    • This paper presents a framework of ICT solutions developed in the EU research project DIADEM that supports environmental management with an enhanced capacity to assess population exposure and health risks, to alert relevant groups and to organize efficient response. The emphasis is on advanced solutions which are economically feasible and maximally exploit the existing communication, computing and sensing resources. This approach enables efficient situation assessment in complex environmental management problems by exploiting relevant information obtained from citizens via the standard communication infrastructure as well as heterogeneous data acquired through dedicated sensing systems. This is achieved through a combination of (i) advanced approaches to gas detection and gas distribution modelling, (ii) a novel service-oriented approach supporting seamless integration of human-based and automated reasoning processes in large-scale collaborative sense making processes and (iii) solutions combining Multi-Criteria Decision Analysis, Scenario-Based Reasoning and advanced human-machine interfaces. This paper presents the basic principles of the DIADEM solutions, explains how different techniques are combined to a coherent decision support system and briefly discusses evaluation principles and activities in the DIADEM project.

    • eHealth indicator and benchmarking activities are rapidly increasing nationally and internationally. The work is rarely based on a transparent methodology for indicator definition. This article describes first results of testing an indicator methodology for defining eHealth indicators, which was reported at the Medical Informatics Europe conference in 2012. The core elements of the methodology are illustrated, demonstrating validation of each of them in the context of Nordic eHealth Indicator work. Validation proved the importance of conducting each of the steps of the methodology, with several scientific as well as practical outcomes. The article is based on a report to be published by the Nordic Council of Ministers.

    • The report presents results of the Nordic eHealth Research Network, a subgroup of the eNordic Council of Ministers Health group. The network defined and collected data for altogether 49 common eHealth indicators from the Nordic Countries. Health information was quite comprehensively electronically available from other organisations in all the Nordic countries by end of 2014. Intensity of use of nationally stored data remained low except in Denmark. Sweden had best availability of the Patient portal functionalities. Patients used patient portal functionalities rarely except in Denmark. Doctors in Iceland had most positive experiences of their health information systems. Even with limitations, the current work presents a solid basis for working towards the goal of the eHealth network: generating comparable information to support development of Nordic welfare.

The core competence concept describes how large companies can prosper and achieve competitive advantage. It is a well-known concept in the strategy domain, frequently practiced in organizations and cited in the literature. Despite this, recent research indicates that in-depth understanding of the concept is scarce. In this thesis, a few shortcomings of the concept are recognized: The vagueness of the original definition, divergence of the existing literature, and invalid appliance. Straightforward core competence identification and verification processes are put forward, applicable to both research and practice. The processes aim at advancing beyond mere identification. Three different core competencies were identified and verified in the empirical part of this thesis. The core competencies were possible to separate through differences in the various influences from competencies, capabilities and resources. The core competencies were also possible to arrange in a complexity continuum, from a compound and highly integrated core competence to a simple and technology-based one, with the third one, a service-based core competence, representing moderate complexity. The two most complex core competencies were, in general, influenced by competencies and capabilities to a larger extent than the simpler core competence. Resources mainly influenced the goods-producing core competencies. The empirical findings of this thesis show that by acknowledging the specifics of the characteristics of competencies, capabilities and resources and their influences, in conceptual and empirical discussions and applications, core competencies can be identified and verified. Furthermore, their influences provide the keys to managing core competencies. A new core competence agenda is proposed. The agenda includes core competence management, which is of particular value to organizations in need of change, for example those facing dynamic business environments, where vital activities and processes – including core competence – must be continually renewed. The agenda is feasible for future core competence research as well, since it alters the research domain, and advocates specificity, convergence and validity.

  • A1Meighted Composite of Endodontic Inflammatory Disease is Linked to a First Myocardial Infarction

    • Purpose: To explore a weighted composite of endodontic inflammatory disease (ED) as a risk factor for suffering a first myocardial infarction (MI).Materials and Methods: Seven tooth-specific conditions related to EID were assessed radiographically in 797 patients suffering a first MI and 796 controls. A weighted composite of EID was calculated as the sum of all teeth, excluding third molars. Using maximum likelihood estimation, each condition was assigned a specific weight. Wth multivariable condi-tional regression, EIDvariables, periodontal disease, and missing teeth were assessed as predictors of a first MLResults: Periodontal disease (OR 1.38; 95% CI 1.13-1.69, p =0.0016) and missing teeth (OR 1.03; 95% CI 1.002-1.05, p =0.034) were related to the risk of a first MI, while none of the ED-related conditions individually were. However, when assessed as an aggregate, a weighted composite of ED (OR 1.97; 95% CI 1.23-3.17, p =0.0050) and periodontal disease (OR 1.34; 95% CI 1.09-1.63, p =0.0046) was associated with the risk of ML Missing teeth did not remain a statistically sig-nificant predictor of MI in the final model.Condusions: A weighted composite of ED was associated with the risk of MI and strengthens the evidence for a direst connection between oral inflammatory diseases and cardiovascular disorders.
