  • A new 3 mm band receiver for the Onsala 20 m antenna

    • A new receiver for the Onsala 20 m antenna with the possibility of being equipped with 3 mm and 4 mm bands has been built and the 3 mm channel has been commissioned during the Spring 2014. For single-dish operation, the receiver uses an innovative on-source/off-source optical switch. In combination with additional optical components and within the same optical layout, the switch provides two calibration loads (for the 3 mm and 4 mm channels), sideband rejection measurement, and tuning possibilities. The optical layout of the receiver employs all cold (4 K) offset elliptical mirrors for both channels, whereas the on-off switch employs flat mirrors only. The 3 mm channel employs a sideband separation (2SB) dual polarization receiver with orthomode transducer (OMT), 4-8 GHz intermediate frequency (IF), x? 2pol x? upper and lower sidebands (USB? +? LSB). The cryostat has four optical windows made of high density polyethylene (HDPE) with anti-reflection corrugations, two for the signal and two for each frequency band cold load. The cryostat uses a two-stage cryocooler produced by Sumitomo HI? RDK? 408D2 with anti-vibration suspension of the cold-head to minimize impact of the vibrations on the receiver stability. The local oscillator (LO) system is based on a Gunn oscillator with aphase lock loop (PLL) and four mechanical tuners for broadband operation, providing independently tunable LO power for each polarization. This paper provides a technical description of the receiver and its technology and could be useful for instrumentation engineers and observers using the Onsala 20 m telescope.

  • Effect of preoperative injection of superparamagnetic iron oxide particles on rates of sentinel lymph node dissection in women undergoing surgery for ductal carcinoma in situ (SentiNot study)

    • Background: One-fifth of patients with a preoperative diagnosis of ductal carcinoma in situ (DCIS) have invasive breast cancer (IBC) on definitive histology. Sentinel lymph node dissection (SLND) is performed in almost half of women having surgery for DCIS in Sweden. The aim of the present study was to try to minimize unnecessary SLND by injecting superparamagnetic iron oxide (SPIO) nanoparticles at the time of primary breast surgery, enabling SLND to be performed later, if IBC is found in the primary specimen. Methods: Women with DCIS at high risk for the presence of invasion undergoing breast conservation, and patients with DCIS undergoing mastectomy were included. The primary outcome was whether this technique could reduce SLND. Secondary outcomes were number of SLNDs avoided, detection rate and procedure-related costs. Results: This was a preplanned interim analysis of 189 procedures. IBC was found in 47 and a secondary SLND was performed in 41 women. Thus, 78.3 per cent of patients avoided SLND (P<0.001). At reoperation, SPIO plus blue dye outperformed isotope and blue dye in detection of the sentinel node (40 of 40 versus 26 of 40 women; P<0.001). Costs were reduced by a mean of 24.5 per cent in women without IBC (3990 versus 5286; P<0.001). Conclusion: Marking the sentinel node with SPIO in women having surgery for DCIS was effective at avoiding unnecessary SLND in this study. Registration number: ISRCTN18430240 (http://www.isrctn.com).

    • Background and purpose: Recommendations for regional radiotherapy (RT) of sentinel lymph node (SLN)-positive breast cancer are debated. We here report a RT quality assessment of the SENOMAC trial.Materials and Methods: The SENOMAC trial randomized clinically node-negative breast cancer patients with 1–2 SLN macrometastases to completion axillary lymph node dissection (cALND) or SLN biopsy only between 2015–2021. Adjuvant RT followed national guidelines. RT plans for patients included in Sweden and Denmark until June 2019 were collected (N = 1176) and compared to case report forms (CRF). Dose to level I (N = 270) and the humeral head (N = 321) was analyzed in detail.Results: CRF-data and RT plans agreed in 99.3 % (breast/chest wall) and in 96.6 % of patients (regional RT). Congruence for whether level I was an intended RT target was lower (78 %). In accordance with Danish national guidelines, level I was more often an intended target in the SLN biopsy only arm (N = 334/611, 55 %,) than in the cALND arm (N = 174/565, 31 %,). When an intended target, level I received prescribed dose to 100 % (IQR 98–100 %) of the volume. However, even when not an intended target, full dose was delivered to > 80 % of level I (IQR 75–90 %). The intentional inclusion of level I in the target volume more than doubled the dose received by ≥ 50 % of the humeral head.Conclusion: Congruence between CRF data and RT plans was excellent. Level I received a high dose coverage even when not intentionally included in the target. Including level I in target significantly increased dose to the humeral head.

  • Disease burden and unmet need for acute allergic reactions - A patient perspective

