    • Reduced lung function is associated with cardiovascular mortality, but the relationships with atherosclerosis are unclear. The population-based Swedish CArdioPulmonary BioImage study measured lung function, emphysema, coronary CT angiography, coronary calcium, carotid plaques and ankle-brachial index in 29,593 men and women aged 50–64 years. The results were confirmed using 2-sample Mendelian randomization. Lower lung function and emphysema were associated with more atherosclerosis, but these relationships were attenuated after adjustment for cardiovascular risk factors. Lung function was not associated with coronary atherosclerosis in 14,524 never-smokers. No potentially causal effect of lung function on atherosclerosis, or vice versa, was found in the 2-sample Mendelian randomization analysis. Here we show that reduced lung function and atherosclerosis are correlated in the population, but probably not causally related. Assessing lung function in addition to conventional cardiovascular risk factors to gauge risk of subclinical atherosclerosis is probably not meaningful, but low lung function found by chance should alert for atherosclerosis.

    • The Swedish CArdioPulmonary bioImage Study (SCAPIS) combines the use of new imaging technologies, large-scale proteomics/metabolomics/genomics, and epidemiological analyses to extensively characterize a Swedish cohort of 30,000 men and women aged between 50 and 64 years. Its main aims are to improve risk prediction and to optimize our ability to study mechanisms of cardiopulmonary diseases. SCAPIS is currently recruiting at six sites in Sweden, and a pilot study was conducted in 2012 to test the feasibility of the comprehensive study protocol. In the planning phase, it was recognized that the detailed phenotyping used in SCAPIS would identify a large number of clinical findings in need of medical attention. This was confirmed by evaluation of results from the pilot study. Here we focus on pulmonary nodules and asymptomatic coronary artery stenosis. These clinical features were observed in a large number of participants, and the clinical handing and prognosis related to these observations are unclear. They thus posed great challenges for the study in their practical and ethical handling. This chapter describes how we developed procedures to handle these findings based on existing evidence and expert consensus as well as deliberations on ethical issues.

  • Leaky lysosomes in lung transplant macrophages: azithromycin prevents oxidative damage

    • Background: Lung allografts contain large amounts of iron (Fe), which inside lung macrophages may promote oxidative lysosomal membrane permeabilization (LMP), cell death and inflammation. The macrolide antibiotic azithromycin (AZM) accumulates 1000-fold inside the acidic lysosomes and may interfere with the lysosomal pool of Fe. Objective: Oxidative lysosomal leakage was assessed in lung macrophages from lung transplant recipients without or with AZM treatment and from healthy subjects. The efficiency of AZM to protect lysosomes and cells against oxidants was further assessed employing murine J774 macrophages. Methods: Macrophages harvested from 8 transplant recipients (5 without and 3 with ongoing AZM treatment) and 7 healthy subjects, and J774 cells pre-treated with AZM, a high-molecular-weight derivative of the Fe chelator desferrioxamine or ammonium chloride were oxidatively stressed. LMP, cell death, Fe, reduced glutathione (GSH) and H-ferritin were assessed. Results: Oxidant challenged macrophages from transplants recipients without AZM exhibited significantly more LMP and cell death than macrophages from healthy subjects. Those macrophages contained significantly more Fe, while GSH and H-ferritin did not differ significantly. Although macrophages from transplant recipients treated with AZM contained both significantly more Fe and less GSH, which would sensitize cells to oxidants, these macrophages resisted oxidant challenge well. The preventive effect of AZM on oxidative LMP and J774 cell death was 60 to 300 times greater than the other drugs tested. Conclusions: AZM makes lung transplant macrophages and their lysososomes more resistant to oxidant challenge. Possibly, prevention of obliterative bronchiolitis in lung transplants by AZM is partly due to this action.

