  • Effect of leisure time physical activity on severe knee or hip osteoarthritis leading to total joint replacement: a population-based prospective cohort study

    • Background: Studies on leisure time physical activity as risk factor or protective factor for knee or hip osteoarthritis (OA) show divergent results. Longitudinal prospective studies are needed to clarify the association of physical activity with future OA. The aim was to explore in a prospective population-based cohort study the influence of leisure time physical activity on severe knee or hip OA, defined as knee or hip replacement due to OA. Methods: Leisure time physical activity was reported by 28320 participants (mean age 58 years (SD 7.6), 60% women) at baseline. An overall leisure time physical activity score, taking both duration and intensity of physical activities into account, was created. The most commonly reported activities were also used for analysis. The incidence of knee or hip replacement due to OA over 11 years was monitored by linkage with the Swedish hospital discharge register. Cox's proportional hazards model (crude and adjusted for potential confounding factors) was used to assess the incidence of total joint replacement, or osteotomy (knee), in separate analyses of leisure time physical activity. Results: There was no significant overall association between leisure time physical activity and risk for knee or hip replacement due to OA over the 11-year observation time. For women only, the adjusted RR (95% CI) for hip replacement was 0.66 (0.48, 0.89) (fourth vs. first quartile), indicating a lower risk of hip replacement in those with the highest compared with the lowest physical activity. The most commonly reported activities were walking, bicycling, using stairs, and gardening. Walking was associated with a lower risk of hip replacement (adjusted RR 0.76 (95% CI 0.61, 0.94), specifically for women (adjusted RR 0.75 (95% CI 0.57, 0.98)). Conclusions: In this population-based study of middle-aged men and women, leisure time physical activity showed no consistent overall relationship with incidence of severe knee or hip OA, defined as joint replacement due to OA, over 11 years. For women, higher leisure time physical activity may have a protective role for the incidence of hip replacement. Walking may have a protective role for hip replacement, specifically for women.

    • Fiberoptic bronchoscopy with bronchoalveolar lavage and protected specimen brush technique has become an established method for etiologic diagnosis in severe forms of pulmonary infections during recent years. In this study, including 62 bronchoscopies in 53 patients, a standardized program, covering all important pulmonary pathogens, has been evaluated in a heterogeneous group of patients. Results providing therapeutic guidelines were obtained in 53 percent (16/30) of the immunocompromised patients (including 5 bronchoscopies on HIV-positive patients), but only 19 percent (6/32) of the immunocompetent patients (p < 0.001). We conclude that bronchoscopy is of great value for diagnosing pulmonary infections in immunocompromised patients. In immunocompetent patients, the diagnostic yield is lower and the indication for bronchoscopy must be established for each individual patient based on clinical importance, resources, and risk. When bronchoscopy is performed, we believe that a standardized program like ours reduces the risk of missing important pathogens.

    • Thirty-five strains of Pasteurella multocida from humans and animals were tested for susceptibility to five cephalosporins by a broth dilution method. Cefcanel showed high activity against all isolates (MIC and MBC, less than or equal to 0.64 micrograms/ml). The corresponding figure for cefaclor and cefuroxime was 2.56 micrograms/ml. Cefadroxil and cephalexin were the least active compounds tested.

    • OBJECTIVES: To determine in a large prospective population-based cohort study relationships between different measures of body mass and incidence of severe knee and hip osteoarthritis (OA) defined as arthroplasty of knee or hip due to OA. MATERIALS AND METHODS: Body mass index (BMI), waist circumference, waist-hip ratio (WHR), weight, and percentage of body fat (BF%) was measured at baseline in 11026 men and 16934 women, 45 to 73 years old, from the general population. Incidence of severe OA was defined as arthroplasty due to knee or hip OA monitored over 11 years of follow-up by linkage with the Swedish hospital discharge register. RESULTS: During follow-up, 471 individuals had knee OA and 551 had hip OA. After adjustment for age, sex, smoking and physical activity, the relative risks (RR) of knee OA (4th vs. 1st quartile) were 8.1 (95% CI 5.3-12.4) for BMI, 6.7 (4.5-9.9) for waist circumference, 6.5 (4.6-9.43) for weight, 3.6 (2.6-5.0) for BF% and 2.2 (1.7-3.0) for WHR. The corresponding RR for hip OA were 2.6 (2.0-3.4) for BMI, 3.0 (2.3-4.0) for weight, 2.5 (1.9-3.3) for waist, 1.3 (0.99-1.6) for WHR and 1.5 (1.2-2.0) for BF%. CONCLUSIONS: All measures of overweight were significantly associated with incidence of knee OA leading to arthroplasty, with the strongest relative risk gradient observed for BMI. Even though incidence of hip OA showed smaller differences between normal weight and obesity, body mass was a significant risk factor also for hip OA leading to arthroplasty. Our results support a major link between overweight and biomechanics in increasing knee and hip OA risk in both men and women, but do not exclude a contributing role of metabolic factors associated with adipose tissue.

    • Peptidoglycan from Staphylococcus aureus mobilized CD66b in human neutrophils but did not upregulate surface activation markers in eosinophils. In addition, Toll-like receptor 2, implicated in the recognition of peptidoglycan, was detected on the surface of resting neutrophils but not on eosinophils. These findings suggest roles for neutrophils but not eosinophils in innate recognition of peptidoglycan.

    • This article presents results from 23 structured interviews focused on creative meetings (a crucial part of the definition being that something new is created in the meeting, through interaction between people). The interviewees represented many different fields of industry and academia, and together many hundred years of experience of meetings. Six main perspectives pertaining to creative processes were discussed: dynamics and social interaction, leadership, actions and results, technical equipment, diversity and language, and size of meetings. The interviewees were asked to relate their personal experiences, and the study is therefore qualitative and retrospective. Opinions were similar for some factors, including the central role of trust and optimal size of creative meetings (6-10 people). Views differed considerably for other factors; for example on the impact of diversity on the creative process, and whether creative meetings happen by chance or by planning and deliberation. For many topics, both positive and negative aspects were mentioned; for example, the influence of diversity and language on creative processes.

    • Lipase production of 425 S. aureus strains isolated from patients with different clinical diagnoses and healthy carriers were measured by a specific method, using emulsified trioleoylglycerol substrate. Strains isolated from patients with septicemia showed significantly higher lipase activity than osteomyelitis strains (p = 0.011), impetigo strains (p = 0.002) and strains isolated from healthy relatives of patients with recurrent furunculosis (p = 0.019). Recurrent furunculosis and pyomyositis strains had significantly higher (p = 0.002 and 0.032, respectively) lipase activity than septicemia strains. S. aureus strains isolated from patients with a significant antibody response in an antilipase ELISA did not show a higher lipase activity in culture supernatants than strains from patients without a significant antibody response. The lipase activity was significantly higher in strains isolated from deep or subcutaneous infections, i.e., septicemia, pyomyositis, osteomyelitis, aerobic and anaerobic furunculosis, than in strains from superficial infections, i.e. impetigo, or from nasal mucosa.

    • We report 2 cases of severe infections due to Pasteurella multocida, both occurring during pregnancy in previously healthy women. Both women had contact with animals (dog and cat) but neither of them had been bitten. Apart from a slight decrease in IgG levels, no immunological defects could be detected. Both women had received oral phenoxymethylpenicillin in the early phase of the disease, but still fell ill with severe infections. One woman had meningitis while the other suffered from cellulitis with deep abscess formation.
