  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

  • ALMA High-frequency Long Baseline Campaign in 2021 : Highest Angular Resolution Submillimeter Wave Images for the Carbon-rich Star R Lep

    • The Atacama Large Millimeter/submillimeter Array (ALMA) was used in 2021 to image the carbon-rich evolved star R Lep in Bands 8-10 (397-908 GHz) with baselines up to 16 km. The goal was to validate the calibration, using band-to-band (B2B) phase referencing with a close phase calibrator J0504-1512, 1.degrees 2 from R Lep in this case, and the imaging procedures required to obtain the maximum angular resolution achievable with ALMA. Images of the continuum emission and the hydrogen cyanide (HCN) maser line at 890.8 GHz, from the J = 10-9 transition between the (1110) and (0400) vibrationally excited states, achieved angular resolutions of 13, 6, and 5 mas in Bands 8-10, respectively. Self-calibration (self-cal) was used to produce ideal images to compare with the B2B phase referencing technique. The continuum emission was resolved in Bands 9 and 10, leaving too little flux for the self-cal of the longest baselines, so these comparisons are made at coarser resolution. Comparisons showed that B2B phase referencing provided phase corrections sufficient to recover 92%, 83%, and 77% of the ideal image continuum flux densities. The HCN maser was sufficiently compact to obtain self-cal solutions in Band 10 for all baselines (up to 16 km). In Band 10, B2B phase referencing as compared to the ideal images recovered 61% and 70% of the flux density for the HCN maser and continuum, respectively.

  • High Angular Resolution ALMA Images of Dust and Molecules in the SN 1987A Ejecta

    • We present high angular resolution (similar to 80 mas) ALMA continuum images of the SN.1987A system, together with CO J = 2 -> 1, J = 6 -> 5, and SiO J = 5 -> 4 to J = 7 -> 6 images, which clearly resolve the ejecta (dust continuum and molecules) and ring (synchrotron continuum) components. Dust in the ejecta is asymmetric and clumpy, and overall the dust fills the spatial void seen in H alpha images, filling that region with material from heavier elements. The dust clumps generally fill the space where CO J = 6 -> 5 is fainter, tentatively indicating that these dust clumps and CO are locationally and chemically linked. In these regions, carbonaceous dust grains might have formed after dissociation of CO. The dust grains would have cooled by radiation, and subsequent collisions of grains with gas would also cool the gas, suppressing the CO J = 6 -> 5 intensity. The data show a dust peak spatially coincident with the molecular hole seen in previous ALMA CO J = 2 -> 1 and SiO J = 5 -> 4 images. That dust peak, combined with CO and SiO line spectra, suggests that the dust and gas could be at higher temperatures than the surrounding material, though higher density cannot be totally excluded. One of the possibilities is that a compact source provides additional heat at that location. Fits to the far-infrared-millimeter spectral energy distribution give ejecta dust temperatures of 18-23 K. We revise the ejecta dust mass to M-dust = 0.2-0.4 M-circle dot for carbon or silicate grains, or a maximum of <0.7 M-circle dot for a mixture of grain species, using the predicted nucleosynthesis yields as an upper limit.

  • Training future generations to deliver evidence-based conservation and ecosystem management

    • 1. To be effective, the next generation of conservation practitioners and managers need to be critical thinkers with a deep understanding of how to make evidence-based decisions and of the value of evidence synthesis.2. If, as educators, we do not make these priorities a core part of what we teach, we are failing to prepare our students to make an effective contribution to conservation practice.3. To help overcome this problem we have created open access online teaching materials in multiple languages that are stored in Applied Ecology Resources. So far, 117 educators from 23 countries have acknowledged the importance of this and are already teaching or about to teach skills in appraising or using evidence in conservation decision-making. This includes 145 undergraduate, postgraduate or professional development courses.4. We call for wider teaching of the tools and skills that facilitate evidence-based conservation and also suggest that providing online teaching materials in multiple languages could be beneficial for improving global understanding of other subject areas.

  • Novel genetic loci associated with hippocampal volume

    • The hippocampal formation is a brain structure integrally involved in episodic memory, spatial navigation, cognition and stress responsiveness. Structural abnormalities in hippocampal volume and shape are found in several common neuropsychiatric disorders. To identify the genetic underpinnings of hippocampal structure here we perform a genome-wide association study (GWAS) of 33,536 individuals and discover six independent loci significantly associated with hippocampal volume, four of them novel. Of the novel loci, three lie within genes (ASTN2, DPP4 and MAST4) and one is found 200 kb upstream of SHH. A hippocampal subfield analysis shows that a locus within the MSRB3 gene shows evidence of a localized effect along the dentate gyrus, subiculum, CA1 and fissure. Further, we show that genetic variants associated with decreased hippocampal volume are also associated with increased risk for Alzheimer's disease (r(g) = -0.155). Our findings suggest novel biological pathways through which human genetic variation influences hippocampal volume and risk for neuropsychiatric illness.

    • This article presents the key results of a major survey carried out by the NEARCH project on the public perception of archaeology and heritage across Europe. The analysis focuses on three main points of significance for contemporary archaeological practice. The first is the image of archaeology and its definition in the perception of the general public. The second concerns the values that archaeology represents for the public. The third focuses on the social expectations placed on archaeologists and archaeology. The NEARCH survey clearly indicates that there is a significant public expectation by Europeans that archaeology should work comprehensively across a broad range of areas, and that cultural heritage management in general needs to engage more with different archaeological and heritage groups.

    • Effects of Elm tree sawdust pretreatments using alkali and alkaline earth metals (NaCl, KCl, CaCl2, MgCl2 and Elm tree ash) and deashing solutions (water, HCl, HNO3 and aqua regia) before the carbonization process on the porosity of produced activated carbons and Pb (II) and Cr (VI) adsorption were studied. The activated carbons were characterized by pore size distribution, surface area, FTIR, and SEM-EDX analysies. Based on the results, HCl leaching pretreatment of the biomass increased the activated carbon adsorption capacity of Cr (VI) from 114 to 190 mg g−1. The treatment of biomass with alkali and alkali earth metal salts, especially MgCl2, remarkably increased the activated carbon adsorption capacity of Pb (II) from 233 to 1430 mg g−1. The results indicated that Pb (II) adsorption was attributed to both the mesoporous structure of activated carbon and the abundance of Mg on the activated carbon's surface. On the other hand, the micropores played a major role in Cr (VI) adsorption capacity. The development of the micro- or mesoporous structure of activated carbons through pretreatment of lignocellulosic precursor could be an approach for providing high performance activated carbons for Pb (II) and Cr (VI) removal from aqueous solutions.

    • ABSTRACT: Full-scale waste combustion tests showed that adding animal waste (AW) to municipal solid waste (MSW) prevented bed agglomeration, and the reason for this fi nding was not fully understood. This study uses thermodynamic modeling to investigate the composition of equilibrium products for two combustion scenarios: monocombustion of MSW (the reference case) and cocombustion of AW with MSW (the AW case). The modeling was performed using FactSage, and experimental data obtained during the full-scale combustion tests were used as input data for the calculations. The results of equilibrium modeling, together with information extracted from ternary phase diagrams, suggest higher bed temperature as the primary cause for formation of bed agglomerates in the reference case. In addition, melt-induced agglomeration is suggested as the bed agglomeration mechanism in this case. In the AW case, however, reduced bed temperature, as well as enriched calcium phosphate and sulfate in the bottom ashes are considered to signi fi cantly decrease the slagging tendency.
