  • 5-Year Outcomes of PCI Guided by Measurement of Instantaneous Wave-Free Ratio Versus Fractional Flow Reserve

    • BACKGROUND: Instantaneous wave-free ratio (iFR) is a coronary physiology index used to assess the severity of coronary artery stenosis to guide revascularization. iFR has previously demonstrated noninferior short-term outcome compared to fractional flow reserve (FFR), but data on longer-term outcome have been lacking.OBJECTIVES: The purpose of this study was to investigate the prespecified 5-year follow-up of the primary composite outcome of all-cause mortality, myocardial infarction, and unplanned revascularization of the iFR-SWEDEHEART trial comparing iFR vs FFR in patients with chronic and acute coronary syndromes.METHODS: iFR-SWEDEHEART was a multicenter, controlled, open-label, registry-based randomized clinical trial using the Swedish Coronary Angiography and Angioplasty Registry for enrollment. A total of 2,037 patients were randomized to undergo revascularization guided by iFR or FFR.RESULTS: No patients were lost to follow-up. At 5 years, the rate of the primary composite endpoint was 21.5% in the iFR group and 19.9% in the FFR group (HR: 1.09; 95% CI: 0.90-1.33). The rates of all-cause death (9.4% vs 7.9%; HR: 1.20; 95% CI: 0.89-1.62), nonfatal myocardial infarction (5.7% vs 5.8%; HR: 1.00; 95% CI: 0.70-1.44), and unplanned revascularization (11.6% vs 11.3%; HR: 1.02; 95% CI: 0.79-1.32) were also not different between the 2 groups. The outcomes were consistent across prespecified subgroups.CONCLUSIONS: In patients with chronic or acute coronary syndromes, an iFR-guided revascularization strategy was associated with no difference in the 5-year composite outcome of death, myocardial infarction, and unplanned revascularization compared with an FFR-guided revascularization strategy.     • Breast tumors from BRCA1 germ line mutation carriers typically exhibit features of the basal-like molecular subtype. However, the specific genes recurrently mutated as a consequence of BRCA1 dysfunction have not been fully elucidated. In this study, we utilized gene expression profiling to molecularly subtype 577 breast tumors, including 72 breast tumors from BRCA1/2 mutation carriers. Focusing on the RB1 locus, we analyzed 33 BRCA1-mutated, 36 BRCA2-mutated and 48 non-BRCA1/2-mutated breast tumors using a custom-designed high-density oligomicroarray covering the RB1 gene. We found a strong association between the basal-like subtype and BRCA1-mutated breast tumors and the luminal B subtype and BRCA2-mutated breast tumors. RB1 was identified as a major target for genomic disruption in tumors arising in BRCA1 mutation carriers and in sporadic tumors with BRCA1 promoter-methylation, but rarely in other breast cancers. Homozygous deletions, intragenic breaks, or microdeletions were found in 33% of BRCA1-mutant tumors, 36% of BRCA1 promoter-methylated basal-like tumors, 13% of non-BRCA1 deficient basal-like tumors, and 3% of BRCA2-mutated tumors. In conclusion, RB1 was frequently inactivated by gross gene disruption in BRCA1-related hereditary breast cancer and BRCA1-methylated sporadic basal-like breast cancer, but rarely in BRCA2-hereditary breast cancer and non-BRCA1-deficient sporadic breast cancers. Together, our findings demonstrate the existence of genetic heterogeneity within the basal-like breast cancer subtype that is based upon BRCA1-status.

  • Characterization of pulp with high enzymatic hydrolyzability

    • Conversion of biomass to biofuels is currently an area that attracts large interest, and lignocellulosic biomass offers the abundance and environmental attributes that can potentially support large-scale biofuel production as an alternative to petroleum-based transportation fuel.In a recent project, Innventia has developed wood based pulps optimized for conversion to biofuels. These novel pulps were produced to target a high level of enzymatic hydrolyzability. To assess the hydrolyzability of these pulps, a laboratory protocol has been established usingan enzyme mixture containing Celluclast 1.5L and Novozyme 188 with an activity of 10 FPU/g pulp (Andersen 2007). Results obtained using this protocol are assumed to be relevant for industrial conditions. In addition to assessment of the produced pulps, the results havebeen compared to commercial cellulose substrates and pulps of a variety of grades.Furthermore, supramolecular properties – specific surface area and average pore size – were determined by an in-house method utilizing solid state nuclear magnetic resonance (Larsson et al. 2013). Kappa numbers, limiting viscosities, ISO-brightness and carbohydrate compositions were determined using standard methods. Molecular mass distributions of cellulose tricarbanilates were determined by size exclusion chromatography with tetrahydrofuran mobile phase (Drechsler et al. 2000).The presentation will discuss the influence of chemical, macromolecular and supramolecular properties of commercial and novel pulp grades on the enzymatic hydrolyzability. Theprotocol used to assess of enzymatic hydrolyzability will be proposed as a benchmark test.

