Six-week follow-up after HIV-1 exposure: a position statement from the Public Health Agency of Sweden and the Swedish Reference Group for Antiviral Therapy

- In 2014 the Public Health Agency of Sweden and the Swedish Reference Group for Antiviral Therapy (RAV) conducted a review and analysis of the state of knowledge on the duration of follow-up after exposure to human immunodeficiency virus (HIV). Up until then a follow-up of 12 weeks after exposure had been recommended, but improved tests and new information on early diagnosis motivated a re-evaluation of the national recommendations by experts representing infectious diseases and microbiology, county medical officers, the RAV, the Public Health Agency, and other national authorities. Based on the current state of knowledge the Public Health Agency of Sweden and the RAV recommend, starting in April 2015, a follow-up period of 6 weeks after possible HIV-1 exposure, if HIV testing is performed using laboratory-based combination tests detecting both HIV antibody and antigen. If point-of-care rapid HIV tests are used, a follow-up period of 8 weeks is recommended, because currently available rapid tests have insufficient sensitivity for detection of HIV-1 antigen. A follow-up period of 12 weeks is recommended after a possible exposure for HIV-2, since presently used assays do not include HIV-2 antigens and only limited information is available on the development of HIV antibodies during early HIV-2 infection. If pre- or post-exposure prophylaxis is administered, the follow-up period is recommended to begin after completion of prophylaxis. Even if infection cannot be reliably excluded before the end of the recommended follow-up period, HIV testing should be performed at first contact for persons who seek such testing.

- The purpose of the study is to explore how Swedish lower secondary school teachers manage blended learning environments, established through using a specific learning management system (LMS) application. In the study, four teachers were followed during a four-month (n)ethnographic fieldwork. Based on analyses of data from video-recordings and observations in physical and virtual classrooms, the study examines teachers’ practices of integrating and segmenting the two classroom domains. In order to unpack the realms of these practices, the study employs affordance and boundary theories. Through the analysis of participants’ boundary practices and their use of communicative affordances in and across space and time, four teacher roles, enacted and emerging through teaching practices, are presented. The paper concludes with a discussion of how participants’ engagement with virtual and physical learning environment compels teachers to reflect upon their preferred teacher role in the new multidimensional classrooms.

- This article examines how older people make sense of their situation in calls to a helpline a few months into the Covid-19 pandemic. By drawing on the sociology of everyday life to analyse callers’ various understandings of the crisis, the article nuances current knowledge about older people’s situation. The thematic analysis shows that the callers make sense of the crisis linked to social relations on a personal, anonymous, and abstract level. The callers’ responses to challenges to their everyday routines – adjustment or critical evaluation – are connected to different approaches to trust: basic trust in a shared social reality with someone or regulating trust in a set of norms independent from that other. Whereas the calls demonstrate very few positive adjustments in personal relations, they show that anonymous and abstract relations serve as important resources for both maintaining and re-evaluating everyday life during a crisis. Although older people’s lack of secure personal relations during the pandemic points to vulnerability, their resourcefulness is apparent in their active engagement in important anonymous and abstract relations.

Pulp Particle Classification Based on Optical Fiber Analysis and Machine Learning Techniques

- In the pulp and paper industry, pulp testing is typically a labor-intensive process performed on hand-made laboratory sheets. Online quality control by automated image analysis and machine learning (ML) could provide a consistent, fast and cost-efficient alternative. In this study, four different supervised ML techniques—Lasso regression, support vector machine (SVM), feed-forward neural networks (FFNN), and recurrent neural networks (RNN)—were applied to fiber data obtained from fiber suspension micrographs analyzed by two separate image analysis software. With the built-in software of a commercial fiber analyzer optimized for speed, the maximum accuracy of 81% was achieved using the FFNN algorithm with Yeo–Johnson preprocessing. With an in-house algorithm adapted for ML by an extended set of particle attributes, a maximum accuracy of 96% was achieved with Lasso regression. A parameter capturing the average intensity of the particle in the micrograph, only available from the latter software, has a particularly strong predictive capability. The high accuracy and sensitivity of the ML results indicate that such a strategy could be very useful for quality control of fiber dispersions.

Serum IgG levels to Epstein-Barr and measles viruses in patients with multiple sclerosis during natalizumab and interferon beta treatment

- Background Patients with multiple sclerosis (MS) demonstrate higher seroprevalence of Epstein-Barr virus (EBV) and increased anti-EBV IgG levels in serum compared with healthy controls. Intrathecal antibody production to measles virus (MeV) is a common finding in patients with MS. Objective To measure serum IgG reactivity to EBV glycoprotein 350 (gp350) and MeV nucleocapsid protein (N-CORE) in patients with MS and healthy controls and to determine if reactivity changed in patients during interferon beta (IFN beta) and/or natalizumab (NAT) treatment. A secondary aim was to determine the seroprevalence of EBV in patients and controls. Methods Patients with MS (n=728) were included from the Swedish pharmacovigilance study for NAT. Paired serum samples from 714 patients drawn before and during NAT treatment and paired samples from 170 patients during prior IFN beta treatment were analysed. In total, 156 patients were included in both groups. Samples from 144 matched blood donors served as controls. Indirect ELISA was applied using recombinant EBVgp350 and MeV N-CORE as antigens. EBVgp350 IgG seronegative samples were also analysed using EBV nuclear antigen 1 and viral capsid antigen (VCA). Results Patients with MS showed higher serum levels of anti-EBVgp350 and anti-MeV N-CORE IgG compared with controls. During NAT treatment, the levels of anti-EBVgp350 and anti-MeV N-CORE IgG declined, compared with the relatively stable levels noted during prior IFN beta treatment. Ten patients failed to demonstrate anti-EBVgp350 IgG but did show detectable anti-VCA IgG, indicating EBV seropositivity. In contrast, 10/144 controls were EBV seronegative. Conclusions Treatment with NAT, which is considered a selective immunosuppressive agent with a compartmentalised effect on the central nervous system, appeared to be associated with a moderate decrease in circulating IgG levels to EBVgp350 and MeV N-CORE. All patients with MS were EBV IgG seropositive, supporting the potential role of EBV in the pathogenesis of MS.

Caregivers' use of robots and their effect on work environment : a scoping review

- Despite the lively discussion on the pros and cons of using robots in health care, little is still known about how caregivers are affected when robots are introduced in their work environment. The present scoping review fills this research gap by mapping previous studies about the relation between robots in care and caregivers’ working life. The paper is based on searches in four databases for peer-reviewed articles about robots in care settings, published 2000 to 2020. The 27 included papers were examined with the questions of 1) how robots are used by caregivers, and 2) how robots affect caregivers’ work environment. The analysis shows that the use of robots can affect both the physical and the psychosocial work environment, in positive as well as in negative ways. Robots are used in care settings to reduce physical and mental demands of the caregivers, but they can, in fact, increase caregivers’ workload. Thus, the review indicates that robots can improve the quality of work, but that they seldom work as a shortcut to increased efficiency or time effectiveness.