  • Intramammary infections and somatic cell counts in meat and pelt producing ewes with clinically healthy udders

    • Mastitis in sheep is common and is important both from an economic and from a welfare point of view. It can be either clinical or subclinical, bacterial or, more seldom, lentiviral. There are no data on the national prevalence of subclinical mastitis (SCM) or intramammary infection (IMI), and causative pathogens, in ewes in Sweden.Diagnosis of SCM and/or IMI is based on inflammatory indicators, measured in milk and additional bacteriology. Somatic cell count (SCC) is the most widely used indicator of SCM and/or IMI and can be measured either directly or indirectly with e.g. California Mastitis Test (CMT). However, the use of SCC as an indicator for mastitis and/or IMI in ewes is not fully evaluated, especially not in meat- and pelt producing herds. The aim of this study was therefore to investigate the prevalence of IMI and bacterial panorama in ewes with clinically healthy udders, in meat- and pelt producing herds in Sweden. Moreover, we wanted to define a cut-off for SCC and CMT for detecting ewes with IMI that would be suitable for use under field conditions.Udder half milk samples (n = 2134) were collected at weaning and lambing from meat- and pelt producing ewes (n = 773), in 22 farms in Sweden. Only clinically healthy animals were included in the study. Milk samples were analysed for bacteriology and SCC and were given a CMT score.Intramammary infection was found in 30% of the ewes and in 14% of the udder half milk samples. No bacteria were found in 74% of the milk samples, and mixed flora was found in 12% of the samples. Among the 287 milk samples where IMI was identified, coagulase negative staphylococci (CoNS) were most prevalent (58%) followed by Staphylococcus (S.) aureus (9%) and Mannheimia (M.) haemolytica (6%). Among the 165 CoNS findings S. simulans was the most common (26%), followed by S. warren, S. equorum, S. xylosus, S. haemolyticus and S. chromogenes. A CMT score of >= 3 and a SCC of 500,000 cells/ml at weaning or CMT score 3 and a SCC of 400,000 cells/ml after lambing gave the highest possible Se at the same time as the highest possible Sp for identifying udder halves with IMI and may be used for screening for IMI in Swedish meet and pelt producing herds.To conclude, one third of Swedish meat- and pelt producing ewes without clinical signs in the udder had IMI M one or both udder halves at weaning and or after lambing, and CoNS was the most common bacterial finding. A high SCC 400,000-500,000 cells/nil) was associated with IMI and a difference in having a high (CMT 3-5) or low (CMT 1-2) between udder halves can be used as an indicator of IMI under field conditions.

  • Methodology for evaluation of hazards from solid waste and landfill-generated leachate

    • A methodology based on an analytical protocol for evaluation of hazards from landfill leachate and solid waste is described. A dynamic analytical protocol, the LAQUA protocol, including measurements of inorganic and water-quality parameters, polar and non-polar organic marker compounds, and toxicity, was constructed. An acute toxicity test, using the brackish water crustacean Artemia salina as test organism, was developed. The methodology was applied to authentic problems such as investigation of different treatment techniques for landfill leachate, evaluation of leaching tests and characterization of solid wastes, and an investigation of a filter material aimed for leachate treatment. Investigated treatment methods comprised in all cases pre-treatment by aeration combined with sedimentation, followed by one of: bioremediation, ozonation, chemical oxidation by Fenton’s reagent, or geo-bed filters. Evaluated filter materials were mixtures of natural or residual waste products. A combination of pre-treatment followed by a geo-bed filter containing a mixture of peat and carbon-containing ash gave an efficient simultaneous removal of metals and organic pollutants. The performance of two leaching tests for characterization of solid waste, the up-flow percolation test (SIS-CEN/TS 14405:2004) and the batch test (SS-EN 12457 -3), was investigated. Solid waste materials (sludge from street gutters and fragmented metallic waste) were characterized using leaching tests and the hazards of the materials were evaluated in the eluate, obtained at specific liquid-to-solid ratios (L/S). The L/S 2 and L/S 10 values were compared with limit values included in the waste acceptance criteria (WAC). The analyses were extended towards specific organic compounds, such as individual phenolic compounds and polychlorinated biphenyls (PCB). Organic compounds were found in eluates from both types of tests, showing the possibility to use these methods to evaluate the leaching of such compounds from waste materials. The use of authentic leachate as leachant, leads to increased concentrations of heavy metals in the eluate, compared to the prescribed use of demineralised water as leachant. Generally good agreement was found between the results of the two leaching methods. A strategy based on batch tests is described for investigation of a filter material for leachate treatment. Batch tests gave suitable information about the leaching from new and used material, and showed high removal efficiencies of metals and non-polar organic compounds. However, for investigation of removal of polar organic markers (e.g. phenolic compounds) a batch test is not sufficient and should be supplemented by a column test.
