  • Surgical site infection after gastrointestinal surgery in children: an international, multicentre, prospective cohort study

    • Surgical site infection (SSI) is one of the most common healthcare-associated infections (HAIs). However, there is a lack of data available about SSI in children worldwide, especially from low-income and middle-income countries. This study aimed to estimate the incidence of SSI in children and associations between SSI and morbidity across human development settings.MethodsA multicentre, international, prospective, validated cohort study of children aged under 16 years undergoing clean-contaminated, contaminated or dirty gastrointestinal surgery. Any hospital in the world providing paediatric surgery was eligible to contribute data between January and July 2016. The primary outcome was the incidence of SSI by 30 days. Relationships between explanatory variables and SSI were examined using multilevel logistic regression. Countries were stratified into high development, middle development and low development groups using the United Nations Human Development Index (HDI).ResultsOf 1159 children across 181 hospitals in 51 countries, 523 (45·1%) children were from high HDI, 397 (34·2%) from middle HDI and 239 (20·6%) from low HDI countries. The 30-day SSI rate was 6.3% (33/523) in high HDI, 12·8% (51/397) in middle HDI and 24·7% (59/239) in low HDI countries. SSI was associated with higher incidence of 30-day mortality, intervention, organ-space infection and other HAIs, with the highest rates seen in low HDI countries. Median length of stay in patients who had an SSI was longer (7.0 days), compared with 3.0 days in patients who did not have an SSI. Use of laparoscopy was associated with significantly lower SSI rates, even after accounting for HDI.ConclusionThe odds of SSI in children is nearly four times greater in low HDI compared with high HDI countries. Policies to reduce SSI should be prioritised as part of the wider global agenda.

  • The SPARC water vapour assessment II: Profile-to-profile comparisons of stratospheric and lower mesospheric water vapour data sets obtained from satellites

Within the framework of the second SPARC (Stratosphere-troposphere Processes And their Role in Climate) water vapour assessment (WAVAS-II), profile-to-profile comparisons of stratospheric and lower mesospheric water vapour were performed by considering 33 data sets derived from satellite observations of 15 different instruments. These comparisons aimed to provide a picture of the typical biases and drifts in the observational database and to identify data-set-specific problems. The observational database typically exhibits the largest biases below 70 hPa, both in absolute and relative terms. The smallest biases are often found between 50 and 5 hPa. Typically, they range from 0.25 to 0.5 ppmv (5 % to 10 %) in this altitude region, based on the 50 % percentile over the different comparison results. Higher up, the biases increase with altitude overall but this general behaviour is accompanied by considerable variations. Characteristic values vary between 0.3 and 1 ppmv (4 % to 20 %). Obvious data-set-specific bias issues are found for a number of data sets. In our work we performed a drift analysis for data sets overlapping for a period of at least 36 months. This assessment shows a wide range of drifts among the different data sets that are statistically significant at the 2 σ uncertainty level. In general, the smallest drifts are found in the altitude range between about 30 and 10 hPa. Histograms considering results from all altitudes indicate the largest occurrence for drifts between 0.05 and 0.3 ppmv decade-1. Comparisons of our drift estimates to those derived from comparisons of zonal mean time series only exhibit statistically significant differences in slightly more than 3 % of the comparisons. Hence, drift estimates from profile-to-profile and zonal mean time series comparisons are largely interchangeable. As for the biases, a number of data sets exhibit prominent drift issues. In our analyses we found that the large number of MIPAS data sets included in the assessment affects our general results as well as the bias summaries we provide for the individual data sets. This is because these data sets exhibit a relative similarity with respect to the remaining data sets, despite the fact that they are based on different measurement modes and different processors implementing different retrieval choices. Because of that, we have by default considered an aggregation of the comparison results obtained from MIPAS data sets. Results without this aggregation are provided on multiple occasions to characterise the effects due to the numerous MIPAS data sets. Among other effects, they cause a reduction of the typical biases in the observational database.

  • Impact of temperature field inhomogeneities on the retrieval of atmospheric species from MIPAS IR limb emission spectra

    • We examine volume mixing ratios (vmr) retrieved from limb emission spectra recorded with the Michelson Interferometer for Passive Atmospheric Sounding (MIPAS). In level 2 (L2) data products of three different retrieval processors, which perform one dimensional (1-D) retrievals, we find significant differences between species' profiles from ascending and descending orbit parts. The relative differences vary systematically with time of the year, latitude, and altitude. In the lower stratosphere their monthly means can reach maxima of 20% for CFC-11, CFC-12, HNO3, H2O, 10% for CH4 and N2O. Relative differences between monthly means of 1-D retrieval results and of the true atmospheric state can be expected to reach half of these percentage values, while relative differences in single vmr profiles might well exceed those numbers. Often there are no physical or chemical reasons for these differences, so they are an indicator for a problem in the data processing. The differences are generally largest at locations where the meridional temperature gradient of the atmosphere is strong. On the contrary, when performing the retrieval with a tomographic two dimensional (2-D) retrieval, L2 products generally do not show these differences. This implies that inhomogeneities in the temperature field, and possibly in the species' fields, which are accounted for in the 2-D algorithm and not in standard 1-D processors, may cause significant deviations in the results. Inclusion of an externally given adequate temperature gradient in the forward model of a 1-D processor helps to reduce the observed differences. However, only the full tomographic approach is suitable to resolve the horizontal inhomogeneities. Implications for the use of the 1-D data, e.g. for validation, are discussed. The dependence of the ascending/descending differences on the observation strategy suggests that this problem is to be expected to affect in general 1-D retrievals of infrared limb sounders, if the line of sight of the instrument has a significant component in the direction of the horizontal temperature variation.

    • Various flavours of a new research field on (socio-)physical or personal analytics have emerged, with the goal of deriving semanticallyrich insights from people’s low-level physical sensing combined with their (online) social interactions. In this paper, we argue for more comprehensive data sources, including environmental and application-specific data, to better capture the interactions between users and their context, in addition to those among users. We provide some example use cases and present our ongoing work towards a synergistic analytics platform: a testbed based on mobile crowdsensing and IoT, a data model for representing the different sources of data and their connections, and a prediction engine for analyzing the data and producing insights.

    • The current work focuses on two specific issues concerning grid-connected solar PV units, i.e. the fault ride-through capability, also called low voltage ride-through capability, and the voltage support function through reactive power injection during faults. With the first one the PV unit can actually provide some limited grid support, whereas with a defined reactive power characteristic it can give a complete dynamic grid support. These two requirements, already known for wind power generation but new for the PV, have been recently introduced in the German technical guidelines for connection to the MV grid. Scope of the paper is to implement these requirements in a large solar PV plant, modeled in DIgSILENT PowerFactory, in order to understand its operation, and to evaluate its behavior and impact on the grid, in terms of stability and voltage support during grid fault.
