    • Although the consequences of hospital ED crowding have been studied extensively, the causes of crowding are still not well understood. Throughput factors in ED crowding models are difficult to study in a controlled fashion in a dynamic environment where healthcare demand changes rapidly, and physical and human resources suddenly become limited. Opportunities for automated, simultaneous, and low-cost observation of the location and movement of multiple units, patients and staff have recently arisen with the introduction of small, non-intrusive real-time location systems (RTLS). One such RTLS deployment reported here has initiated renewed consideration of quality and industrial statistics as applied to healthcare operations management. Novel metrics for essential constructs of throughput factors in ED crowding such as efficiency and effectiveness are proposed. In particular, causality is explained in terms of understanding of each construct, modeled in terms of entropy, information, and order. Experimental demonstration is given of how labor reduction and the probability of patients, personnel and equipment meeting in terms of less uncertainty can be explained. These novel metrics are expected to facilitate monitoring of how an ED reacts to different levels of crowding, provide insight into crowding dynamics, help evaluate interventions to decrease crowding, and ultimately improve care. © 2021 The Author(s).

    • Introduction: Twelve-lead electrocardiogram (ECG) is used to screen for hypertrophic cardiomyopathy (HCM), but up to 25% of HCM patients do not have distinctly abnormal ECGs, whereas up to 5% to 15% of healthy athletes do. We hypothesized that an approximately 5-minute resting advanced 12-lead ECG test ("A-ECG score") could detect HCM with greater sensitivity than pooled conventional ECG criteria and distinguish healthy athletes from HCM with greater specificity. Materials and methods: Five-minute 12-lead ECGs were obtained from 56 HCM patients, 56 age/sex-matched healthy controls, and 69 younger endurance-trained athletes. Electrocardiograms were analyzed using recently suggested pooled conventional ECG criteria and also A-ECG scoring techniques that considered results from multiple advanced and conventional ECG parameters. Results: Compared with pooled criteria from the strictly conventional ECG, an A-ECG logistic score incorporating results from just 3 advanced ECG parameters (spatial QRS-T angle, unexplained portion of QT variability, and T-wave principal component analysis ratio) increased the sensitivity of ECG for identifying HCM from 89% (78%-96%) to 98% (89%-100%; P = .025), while increasing specificity from 90% (83%-94%) to 95% (92%-99%; P = .020). Conclusions: Resting 12-lead A-ECG scores that are simultaneously more sensitive than pooled conventional ECG criteria for detecting HCM and more specific for distinguishing healthy athletes and other healthy controls from HCM can be constructed. Pending further prospective validation, such scores may lead to improved ECG-based screening for HCM. Published by Elsevier Inc.

  • Virtual implantations to transition from porcine to bovine animal models for a total artificial heart

    • Realheart total artificial heart (TAH) is a novel, pulsatile, four-chamber total artificial heart which had been successfully tested acutely in a porcine animal model. However, the bovine model is better suited for long-term testing and thus an evaluation of how the design would fit the bovine anatomy was required. Virtual implantation is a method that enables a computer simulated implantation based on anatomical 3D-models created from computer tomography images. This method is used clinically, but not yet adopted for animal studies. Herein, we evaluated its suitability in the redesign of the outer dimensions and vessel connections of Realheart TAH to transition from the porcine to the bovine animal model. Virtual implantations in combination with bovine cadaver studies enabled a series of successful acute bovine implantations. Virtual implantations are a useful tool to replace the use of animals in early device development and refine subsequent necessary in vivo experiments. The next steps are to carry out human virtual implantations and cadaver studies to ensure the design is optimized for all stages of testing as well as the final recipient.
