    • Syftet med detta projekt har varit att med hjälp av en intervjustudie och kunskapsgenomgång redovisahur utomhusmiljö, gemensamhetslokaler och bostäder kan utformas för att minska risken för att äldreoch personer med nedsatt funktionsförmåga blir medtagna eller dör i förtid under värmeböljor. Måletär att öka medvetenheten och intresset för anpassning av miljö och byggnader hos personer ansvarigaför samhällsplanering, bostadsbestånd, trygghets-, vård- och omsorgsboenden.Städer är normalt varmare och mindre blåsiga än det omgivande landskapet. Städernas ”varmareklimat” beror främst på den större värmelagring som kan ske i byggnader, gator, trottoarer mm,begränsat med vegetation som kan skugga och avge fukt samt aktiviteter som trafik och eldning vilketgenererar värme. Under värmeböljor ökar dödligheten mer i städer. Att leva ensam, vara sängbundenoch bo på översta våningen har visats vara riskfaktorer.Åtgärderna för att minska stadens värmeö och värmeböljornas effekter på människor brukar iblanddelas in i ”mjuka åtgärder” (information, varningssystem för värmeböljor, insatser för känsligagrupper), ”gröna åtgärder” (göra staden till en grönare miljö) och ”tekniska åtgärder” (skuggandekonstruktioner, modifiering av väggar, kylning/luftkonditionering inomhus etc.), vilka kompletterarvarandra. I vissa länder, bl. a. England, ska äldreboenden ha ett samlingsrum som kan hållas svaltäven under värmeböljor, men det är oklart vilken juridisk status som bestämmelserna har.Intervjustudien syftade till att belysa hur problemen uppfattas av personal inom äldreomsorgen iSverige. Som datainsamlingsmetod genomfördes 20 semistrukturerade intervjuer medomvårdnadspersonal i Botkyrka kommun under oktober 2011. Urvalet baserades påtillgänglighetsprincipen. Innehållsanalyser gjordes på transkriberad intervjudata och kategorier ochunderkategorier skapades utifrån återkommande teman som återfanns i texten. Slutsatserna frånstudien pekar på att de utbildnings- och informationsinsatser angående värmeböljors effekter påkänsliga grupper som riktas till personal inom äldreomsorgen borde intensifieras, samt attpersonalens kunskap om verksamheten och vårdtagarnas behov borde tas tillvara redan iplaneringsstadiet för äldreboenden.

    • Arbetsgivares juridiska ansvar för arbetsmiljö och ett hållbart arbete återfinns i en mängd olika bestämmelser av varierande karaktär, samt i domar och beslut från Arbetsmiljöverket, vilket inte underlättar vare sig överskådlighet och förståelse av regelverkets innebörd. Avsikten är att i den här artikeln dels analysera arbetsgivares juridiska ansvar för arbetstagares arbetsbelastning inom arbetsmiljörätten, dels undersöka hur problem med arbetsbelastning upplevs samt om, och i så fall hur, dessa problem hanteras inom såväl manligt och kvinnligt dominerade arbetsplatser som inom privat och offentlig verksamhet.

  • Comparison of self-assessment and expert assessment of occupational exposure to chemicals

    • OBJECTIVES: Occupational assessments of chemical exposure are often inadequate because of difficulties in obtaining sufficient numbers of measurements by trained professionals (experts). The objective of this study was to determine whether workers can provide unbiased data via self-assessments of exposure facilitated by the use of simple passive monitors for personal sampling.METHODS: Untrained workers obtained personal measurements of their exposures to gaseous contaminants (terpenes in sawmills and styrene in reinforced plastics factories) with passive monitors and written instructions. To study the validity of the self-assessments, an occupational hygienist performed exposure measurements on the same occupational groups after the workers had obtained two or more measurements independently. The potential bias of the self-assessments was evaluated by comparing the self-assessments with the expert assessments in mixed-effects statistical models.RESULTS: A total of 153 terpene (97 self and 56 expert) and 216 styrene (159 self and 57 expert) measurements were obtained from four sawmills and six reinforced plastics factories, respectively. No significant differences in the geometric mean exposures were observed between the self-assessments and the expert assessments in 3 of 4 sawmills and 5 of 6 reinforced plastics factories (P > 0.10). The potential bias of the self-assessments of exposure ranged from less than 0.1% to 102% and was less than 17% in 9 of the 10 groups investigated.CONCLUSIONS: The results indicate that untrained, unsupervised workers are able to collect consistently unbiased exposure data by employing currently available passive monitors.

