  • ApoCIII-Enriched LDL in Type 2 Diabetes Displays Altered Lipid Composition, Increased Susceptibility for Sphingomyelinase and Increased binding to Biglycan.

    • Objective- Apolipoprotein CIII (apoCIII) is an independent risk factor for cardiovascular disease, but the molecular mechanisms involved are poorly understood. Here, we investigated potential proatherogenic properties of apoCIII-containing LDL from hypertriglyceridemic patients with type 2 diabetes. Research design and methods - LDL was isolated from controls and subjects with type 2 diabetes, and from apoB transgenic mice. LDL-biglycan binding was analyzed with a solid-phase assay using immunoplates coated with biglycan. Lipid composition was analyzed with mass spectrometry. Hydrolysis of LDL by sphingomyelinase was analyzed after labeling plasma LDL with [(3)H]sphingomyelin. ApoCIII isoforms were quantified after isoelectric focusing. Human aortic endothelial cells were incubated with desialylated apoCIII or with LDL enriched with specific apoCIII isoforms. Results- We showed that enriching LDL with apoCIII only induced a small increase in LDL-proteoglycan binding, and this effect was dependent on a functional Site A in apoB100. Our findings indicated that intrinsic characteristics of the diabetic LDL other than apoCIII per se are responsible for further increased proteoglycan binding of diabetic LDL with high endogenous apoCIII, and we showed alterations in the lipid composition of diabetic LDL with high apoCIII. We also demonstrated that high apoCIII increased susceptibility of LDL to hydrolysis and aggregation by SMase. In addition, we demonstrated that sialylation of apoCIII increased with increasing apoCIII content, and that sialylation of apoCIII was essential for its proinflammatory properties. Conclusions- We have demonstrated a number of features of apoCIII-containing LDL from hypertriglyceridemic patients with type 2 diabetes that could explain the proatherogenic role of apoCIII.

  • Enabling relationship formation, development, and closure in a one-year female mentoring program at a non-governmental organization : a mixed-method study

    • Background: Mental health problems among young women aged 16-24 have increased significantly in recent decades, and interventions are called for. Mentoring is a well-established preventative/promotive intervention for developing adolescents, but we have yet to fully understand how the relationship between the mentor and the protégé forms, develops, and closes. In this study, we focused on a female mentoring program implemented by a Swedish non-governmental organization, The Girls Zone. First, we examined the psychological and social characteristics of the young women who chose to take part in the program as protégés. Second, we investigated adolescent female protégés’ own experiences of the relationship process based on a relational-cultural theory perspective.Methods: The mixed-method study included 52 questionnaires and five semi-structured interviews with young women aged 15–26 who had contacted The Girls Zone between 2010 and 2012 in order to find a mentor. Their experience of the mentoring relationships varied in duration. Data were analysed statistically and with inductive qualitative content analysis.Results: The group of protégés was heterogeneous in that some had poor mental health and some had good mental health. On the other hand, the group was homogenous in that all its members had shown pro-active self-care by actively seeking out the program due to experiences of loneliness and a need to meet and talk with a person who could listen to them. The relationships were initially characterized by feelings of nervousness and ambivalence. However, after some time, these developed into authentic, undemanding, non-hierarchical relationships on the protégés’ terms. The closure of relationships aroused feelings of both abandonment and developing strength.Conclusions: Mentorships that are in line with perspectives of the relational-cultural theory meet the relationship needs expressed by the female protégés. Mentor training should focus on promoting skills such as active listening and respect for the protégé based on an engaged, empathic, and authentic approach in a non-hierarchical relationship. These insights have the potential to inform interventions in several arenas where young women create authentic relationships with older persons, such as in school, in traditional health care contexts, and in youth recreation centres.

    • The transition from adolescence to adulthood can bring with it mental health problems, resulting in reduced mental well-being among young women and an increasing public health issue. Perceived mental health problems can be a major obstacle to personal development and opportunities for becoming established in society. Thus, promotive interventions are needed.The overall aim of this thesis was to explore women’s experience of mentoring relationships as health promotion from the perspectives of both parties in the dyad: the young woman transitioning from adolescence to adulthood (the protégé), and her ten-year-older female mentor.This thesis used a practice-based approach to investigate a group of participants involved in a Swedish non-governmental organization, the Girls Zone. Data collection was conducted including interviews (n = 5) and surveys (n = 52) with female protégés, and interviews with female mentors (n = 12). Study I explored the characteristics of the female protégés and the development of the mentoring relationship, and used mixed methods. Study II, which investigated mentors’ initial motives and the organizational context which enabled the mentors’ engagement, used an explorative qualitative method.This thesis showed that female mentoring relationships seem to have potential to be a health-promoting intervention. A variety of young women were attracted to the mentoring program, and mentorships in line with the perspectives of relational-cultural theory could meet the relationship needs expressed by the female protégés. Further, mentors’ motivations for engaging as mentors were linked to the fulfillment of basic psychological needs for autonomy, competence, and relatedness, in accordance with the perspective of self-determination theory.

