    • The questionnaire CICI:PQ for measuring needs, concerns and coping of parents of chronically ill children was mailed to 85 families with children with cystic fibrosis (CF). Some questions about the parents' experiences of the health care system were also included. Fifty-four per cent responded--in all cases but two the mothers. More than half of the mothers wanted help with or opportunities to discuss the state of health and the emotional development of the child and dietary issues. Many of them were concerned about the child's future. Shortage of time was another problem. The coping strategies used when having problems with the child were partly different from those used when problems arose with the partly different from those used when problems arose with the spouse. Most mothers were satisfied with the hospital care and the CF paediatrician. They wanted further support from the physiotherapist and many of them missed a CF nurse. The project illuminated many areas for further investigation.

    • Background: For most critically ill patients requiring mechanical ventilation in the intensive care unit (ICU) weaning is uncomplicated. For the remainder, weaning is a challenge and may result in further complications and increased risk of mortality. Critical care nurses (CCNs) require substantial knowledge and experience to manage patients experiencing prolonged weaning. Aim: The aim of this study was to explore CCNs approach for management of patients experiencing prolonged weaning in the ICU. Methods: Semi-structured interviews with 19 experienced CCNs were conducted. Data were analysed using content analysis. Results: Participants used various strategies for weaning that were grouped into four categories: individualized planning for the weaning process, assessing patient's capacity, managing the process and team interaction. The overall theme that emerged was: CCNs drive the weaning process using both a patient centred and targeted approach. Conclusion: CCNs in these ICUs performed weaning using a patient centred approach to plan, initiate, assess and establish a holistic weaning process. Team collaboration among all health care practitioners was important. CCNs have a key role in prioritizing weaning and driving the process forward. Relevance to clinical practice: Closeness to the patient and a key role in management of the mechanical ventilated patient in ICU gives the CCN unique potential to develop and create models and tools for prolonged weaning. These tools, specially designed for patients undergoing prolonged weaning, would give focus on continuous planning, collaborating, managing and evaluation in the process of liberating patients from mechanical ventilation.

    • The aim of this article was to outline the basis for an introductory course to higher education focusing on students' approaches to learning and the role of teaching in higher education. The framework is a discussion on contemporary literature on approaches to learning. In the article experiences are also reported of developing and implementing an introductory course at the Sahlgrenska Academy, Göteborg University, Sweden. This was made within the LearnAble project, which was a two‐tier approach in which students were given an introductory course to higher education parallel with a course in pedagogy in higher education for teachers. The courses were evaluated with different instruments, which is also to some extent accounted for. The transition from upper secondary school to studies in higher education is discussed, as is the importance of a scholarship of teaching in the context of the courses mentioned above. In this article the authors argue that a well‐planned and stimulating introduction to higher education could be a natural part of the transition process, which can help students develop better prerequisites to manage their studies in higher education.

    • The pap gene clusters encode P fimbriae and fimbriae-associated G adhesins. DNA sequence analysis has resolved three G adhesin variants (papGJ96, papGIA2 and prsGJ96) that differ in receptor specificity and therefore in binding to epithelial cells. In this study, DNA probes specific for the pap gene cluster or the papGJ96, papGIA2 and prsGJ96 adhesin sequences were used to examine 74 fecal and 204 urinary Escherichia coli isolates (67 from acute pyelonephritis, 71 from acute cystitis and 66 from asymptomatic bacteriuria). In accordance with previous studies, a higher frequency of pap+ strains was found in the urinary strains (71%) than in the fecal (20%) E. coli isolates. The papGIA2, and prsGJ96 sequences were more frequent among urinary (42% papG+IA2, 23% prsG+J96) than among fecal (18% papG+IA2, 5% prsG+J96) isolates. None of the isolates hybridized with the papGJ96 probe. Pap+ strains accounted for 82% of the pyelonephritis, 69% of the cystitis and 61% of the asymptomatic bacteriuria strains. The papGIA2 genotype dominated in acute pyelonephritis strains (72% papG+IA2, 16% prsG+J96). The prsGJ96 genotype was most frequent in cystitis strains (25% papG+IA2, 37% prsG+J96). The asymptomatic bacteriuria strains formed an intermediate group (30% papG+IA2, 14% prsG+J96). Most of the papG+IA2 strains expressed P fimbriae which agglutinated human erythrocytes, sheep erythrocytes and Gal alpha 1-4Gal beta latex beads. The prsG+J96 strains varied in agglutination of human and sheep erythrocytes and Gal alpha 1-4Gal beta-latex beads. The results demonstrated that the papGIA2 and prsGJ96 adhesin DNA sequences differ in disease association.
