    • Introduction Lymph node metastases are a key prognostic factor in prostate cancer (PCa), but detecting lymph node lesions from PET/CT images is a subjective process resulting in inter-reader variability. Artificial intelligence (AI)-based methods can provide an objective image analysis. We aimed at developing and validating an AI-based tool for detection of lymph node lesions. Methods A group of 399 patients with biopsy-proven PCa who had undergone(18)F-choline PET/CT for staging prior to treatment were used to train (n = 319) and test (n = 80) the AI-based tool. The tool consisted of convolutional neural networks using complete PET/CT scans as inputs. In the test set, the AI-based lymph node detections were compared to those of two independent readers. The association with PCa-specific survival was investigated. Results The AI-based tool detected more lymph node lesions than Reader B (98 vs. 87/117;p = .045) using Reader A as reference. AI-based tool and Reader A showed similar performance (90 vs. 87/111;p = .63) using Reader B as reference. The number of lymph node lesions detected by the AI-based tool, PSA, and curative treatment was significantly associated with PCa-specific survival. Conclusion This study shows the feasibility of using an AI-based tool for automated and objective interpretation of PET/CT images that can provide assessments of lymph node lesions comparable with that of experienced readers and prognostic information in PCa patients.

  • Risk of retinal detachment after pediatric cataract surgery

    • PURPOSE: To determine the long-term risk of retinal detachment following pediatric cataract surgery and to identify risk factors for retinal detachment.METHODS: We included all children (aged 0 to 17 years) who during the time period of 1977 to 2005 underwent pediatric cataract surgery in Denmark, excluding cataract cases caused by trauma, or acquired systemic or acquired ocular pathology, and cases with ocular anomalies associated with the development of retinal detachment. Cases of cataract were ascertained from the mandatory Danish National Patient Register, and information on retinal detachment was based on medical chart review.RESULTS: Among 1043 eyes of 656 children undergoing surgery for pediatric cataract, 25 eyes (23 children) developed retinal detachment at a median time of 9.1 years after surgery. The overall 20-year risk of retinal detachment was 7% (95% confidence interval [CI]: 3%-11%) among cataract patients. In otherwise normal children having isolated cataract, the risk was 3% (95% CI: 0%-7%). A significantly higher risk of developing retinal detachment was found in children with mental retardation (23% [95% CI: 9%-35%]) or in cataract cases with other ocular or systemic anomalies (16% [95% CI: 6%-24%]).CONCLUSIONS: The estimated overall risk of retinal detachment 20 years after pediatric cataract surgery was 7%, but only 3% for isolated cataract. Particularly high risks of retinal detachment after cataract surgery were associated with mental retardation and having other ocular or systemic diseases.

  • A low-gluten diet induces changes in the intestinal microbiome of healthy Danish adults

    • © 2018, The Author(s). Adherence to a low-gluten diet has become increasingly common in parts of the general population. However, the effects of reducing gluten-rich food items including wheat, barley and rye cereals in healthy adults are unclear. Here, we undertook a randomised, controlled, cross-over trial involving 60 middle-aged Danish adults without known disorders with two 8-week interventions comparing a low-gluten diet (2 g gluten per day) and a high-gluten diet (18 g gluten per day), separated by a washout period of at least six weeks with habitual diet (12 g gluten per day). We find that, in comparison with a high-gluten diet, a low-gluten diet induces moderate changes in the intestinal microbiome, reduces fasting and postprandial hydrogen exhalation, and leads to improvements in self-reported bloating. These observations suggest that most of the effects of a low-gluten diet in non-coeliac adults may be driven by qualitative changes in dietary fibres.

  • Norbornadiene-Based Photoswitches with Exceptional Combination of Solar Spectrum Match and Long-Term Energy Storage

    • Norbornadiene-quadricyclane (NBD-QC) photoswitches are candidates for applications in solar thermal energy storage. Functionally, they rely on an intramolecular [2+2] cycloaddition reaction, which couples the S0 landscape on the NBD side to the S1 landscape on the QC side of the reaction and vice-versa. This commonly results in an unfavourable correlation between the first absorption maximum and the barrier for thermal back-conversion. This work demonstrates that this correlation can be counteracted by using steric repulsion to hamper the rotational motion of the side groups along the back-conversion path. It is shown that this modification reduces the correlation between the effective back-conversion barrier and the first absorption maximum and also increases the back-conversion entropy. The resulting molecules exhibit exceptionally long half-lives for their metastable forms without significantly affecting other properties, most notably solar spectrum match and storage density.

  • Tuning Molecular Solar Thermal Properties by Modification of a Promising Norbornadiene Photoswitch

    • Carefully targeted modifications to the norbornadiene-quadricyclane couple should give molecules with great potential for solar energy storage. Here we report the synthesis of derivatives of a well-studied compound, 2-cyano-3-anisyl norbornadiene (NBD5). The conjugation was extended by conversion of the nitrile into acrylonitrile and acrylate derivatives over two steps. Despite red-shifting the absorbance properties, the metastable quadricyclanes exhibited extremely short lifetimes. Meanwhile, treatment of NBD5 with acetyl halides in the presence of aluminum trihalides led to the formation of 7-acetyl norbornenes through a Wagner–Meerwein rearrangement. Subsequent transformations made it possible to synthesise norbornadienes with varying degrees of steric bulk at the 7-position of the molecular scaffold. It was found that the bulkier this group was, the more stable the quadricyclane form. This reaction sequence allows for the synthesis of norbornadiene derivatives which are more suited to molecular solar thermal applications in domestic heating despite providing a slightly lower energy density.