    • Conversion of biomass to biofuels and other products is a research area that is currently attracting a great amount of interest, particularly because such production may be envisaged as a key part of any bio-based economy. Lignocellulosic biomass is abundant and sustainable, and can therefore potentially support large-scale production of biofuel as an alternative to petroleum-based fuel.The enzymatic hydrolysability of three industrial pulps, five lab made pulps, and one microcrystalline cellulose powder was assessed using commercial cellulolytic enzymes. To gain insight into the factors that influence the hydrolysability, a thorough characterization of the samples was done, including their chemical properties (cellulose content, hemicellulose content, lignin content, and kappa number), their macromolecular properties (peak molar mass, number-average molar mass, weight-average molar mass, polydispersity, and limiting viscosity) and their supramolecular properties (fibre saturation point, specific surface area, average pore size, and crystallinity). The hydrolysability was assessed by determination of initial conversion rate and final conversion yield, with conversion yield defined as the amount of glucose in solution per unit of glucose in the substrate. Multivariate data analysis revealed that for the investigated samples the conversion of cellulose to glucose was mainly dependent on the supramolecular properties, such as specific surface area and average pore size. The molar mass distribution, the crystallinity, and the lignin content of the pulps had no significant effect on the hydrolysability of the investigated samples.In addition, experiments were carried out aiming at identifying suitable conditions for pre-treatment of wheat straw, for the purpose of making cellulose rich pulps with improved enzymatic reactivity. Two sets of conditions for pre-treatment of wheat straw were identified; a combination of low temperature alkaline washing and acid pre-hydrolysis, or high temperature acid pre-hydrolysis. Both bleached wheat straw pulps showed similar enzymatic reactivity. However, the enzymatic reactivity of both bleached wheat straw pulps was found to be significantly less than what has been achieved for wood pulps. A probable explanation for the low enzymatic reactivity of the bleached wheat straw pulp can be the small pore size, limiting the access for enzymes to the cellulose surfaces in the fibre wall interior.Text, figures and tables in an extended abstract (< 4 pages with title and references).

    • The adipocyte-derived hormone adiponectin regulates glucose and lipid metabolism and influences the risk for developing obesity, type 2 diabetes, and cardiovascular disease. Adiponectin binds to two different seven-transmembrane domain receptors termed AdipoR1 and AdipoR2. To study the physiological importance of these receptors, AdipoR1 gene knockout mice (AdipoR1−/−) and AdipoR2 gene knockout mice (AdipoR2−/−) were generated. AdipoR1−/− mice showed increased adiposity associated with decreased glucose tolerance, spontaneous locomotor activity, and energy expenditure. However, AdipoR2−/− mice were lean and resistant to high-fat diet–induced obesity associated with improved glucose tolerance and higher spontaneous locomotor activity and energy expenditure and reduced plasma cholesterol levels. Thus, AdipoR1 and AdipoR2 are clearly involved in energy metabolism but have opposing effects.

    • In a politician-decided top-down implementation of digital tools into the school curricula, chemistry education researchers and teacher educators try to develop relevant and meaningful digital tools possible to use to increase students’ learning. To exemplify and explore the impact of digital tools on students’ learning processes, two chemistry education projects are discussed in this presentation. When are digital tools applicable to enhance learning and how should teachers embed and frame this application of the digital tools? The projects present how Virtual Reality (VR), Augmented Reality (AR) and gamification can be used to enhance students’ perceived interest and value.

    • Today there is a digital boom within education, in Sweden all school curricula are re-written where use of digital tools has been added as mandatory in all subject syllabi from the autumn of 2018. This has made teachers, as well as educational researchers, interested to find relevant digital tools where students enhance their learning, not only finding them fun and exciting. In this presentation, the role of technologies as Virtual Reality (VR), Augmented Reality (AR) and gamification is explored to study how students learn chemistry, both regarding affective as well as cognitive aspects of learning [1]. Students’ perceived interest and value are studied using Krapp and Prenzel’s framework of interest [2] and Wenger and colleagues framework of value creation in communities and networks [3]. Part of the interest in VR technology has to do with the availability when a smartphone can be converted to a VR headset at a very low cost. AR technology is much more complicated and expensive, however, the multiple sensory modalities makes it interesting [4]. Gamification in the classroom, where application of game-design in learning processes, has recently attracted a lot of attention [5]. The main aim with gamification is to enhance students’ internal motivation through for example clues and possibilities to “level-up”.To explore how digital tools influence students learning, we will present two projects. The first is a university organic chemistry course where students practice their spatial competence using VR as a tool to visualise stereochemistry. The students study stereoisomers (for example simple molecules as 2-chlorobutane and more complex stereoisomers as muscarine and nicotine) and we have studied their perceived interest and value of the digital tools using a survey, interviews and observations. In the second project, engineering students have developed a teaching module for upper secondary chemistry using gamification, VR and AR as a way to motivate school students to learn about the protein synthesis. Here, we have conducted a survey and interviews with both the engineering students who developed the module, as well with the school students and teachers who have used the module. In the presentation, possibilities and challenges with the digital tools will be discussed, and examples for practice will be demonstrated.

    • At Swedish upper secondary school, all students have to pass a mandatory course with thename “Gymnasiearbete”. This course is different from all other courses due to severalreasons, for example, being a project work related only to the programme orientation andwithout an explicit course curriculum. In this project, we have followed students from theNatural Science Programme taking this course, and studied their interest, engagement andepistemic beliefs. Through observations, interviews, and questionnaires, we have foundtriggers important to emphasise to make students more engaged and interested to enhancetheir knowledge. We will, from both a teacher and researcher perspective, discuss this courserelated to both affective and cognitive variables.