    • Mentoring aimed at supporting young people and their development shows promising results, but its delivery is threatened by the difficulty of recruiting sufficient numbers of mentors and keeping them engaged over time. The aim of this study was to help overcome this problem by examining female mentors’ motives for engaging in formal voluntary mentoring of young women, and exploring how organizations can facilitate these mentors’ satisfaction in staying engaged over time. Based on qualitative interviews with 12 mentors in a Swedish non-governmental organization, the Girls Zone, we show six categories of mentor motives related to initial motivation for engagement: self-interested reasons, empowering women, being a responsible citizen, sense of compassion, self-awareness, and longing for meaningfulness. In addition, we show five categories related to the organizational work of satisfying mentors: a win-win relationship, a feeling of ambivalence despite clear responsibilities and contributions, customized support and guidance, a caring organizational identity, and a commitment to pursue with feelings of duty and emotional connection. Using Self-Determination Theory as the framework to guide our understanding of the findings, we conclude that mentors’ motivations for engaging as mentors are linked to the fulfillment of basic psychological needs for autonomy, competence, and relatedness. Practical recommendations are offered in light of the findings.

    • Objectives Exchangeable low-density lipoprotein (LDL)-associated proteins can affect the atherogenic properties of LDL. Our aim was to analyze the protein composition of LDL from individuals with or without type 2 diabetes and the metabolic syndrome (T2DM) in relation to other LDL-particle characteristics, to assess whether certain proteins associate more with certain subclasses of LDL typical for T2DM, such as small, apoCIII-rich LDL. Design LDL from two cohorts of 61-year-old men (n = 19 and 64) with or without T2DM was isolated using size-exclusion chromatography or deuterium oxide-based ultracentrifugation. LDL-associated proteins were identified using mass spectrometry and quantified using two-dimensional gel electrophoresis or enzyme-linked immunosorbent assay. Differently expressed LDL-associated proteins apolipoprotein (apo)J and lysozyme were also measured in serum from a third cohort of women (n = 71) with or without T2DM. Lysozyme binding to advanced glycation end product (AGE)-LDL was examined in vitro. Results ApoJ and lysozyme were increased in LDL particles with increased apoCIII content and decreased cholesterol content. When isolated with SEC, LDL from individuals with T2DM contained more apoJ and lysozyme and less apoA1 than LDL from control individuals. LDL content of apoJ correlated with a smaller LDL-particle size. Serum levels of lysozyme, but not apoJ, were increased in individuals with T2DM. In vitro, lysozyme associated more with AGE-LDL than with unmodified LDL. Conclusions Our results indicate that apoJ and lysozyme are increased in LDL with characteristics of small dense LDL in T2DM. Small dense LDL is easily glycated, and the increased affinity of lysozyme for AGE-LDL provides a possible partial explanation for an increase of lysozyme from those with type 2 diabetes.

  • Studies on the atherogenicity of apoB-containing lipoproteins in type 2 diabetes

    • Type 2 diabetes (T2D) and the metabolic syndrome (MetS), two conditions that are rapidly increasing in prevalence, as well as the dyslipoproteinemia and subclinical inflammation characteristic for these conditions, are associated with an increased risk for developing and dying of cardiovascular disease (CVD). The aim of this thesis was to investigate possible atherogenic properties of very low-density lipoprotein (VLDL) and low-density lipoprotein (LDL) in T2D and MetS. We compared the susceptibility to lipolysis by secretory phospholipase A2 group V (sPLA2-V) of VLDL and LDL from individuals with T2D and MetS and from healthy individuals. We also characterized LDL-associated proteins, and compared the protein composition of LDL in T2D and MetS with LDL from healthy individuals. Finally, we investigated if lysozyme, one of the proteins that was increased in T2D-MetS-LDL, was elevated in serum of individuals with T2D and MetS as well. Lipid-enriched VLDL and small, cholesterol-poor and triglyceride-rich LDL from T2D-MetS-individuals were more extensively lipolyzed by sPLA2-V than control VLDL and LDL. 31 LDL-associated proteins, important for lipoprotein metabolism, complement, coagulation, oxidation, and inflammation, were identified in LDL. VLDL and LDL from T2D-MetS-individuals contained more apolipoprotein (apo) C3 per particle, and an increased LDL-apoC3 content correlated with a lower cholesterol content of LDL and a smaller LDL-size. T2D-MetS-LDL also contained less apoA1 and more apoJ and lysozyme than did control LDL, and higher abundances of apoJ and lysozyme also correlated with a lower cholesterol content in LDL. Lysozyme was also found to be elevated in serum of T2D-MetS individuals, and lysozyme levels correlated with serum creatinine and insulin levels. The identified LDL-associated proteins might be of importance for the inflammation following LDL retention in the intima. An increased sPLA2-V-mediated lipolysis of VLDL and LDL in individuals with T2D and MetS may cause increased retention of LDL and lead to high local concentrations in the intima of proinflammatory fatty acids and lysophosphatidylcholine. This might lead to an accelerated atherosclerosis development in these individuals. An increased understanding of lipoprotein alterations in diabetes may furthermore serve as a basis for the development of new treatment strategies for atherosclerosis in T2D and MetS.

    • Evidence based goals for the treatment and prevention of atherosclerosis in diabetes are given in international and national guidelines. The importance of optimal control of lipids and blood pressure has been shown in several studies. With available drugs and behavioural modifications the treatment goals can be reached in most cases. However, only a few patients with diabetes are treated optimally today. A major possibility to reduce cardiovascular disease in diabetes is to treat patients according to guidelines. New treatment targets may include specific treatment of the dyslipidaemia, manifested in high levels of small dense LDL and low HDL, active anti-inflammatory treatments, specific reduction of inflammatory activity in adipose tissue, reduced volume of adipose tissue, antioxidants and reduction of advanced glycosylation endproducts production. Possible strategies for these treatments are available, and should be evaluated in clinical trials.