    • Background: Acute allergic reactions (AARs) occur shortly after exposure to an allergen, and the severity is on a continuum. Systemic corticosteroids (CS) are mainstay treatment of moderate to severe AARs, whereas those at risk of the most severe AARs (ie, anaphylaxis) are also recommended prescription of epinephrine autoinjectors. There is limited research on the impact of AARs not fulfilling the criteria for anaphylaxis. We have characterized a sample with a history of moderate to severe AARs and evaluated their self-reported disease burden (ie, daily life impact, anxiety, and treatment impediments).Methods: Survey study of adults with experience of AARs treated with CS. Participants recruited from a web-based panel and using social media were asked to complete a questionnaire related to their allergy and experience of AARs. The results were summarized for the whole sample and across subgroups with and without prescription of epinephrine.Results: The final study sample included 387 participants (80% women, mean age 41), of which 129 (33%) had at some point been prescribed epinephrine. The most common symptoms were respiratory (80%) and skin (78%) manifestations, and the mean (standard deviation, SD) self-rated severity score (scale from 0 [very mild] to 10 [very severe]) of the most recent AAR was 6.1 (2.0). More than 80% had experience of AARs interrupting daily activities and 50% of AARs that had limited work/studies or participation in leisure activities. Most of the respondents reported some degree of anxiety related to AARs and 43% had feared for their lives. Moreover, difficulties swallowing allergy medicine at an AAR was experienced by 26% and not having the medicine available when needed by 66%. Participants with prescription of epinephrine experienced more severe AARs than those without such prescription (mean [SD] severity 6.8 [2.1] vs 5.8 [1.8], p < 0.0001); however, also those without epinephrine prescription reported considerable anxiety and impact on daily life and to a similar degree as those with prescription.Conclusions: In this sample, subjects with experience of AARs treated with CS showed a considerable disease burden with anxiety and interruption on daily life, as well as problems related to access to, and swallowing of, medication. Although respondents with epinephrine prescription had more severe disease, a high disease burden was also evident among those without epinephrine. The study increases the knowledge of people with moderate to severe AARs, a patient population that has previously been underrepresented in the research literature.

    • The emphasis on higher education (HE) in today’s knowledge society has increasedthe amount of students enrolled in universities in Sweden. Two percentof these students are people with disabilities, of whom more than half are studentswith dyslexia. Dyslexia is a disability that involves difficulty with writtentext which is the essence of HE. The disability affects these students’ readingand my assumption was therefore that their possibility to complete their educationon an equal footing with their peers was limited.The overall aim of this thesis is to illuminate the prerequisites for participationfor Swedish students with dyslexia in higher education, and to analyse if support,and information about support, offered by higher-education institutionsare accessible to them. Due to lack of previous research, this is a descriptivestudy using the theoretical framework of cultural-historical activity theory(CHAT), a theory that can be summarized in five principles: an activity systemis the unit of analysis, it has historicity, multi-voicedness, it views contradictionsas sources of change, and it has the potential for expansive transformation.To orient myself within the large field of HE and dyslexia, the following questionshave guided my work: What HE measures are taken to ensure accessibilityand participation for Swedish students with dyslexia? What tensions, as definedwithin CHAT, may be found in the HE activities in relation to studentswith dyslexia? How do students with dyslexia regard the support offered? Inorder to answer these questions, I have listened to some of the voices withinHE, by collecting data through a questionnaire sent to all coordinators for studentswith disabilities; by having focus group interviews with university teachers,as well as having individual interviews with students with dyslexia. Thedata were collected and analysed within a CHAT activity system, which depictedHE activities by coordinators and teachers in relation to students withdyslexia. The graph was used in the final discussion to formulate some ideas forexpansive transformation.

  • Modelling and simulation of building components : thermal interaction between multilayer wall and hydronic radiator

    • Background and Scope: The scope of this thesis is to investigate the thermal behaviour of building components as hydronic radiator and multilayer walls subjected to dynamic conditions. The modelling and simulation of these building components provide information on how these components thermally interact among each other. The thermal interaction is fundamental to know how the energy is used in buildings. In particular, the thermal energy used in rooms can be expressed as the efficiencies for emission in a space heating system. This thesis analyzes the efficiencies for emission of a space heating system equipped with hydronic radiator for Swedish buildings by providing a comprehensive and detailed approach on this topic.Methodology: The methods used in this thesis are: experiment, modelling of multilayer wall and hydronic radiator, the dynamic simulation of the building and the efficiencies for emission of a space heating system. Here, the experiment, known as step response test, shows the heating up process of a hydronic radiator. The observation of the qualitative measurements suggests the most suitable technique of modelling the radiator known as transient modelling with multiple storage elements. The multilayer wall has been discretized both in space and time variable with a Finite Difference Method. Dynamic simulation of the building provides the efficiencies for emission of a space heating system.Findings: The experimental results show how the radiator performs the charging phase. The performance of the transient model is compared with lumped steady state models in terms of temperature of exhaust flow and total heat emitted. Results of the dynamic simulation show how buildings located in a Northern climate use the energy in a better way than Southern climates in Sweden. Heavy active thermal mass provides higher efficiencies for emission than light thermal mass. Radiators with connection pipes located on the same side react faster at the thermodynamic changing of the mass flow rate by providing higher efficiencies for emission than radiators with connection pipes located on the opposite side.Conclusion and Outlook: This thesis increases the knowledge about the modelling and simulation of hydronic radiators and multilayer walls. More research is needed on this topic to encompass modelling details of building components often ignored. The modelling and simulation of building components are the key to understand how building components thermally interact with each other. The thermal interaction among building components is a fundamental parameter for the assessment of efficiencies of emission of the space heating system. In the near future, the concept of efficiencies of emission can be implemented in National Building Code, therefore, this study provides guidelines on how to assess these efficiencies.

I anslutning till Boverkets regeringsuppdrag (N2014/75/E) att definiera nya energihus¬håll¬ningskrav, finns det all anledning att diskutera om vi är på rätt väg med de krav på bygg¬naders energi¬prestanda som ställs i Boverkets Byggregler (BBR). Speciellt om dess uppfyllelse ska vara juridiskt bindande.
