    • Many real-world production systems are complex in nature and it is a real challenge to find an efficient scheduling method that satisfies the production requirements as well as utilizes the resources efficiently. Tools like discrete event simulation (DES) are very useful for modeling these systems and can be used to test and compare different schedules before dispatching the best schedules to the targeted systems. DES alone, however, cannot be used to find the "optimal" schedule. Simulation-based optimization (SO) can be used to search for optimal schedules efficiently without too much user intervention. Observing that long computing time may prohibit the interest in using SO for industrial scheduling, various techniques to speed up the SO process have to be explored. This paper presents a case study that shows the use of a Web-based parallel and distributed SO platform to support the operations scheduling of a machining line in an automotive factory.

    • The concept of bonding and antibonding orbitals is fundamental in chemistry. The population of those orbitals and the energetic difference between the two reflect the strength of the bonding interaction. Weakening the bond is expected to reduce this energetic splitting, but the transient character of bond-activation has so far prohibited direct experimental access. Here we apply time-resolved soft X-ray spectroscopy at a free electron laser to directly observe the decreased bonding antibonding splitting following bond-activation using an ultrashort optical laser pulse.

    • Aims: Health literacy, the degree to which individuals understand and act upon health information, may have a pivotal role in the prevention of cardiovascular disease (CVD), with low health literacy potentially explaining poorer adherence to prevention guidelines. We investigated the associations between health literacy, ultrasound-detected carotid atherosclerosis and cardiovascular risk factors.Methods: Baseline data (cross-sectional analysis) from a randomized controlled trial, integrated within the Västerbotten Intervention Program, Northern Sweden, was used. We included 3459 individuals, aged 40 or 50 years with ≥1 conventional risk factor or aged 60 years old. The participants underwent clinical examination, blood sampling, carotid ultrasound assessment of intima-media wall thickness (CIMT) and plaque formation, and answered a questionnaire on health literacy – the Brief Health Literacy Screen. The European Systematic Coronary Risk Evaluation and Framingham Risk Score were calculated.Results: About 20% of the participants had low health literacy. Low health literacy was independently associated with the presence of ultrasound-detected carotid artery plaques after adjustment for age and education, odds ratio (95% confidence interval) 1.54 (1.28–1.85), demonstrating a similar level of risk as for smoking. Health literacy was associated with CIMT in men. Low health literacy was associated with higher CVD risk scores. Sensitivity analyses with low health literacy set to 9% or 30% of the study sample, respectively, yielded essentially the same results.Conclusions: Low health literacy was independently associated with carotid artery plaques and a high level of CVD risk scores. Presenting health information in a fashion that is understood by all patients may improve preventive efforts.

    • Sentinel lymph node dissection is widely used in the staging of endometrial cancer. Variation in surgical techniques potentially impacts diagnostic accuracy and oncologic outcomes, and poses barriers to the comparison of outcomes across institutions or clinical trial sites. Standardization of surgical technique and surgical quality assessment tools are critical to the conduct of clinical trials. By identifying mandatory and prohibited steps of sentinel lymph node (SLN) dissection in endometrial cancer, the purpose of this study was to develop and validate a competency assessment tool for use in surgical quality assurance. A Delphi methodology was applied, included 35 expert gynecological oncology surgeons from 16 countries. Interviews identified key steps and tasks which were rated mandatory, optional, or prohibited using questionnaires. Using the surgical steps for which consensus was achieved, a competency assessment tool was developed and subjected to assessments of validity and reliability. Seventy percent consensus agreement standardized the specific mandatory, optional, and prohibited steps of SLN dissection for endometrial cancer and informed the development of a competency assessment tool. Consensus agreement identified 21 mandatory and three prohibited steps to complete a SLN dissection. The competency assessment tool was used to rate surgical quality in three preselected videos, demonstrating clear separation in the rating of the skill level displayed with mean skills summary scores differing significantly between the three videos (F score=89.4; P<0.001). Internal consistency of the items was high (Cronbach α=0.88). Specific mandatory and prohibited steps of SLN dissection in endometrial cancer have been identified and validated based on consensus among a large number of international experts. A competency assessment tool is now available and can be used for surgeon selection in clinical trials and for ongoing, prospective quality assurance in routine clinical care.

    • Computer simulation has been described as the most effective tool for de-signing and analyzing systems in general and discrete-event systems (e.g., production or logistic systems) in particular (De Vin et al. 2004). Historically, the main disadvantage of simulation is that it was not a real optimization tool. Recently, research efforts have been focused on integrating metaheuristic algorithms, such as genetic algorithms (GA) with simulation software so that “optimal” or close to optimal solutions can be found automatically. An optimal solution here means the setting of a set of controllable design variables (also known as decision variables) that can minimize or maximize an objective function. This approach is called simulation optimization or simulation-based optimization (SBO), which is perhaps the most important new simulation technology in the last few years (Law and McComas 2002). In contrast to other optimization problems, it is assumed that the objective function in an SBO problem cannot be evaluated analytically but have to be estimated through deterministic/ stochastic simulation.
