  • Rationale and Study Design for an Individualized Perioperative Open Lung Ventilatory Strategy in Patients on One-Lung Ventilation (iPROVE-OLV)

    • Objective: The aim of this clinical trial is to examine whether it is possible to reduce postoperative complications using an individualized perioperative ventilatory strategy versus using a standard lung-protective ventilation strategy in patients scheduled for thoracic surgery requiring one-lung ventilation. Design: International, multicenter, prospective, randomized controlled clinical trial. Setting: A network of university hospitals. Participants: The study comprises 1,380 patients scheduled for thoracic surgery. Interventions: The individualized group will receive intraoperative recruitment maneuvers followed by individualized positive end-expiratory pressure (open lung approach) during the intraoperative period plus postoperative ventilatory support with high-flow nasal cannula, whereas the control group will be managed with conventional lung-protective ventilation. Measurements and Main Results: Individual and total number of postoperative complications, including atelectasis, pneumothorax, pleural effusion, pneumonia, acute lung injury; unplanned readmission and reintubation; length of stay and death in the critical care unit and in the hospital will be analyzed for both groups. The authors hypothesize that the intraoperative application of an open lung approach followed by an individual indication of high-flow nasal cannula in the postoperative period will reduce pulmonary complications and length of hospital stay in high-risk surgical patients.   • Community Diarrhea Incidence Before and After Rotavirus Vaccine Introduction in Nicaragua

    • We estimated the incidence of watery diarrhea in the community before and after introduction of the pentavalent rotavirus vaccine in Leon, Nicaragua. A random sample of households was selected before and after rotavirus vaccine introduction. All children < 5 years of age in selected households were eligible for inclusion. Children were followed every 2 weeks for watery diarrhea episodes. The incidence rate was estimated as numbers of episodes per 100 child-years of exposure time. A mixed effects Poisson regression model was fit to compare incidence rates in the pre-vaccine and vaccine periods. The pre-vaccine cohort (N = 726) experienced 36 episodes per 100 child-years, and the vaccine cohort (N = 826) experienced 25 episodes per 100 child-years. The adjusted incidence rate ratio was 0.60 (95% confidence interval [CI] 0.40, 0.91) during the vaccine period versus the pre-vaccine period, indicating a lower incidence of watery diarrhea in the community during the vaccine period.

  • Etiology of Childhood Diarrhea After Rotavirus Vaccine Introduction A Prospective, Population-based Study in Nicaragua

    • Background: Nicaragua was the first developing nation to implement routine immunization with the pentavalent rotavirus vaccine (RV5). In this RV5-immunized population, understanding infectious etiologies of childhood diarrhea is necessary to direct diarrhea treatment and prevention efforts. Methods: We followed a population-based sample of children less than5 years in Leon, Nicaragua for diarrhea episodes through household visits. Information was obtained on RV5 history and sociodemographics. Stool samples collected during diarrhea episodes and among healthy children underwent laboratory analysis for viral, bacterial and parasitic enteropathogens. Detection frequency and incidence of each enteropathogen was calculated. Results: The 826 children in the cohort experienced 677 diarrhea episodes during 607.5 child-years of exposure time (1.1 episodes per child-year). At least 1 enteropathogen was detected among 61.1% of the 337 diarrheal stools collected. The most common enteropathogens among diarrheal stools were: norovirus (20.4%), sapovirus (16.6%), enteropathogenic Escherichia coli (11.3%), Entamoeba histolytica/dispar (8.3%), Giardia lamblia (8.0%) and enterotoxigenic E. coli (7.7%), with rotavirus detected among 5.3% of diarrheal stools. Enteropathogenic Escherichia coli and enterotoxigenic E. coli were frequently detected among stools from healthy children. Among children with diarrhea, norovirus was more commonly detected among younger children (less than2 years) and G. lamblia was more commonly detected among older children (2-4 years). The mean age of rotavirus detection was 34.6 months. Conclusions: In this Central American community after RV5 introduction, rotavirus was not commonly detected among children with diarrhea. Prevention and appropriate management of norovirus and sapovirus should be considered to further reduce the burden of diarrheal disease.