  • How does the distribution of work tasks among home care personnel relate to workload and health-related quality of life?

    • Background: The work for Swedish home care workers is challenging with a variety of support and healthcare tasks for home care recipients. The aim of our study is to investigate how these tasks relate to workload and health-related quality of life among home care workers in Sweden. We also explore staff preferences concerning work distribution.Methods: A cross-sectional study was conducted in 16 municipalities in Northern Sweden. Questionnaires with validated instruments to measure workload (QPSNordic) and health-related quality of life (EQ-5D), were responded by 1154 (~ 58%) of approximately 2000 invited home care workers. EQ-5D responses were translated to a Quality-adjusted life-year (QALY) score. For 15 different work task areas, personnel provided their present and preferred allocation. Absolute risk differences were calculated with propensity score weighting.Results: Statistically significantly more or fewer problems differences were observed for: higher workloads were higher among those whose daily work included responding to personal alarms (8.4%), running errands outside the home (14%), rehabilitation (13%) and help with bathing (11%). Apart from rehabilitation, there were statistically significantly more (8–10%) problems with anxiety/depression for these tasks. QALY scores were lower among those whose daily work included food distribution (0.034) and higher for daily meal preparation (0.031), both explained by pain/discomfort dimension. Personnel preferred to, amongst other, spend less time responding to personal alarms, and more time providing social support.Conclusion: The redistribution of work tasks is likely to reduce workload and improve the health of personnel. Our study provides an understanding of how such redistribution could be undertaken.

  • Chemical exposure in the work place : mental models of workers and experts

    • Many workers are daily exposed to chemical risks in their work place that has to be assessed and controlled. Due to exposure variability, repeated and random measurements should be conducted for valid estimates of the average exposure. Traditionally, experts such as safety engineers, work environment inspectors, and occupational hygienists, have performed the measurements. In self assessment of exposure (SAE), the workers perform unsupervised exposure measurements of chemical agents. This thesis studies a prerequisite for SAE, i.e. the workers’ mental models of chemical exposure. Further, the workers’ mental models are contrasted with experts’ reasons and decision criteria for measurement. Both qualitative and quantitative data generated from three studies (Paper I, II, and III) were used to describe the workers’ mental model of chemical exposure. SAE was introduced to workers in three different industries; transports (benzene), sawmill industry (monoterpenes), and reinforced plastic industry (styrene). By interviews, qualitative data were collected on the workers’ interpretation of measurement results and preventive actions. To evaluate the validity of worker measurement, the measurements were compared with expert measurements. The association between each worker’s number of performed measurement and mean level and variability in exposure concentrations was calculated. Mean absolute percent/forecast error (MAPE) was used to assess whether the workers’ decision models were in accordance with a coherence or correspondence model. In Paper IV, experts (safety engineers, work environment inspectors, and occupational hygienists) were interviewed to elucidate their mental models about the triggers and decision criteria for exposure measurements. The results indicate that the workers’ measurement results were in agreement with experts’. However, the measurement results were not a strong enough signal to induce workers to take preventive actions and sustained exposure measurements even if the measurement result were close to the occupational exposure limit. The fit was best for the median model, indicating that the workers’ mental models for interpretation of measurement data can best be described by the coherence theory rather than by the correspondence theory. The workers seemed to mentally reduce the variation in the exposure to a measure of central tendency (the median), and underestimated the average exposure level. The experts were found to directly take preventive actions instead of performing exposure measurements. When they performed exposure measurements, a worst case sampling strategy was most common. An important trigger for measurement for the experts was “request from the employer” (safety engineers), “legal demands” (work environment inspectors), and “symptoms among workers” (occupational hygienists). When there was a trigger, all experts mentioned expectations of high exposure level as a decision criterion for measurements. In conclusion, the studies suggest that workers’ mental interpretation model is best described in terms of a coherence model rather than a model of correspondence. The workers reduced the variation mentally in favor of an estimate of average exposure (median), which may imply that they underestimate short-term, high exposure health risks. A consequence is that interpretation of measurements such as SAE cannot be given to the individual worker without some support, e.g. from an expert. However, experts often chose to directly take preventive actions, without measuring the exposure. The results indicate that also the experts need support e.g. from the legal system if exposure measurements are to be done.
