple drivers

    • The subarctic environment of northernmost Sweden has changed over the past century, particularly elements of climate and cryosphere. This paper presents a unique geo-referenced record of environmental and ecosystem observations from the area since 1913. Abiotic changes have been substantial. Vegetation changes include not only increases in growth and range extension but also counterintuitive decreases, and stability: all three possible responses. Changes in species composition within the major plant communities have ranged between almost no changes to almost a 50 per cent increase in the number of species. Changes in plant species abundance also vary with particularly large increases in trees and shrubs (up to 600%). There has been an increase in abundance of aspen and large changes in other plant communities responding to wetland area increases resulting from permafrost thaw. Populations of herbivores have responded to varying management practices and climate regimes, particularly changing snow conditions. While it is difficult to generalize and scale-up the site-specific changes in ecosystems, this very site-specificity, combined with projections of change, is of immediate relevance to local stakeholders who need to adapt to new opportunities and to respond to challenges. Furthermore, the relatively small area and its unique datasets are a microcosm of the complexity of Arctic landscapes in transition that remains to be documented.