  • Iterative Dual-Metal and Energy Transfer Catalysis Enables Stereodivergence in Alkyne Difunctionalization: Carboboration as Case Study

    • Stereochemically defined tetrasubstituted olefins are widespread structural elements of organic molecules and key intermediates in organic synthesis. However, flexible methods enabling stereodivergent access to E and Z isomers of fully substituted alkenes from a common precursor represent a significant challenge and are actively sought after in catalysis, especially those amenable to complex multifunctional molecules. Herein, we demonstrate that iterative dual-metal and energy transfer catalysis constitutes a unique platform for achieving stereodivergence in the difunctionalization of internal alkynes. The utility of this approach is showcased by the stereodivergent synthesis of both stereoisomers of tetrasubstituted beta-boryl acrylates from internal alkynoates with excellent stereocontrol via sequential carboboration and photoisomerization. The reluctance of electron-deficient internal alkynes to undergo catalytic carboboration has been overcome through cooperative Cu/Pd-catalysis, whereas an Ir complex was identified as a versatile sensitizer that is able to photoisomerize the resulting sterically crowded alkenes. Mechanistic studies by means of quantum-chemical calculations, quenching experiments, and transient absorption spectroscopy have been applied to unveil the mechanism of both steps.

    • Wind and wave effects have a major impact on the design of mooring and anchoring systems, whose purpose is to prevent the movement of the cargo and the moored ship while optimizing the operations which have to take place in port. The dynamic behavior of a ship moored in waves has been mathematically described and interactions between ships and environmental loads have been commonly tested for different docks and mooring systems in scale models. However, the behavior of real ships in true mooring conditions has not been properly addressed in scientific literature. This paper proposes a novel computer vision technique to monitor moored ships. This approach uses the correlation of visual features in the images of the ship to estimate its movements along time. The proposed technique has been validated in laboratory conditions and applied in a real scenario to study the behavior of the ship Urania Mella in the Outer Port of A Coruna (Spain), in Punta Langosteira, obtaining very promising results.

    • The study and characterization of the oscillations of a moored vessel has a major importance in port operational efficiency, not only in the design of mooring and anchoring systems, but also in order to prevent the movement of the cargo, minimize ship and dock damage and optimizing the operations which have to take place while the ship is moored.The dynamic behavior of a vessel moored in waves has been mathematically described and interactions between ships and environmental loads such as waves, wind, currents and ice are commonly tested for different docks and mooring systems in scale models. However, field data studies of the behavior moored ships have not been properly addressed so far.This paper proposes a novel application of Inertial Measurement Units to estimate the angular movements (roll, pitch and yaw) of a moored vessel. The proposed technique has been validated in laboratory conditions and the behavior of the ship Urania Mella in the Outer Port of Punta Langosteira (A Coruña, Spain), has been analyzed, obtaining very promising results.

  • TOI-4438 b: a transiting mini-Neptune amenable to atmospheric characterization

    • We report the confirmation and mass determination of a mini-Neptune transiting the M3.5 V star TOI-4438 (G 182-34) every 7.44 days. A transit signal was detected with NASA's TESS space mission in the sectors 40, 52, and 53. In order to validate the planet TOI-4438 b and to determine the system properties, we combined TESS data with high-precision radial velocity measurements from the CARMENES spectrograph, spanning almost one year, and ground-based transit photometry. We found that TOI-4438 b has a radius of Rb = 2.52 ± 0.13 R⊕ (5% precision), which together with a mass of Mb = 5.4 ± 1.1 M⊕ (20% precision), results in a bulk density of ρb = 1.85-0.44+0.51 g cm-3 (~28% precision), aligning the discovery with a volatile-rich planet. Our interior structure retrieval with a pure water envelope yields aminimum water mass fraction of 46% (1σ). TOI-4438 b is a volatile-rich mini-Neptune with likely H/He mixed with molecules, such as water, CO2, and CH4. The primary star has a J-band magnitude of 9.7, and the planet has a high transmission spectroscopy metric (TSM) of 136 ± 13. Taking into account the relatively warm equilibrium temperature of Teq = 435 ± 15 K, and the low activity level of its host star, TOI-4438 b is one of the most promising mini-Neptunes around an M dwarf for transmission spectroscopy studies.