  • Potential synergy between PSMA uptake and tumour blood flow for prediction of human prostate cancer aggressiveness

    • Background Both prostate-specific membrane antigen (PSMA) uptake and tumour blood flow (TBF) correlate with International Society of Urological Pathology (ISUP) Grade Group (GG) and hence prostate cancer (PCa) aggressiveness. The aim of the present study was to evaluate the potential synergistic benefit of combining the two physiologic parameters for separating significant PCa from insignificant findings. Methods From previous studies of [Rb-82]Rb positron emission tomography (PET) TBF in PCa, the 43 patients that underwent clinical [Ga-68]Ga-PSMA-11 PET were selected for this retrospective study. Tumours were delineated on [Ga-68]Ga-PSMA-11 PET or magnetic resonance imaging. ISUP GG was recorded from 52 lesions. Results [Ga-68]Ga-PSMA-11 maximum standardized uptake value (SUVmax) and [Rb-82]Rb SUVmax correlated moderately with ISUP GG (rho = 0.59 and rho = 0.56, both p < 0.001) and with each other (r = 0.65, p < 0.001). A combined model of [Ga-68]Ga-PSMA-11 and [Rb-82]Rb SUVmax separated ISUP GG > 2 from ISUP GG 1-2 and benign with an area-under-the-curve of 0.85, 96% sensitivity, 74% specificity, and 95% negative predictive value. The combined model performed significantly better than either tracer alone did (p < 0.001), primarily by reducing false negatives from five or six to one (p <= 0.025). Conclusion PSMA uptake and TBF provide complementary information about tumour aggressiveness. We suggest that a combined analysis of PSMA uptake and TBF could significantly improve the negative predictive value and allow non-invasive separation of significant from insignificant PCa.

    • Aim : To develop a convolutional neural network (CNN) based automated method for quantification of 18F-choline uptake in the prostate gland in PET/CT studies and to study the association between this measure, clinical data and overall survival in patients with prostate cancer. Methods : A CNN was trained to segment the prostate gland in CT images using manual segmentations performed by a radiologist in a group of 100 patients, who had undergone 18F-FDG PET/CT. After the training process, the CNN automatically segmented the prostate gland in the CT images and SUV values in the corresponding PET images were automatically analyzed in a separate validation group consisting of 45 patients with biopsy-proven hormone-naïve prostate cancer. All patients had undergone an 18F-choline PET/CT as part of a previous research project. Voxels localized in the prostate gland and having a SUV >2.65 were defined as abnormal, as proposed by Reske S et al. (2006). Automated calculation of the following five PET measurements was performed: maximal SUV within the prostate gland - SUVmax; average SUV for voxels with SUV >2.65 - SUVmean; volume of voxels with SUV >2.65 - VOL; fraction of VOL related to the whole volume of the prostate gland - FRAC; product SUVmean x FRAC defined as Total Lesion Uptake - TLU. The association between the automated PET measurements, age, PSA, Gleason score and overall survival (OS) was evaluated using a univariate Cox proportional hazards regression model. Kaplan-Meier analysis was used to estimate the survival difference (log-rank test). Results : TLU and FRAC were significantly associated with OS in the Cox analysis while the other three PET measurements; age, PSA and Gleason score were not. Kaplan-Meier analysis showed that patients with SUVmax <5.3, SUVmean <3.5 and TLU <1 showed significantly longer survival times than patients with values higher than these thresholds. No significant differences were found when patients were stratified based on the other two PET measurements, PSA or Gleason score. Conclusion : Measurements reflecting 18F-choline PET uptake in the prostate gland obtained using a completely automated method were significantly associated with OS in patients with hormone-naïve prostate cancer. This type of objective quantification of PET/CT studies could be of value also for other PET tracers and other cancers in the future.

    • The introduction of various photochromic units into the same molecule is an attractive approach for the development of novel molecular solar thermal (MOST) energy storage systems. Here, we present the synthesis and characterisation of a series of covalently linked norbornadiene/dihydroazulene (NBD/DHA) conjugates, using the Sonogashira coupling as the key synthetic step. Generation of the fully photoisomerized quadricyclane/vinylheptafulvene (QC/VHF) isomer was found to depend strongly on how the two units are connected - by linear conjugation (a para-phenylene bridge) or cross-conjugation (a meta-phenylene bridge) or by linking to the five- or seven-membered ring of DHA - as well as on the electronic character of another substituent group on the NBD unit. When the QC-VHF system could be reached, the QC-to-NBD back-reaction occurred faster than the VHF-to-DHA back-reaction, while the latter could be promoted simply by the addition of Cu(i) ions. The absence or presence of Cu(i) can thus be used to control whether heat releases should occur on different or identical time scales. The experimental findings were rationalized in a computational study by comparing natural transition orbitals (NTOs). Moreover, the calculations revealed an energy storage capacity of 106-110 kJ mol(-1) of the QC-VHF isomers, which is higher than the sum of the capacities of the individual, separate units. The major contribution to the energy storage relates to the energetic QC form, while the major contribution to the absorption of visible light originates from the DHA photochrome; some of the NBD-DHA conjugates had absorption onsets at 450 nm or beyond.
