- Using electronic health records data and machine learning to guide future decisions needs to address challenges, including 1) long/short-term dependencies and 2) interactions between diseases and interventions. Bidirectional transformers have effectively addressed the first challenge. Here we tackled the latter challenge by masking one source (e.g., ICD10 codes) and training the transformer to predict it using other sources (e.g., ATC codes).

- Modern information and communication technologies make it easier for individuals to be involved in their own health and social care. They also facilitate contact between individuals and service providers and deliver more efficient tools for healthcare staff. Artificial Intelligence (AI) promises to bring even more benefits in the future, with more effectiveness and the provision of decision support.This book presents the proceedings of the 33rd Medical Informatics Europe Conference, MIE2023, held in Gothenburg, Sweden, from 22 to 25 May 2023. The theme of MIE2023 was ‘Caring is Sharing – Exploiting Value in Data for Health and Innovation’, stressing the increasing importance of sharing digital-health data and the related challenges. The sharing of health data is developing rapidly, both in Europe and beyond, so the focus of the conference was on the enabling of trustworthy sharing of data to improve health. Topics covered include healthcare, community care, self-care, public health, and the innovation and development of future-proof digital-health solutions, and the almost 300 papers divided into 10 chapters also cover important advances in the sub domains of biomedical informatics: decision support systems, clinical information systems, clinical research informatics, knowledge management and representation, consumer health informatics, natural language processing, public health informatics, privacy, ethical and societal aspects among them.Describing innovative approaches to the collection, organization, analysis, and data-sharing related to health and wellbeing, the book contributes to the expertise required to take medical informatics to the next level, and will be of interest to all those working in the field.

- The responsibilities for delivery of care in Sweden is divided between the regions and the municipalities. The regions run the hospitals and the primary care centres (PCCs) whereas the municipalities are responsible for homecare nursing and nursing homes. The homecare nurses and the doctors they need to seek advice from, thus belong to different organizations/contexts. As more patients with multi- and long-term illnesses are taken care of in their homes the workload of the homecare nurses has increased. A new healthcare agreement has thus been signed between a region in South Sweden and its municipalities. The healthcare agreement states that doctors from the PCCs are to form mobile teams together with the homecare nurses. This paper reports from a pre-study investigating how the agreement, in terms of translation sociology, is interpreted in four of the municipalities. The aim of the research project as a whole is to develop digital support systems for the mobile teams.

- The COVID-19 pandemic has significantly increased the use of remote services such as video consultations (VCs). In Sweden, private healthcare providers offering VCs have grown substantially since 2016 and have been controversial. Few studies have focused on physicians' experiences of providing care in this context. Our overall aim was to study physicians' experiences of VCs, here focusing on their suggestions for future improvements. Twenty-two semi-structured interviews were performed with physicians working for an online healthcare provider in Sweden, and analyzed through inductive content analysis. Two themes emerged related to desired future improvements of VCs; blended care and technical innovation.

- Vaccinations are one of the most significant interventions to public health, but vaccine hesitancy and skepticism are raising serious concerns for a portion of the population in many countries, including Sweden. In this study, we use Swedish social media data and structural topic modeling to automatically identify mRNA-vaccine related discussion themes and gain deeper insights into how people's refusal or acceptance of the mRNA technology affects vaccine uptake. Our point of departure is a scientific study published in February 2022, which seems to once again sparked further suspicion and concern and highlight the necessity to focus on issues about the nature and trustworthiness in vaccine safety. Structural topic modelling is a statistical method that facilitates the study of topic prevalence, temporal topic evolution, and topic correlation automatically. Using such a method, our research goal is to identify the current understanding of the mechanisms on how the public perceives the mRNA vaccine in the light of new experimental findings.

- OBJECTIVE: Human and Organizational Factors (HOF) studies in health technology involve human beings and thus require Institutional Review Board (IRB) approval. Yet HOF studies have specific constraints and methods that may not fit standard regulations and IRB practices. Gaining IRB approval may pose difficulties for HOF researchers. This paper aims to provide a first overview of HOF study challenges to get IRB review by exploring differences and best practices across different countries.METHODS: HOF researchers were contacted by email to provide a testimony about their experience with IRB review and approval. Testimonies were thematically analyzed and synthesized to identify and discuss shared themes.RESULTS: Researchers from seven European countries, Argentina, Canada, Australia, and the United States answered the call. Four themes emerged that indicate shared challenges in legislation, IRB inefficiencies and inconsistencies, general regulation and costs, and lack of HOF study knowledge by IRB members. We propose a model for IRB review of HOF studies based on best practices.CONCLUSION: International criteria are needed that define low and high-risk HOF studies, to allow identification of studies that can undergo an expedited (or exempted) process from those that need full IRB review. Enhancing IRB processes in such a way would be beneficial to the conduct of HOF studies. Greater knowledge and promotion of HOF methods and evidence-based HOF study designs may support the evolving discipline. Based on these insights, training and guidance to IRB members may be developed to support them in ensuring that appropriate ethical issues for HOF studies are considered.
