  • Prevalence of Subclinical Coronary Artery Atherosclerosis in the General Population

    • Background: Early detection of coronary atherosclerosis using coronary computed tomography angiography (CCTA), in addition to coronary artery calcification (CAC) scoring, may help inform prevention strategies. We used CCTA to determine the prevalence, severity, and characteristics of coronary atherosclerosis and its association with CAC scores in a general population.Methods: We recruited 30 154 randomly invited individuals age 50 to 64 years to SCAPIS (the Swedish Cardiopulmonary Bioimage Study). The study includes individuals without known coronary heart disease (ie, no previous myocardial infarctions or cardiac procedures) and with high-quality results from CCTA and CAC imaging performed using dedicated dual-source CT scanners. Noncontrast images were scored for CAC. CCTA images were visually read and scored for coronary atherosclerosis per segment (defined as no atherosclerosis, 1% to 49% stenosis, or ≥50% stenosis). External validity of prevalence estimates was evaluated using inverse probability for participation weighting and Swedish register data.Results: In total, 25 182 individuals without known coronary heart disease were included (50.6% women). Any CCTA-detected atherosclerosis was found in 42.1%; any significant stenosis (≥50%) in 5.2%; left main, proximal left anterior descending artery, or 3-vessel disease in 1.9%; and any noncalcified plaques in 8.3% of this population. Onset of atherosclerosis was delayed on average by 10 years in women. Atherosclerosis was more prevalent in older individuals and predominantly found in the proximal left anterior descending artery. Prevalence of CCTA-detected atherosclerosis increased with increasing CAC scores. Among those with a CAC score >400, all had atherosclerosis and 45.7% had significant stenosis. In those with 0 CAC, 5.5% had atherosclerosis and 0.4% had significant stenosis. In participants with 0 CAC and intermediate 10-year risk of atherosclerotic cardiovascular disease according to the pooled cohort equation, 9.2% had CCTA-verified atherosclerosis. Prevalence estimates had excellent external validity and changed marginally when adjusted to the age-matched Swedish background population.Conclusions: Using CCTA in a large, random sample of the general population without established disease, we showed that silent coronary atherosclerosis is common in this population. High CAC scores convey a significant probability of substantial stenosis, and 0 CAC does not exclude atherosclerosis, particularly in those at higher baseline risk.

  • Fitness, strength and severity of COVID-19: a prospective register study of 1 559 187 Swedish conscripts

    • Objective To investigate the possible connection between cardiorespiratory fitness (CRF) and muscle strength in early adulthood and severity of COVID-19 later in life. Design Prospective registry-based cohort study. Participants 1 559 187 Swedish men, undergoing military conscription between 1968 and 2005 at a mean age of 18.3 (SD 0.73) years. Main outcome measures Hospitalisation, intensive care or death due to COVID-19 from March to September 2020, in relation to CRF and muscle strength. Results High CRF in late adolescence and early adulthood had a protective association with severe COVID-19 later in life with OR (95% CI) 0.76 (0.67 to 0.85) for hospitalisation (n=2 006), 0.61 (0.48 to 0.78) for intensive care (n=445) and 0.56 (0.37 to 0.85) for mortality (n=149), compared with the lowest category of CRF. The association remains unchanged when controlled for body mass index (BMI), blood pressure, chronic diseases and parental education level at baseline, and incident cardiovascular disease before 2020. Moreover, lower muscle strength in late adolescence showed a linear association with a higher risk of all three outcomes when controlled for BMI and height. Conclusions Physical fitness at a young age is associated with severity of COVID-19 many years later. This underscores the necessity to increase the general physical fitness of the population to offer protection against future viral pandemics.

  • Novel subgroups of adult-onset diabetes and their association with outcomes : a data-driven cluster analysis of six variables

    •  BackgroundDiabetes is presently classified into two main forms, type 1 and type 2 diabetes, but type 2 diabetes in particular is highly heterogeneous. A refined classification could provide a powerful tool to individualise treatment regimens and identify individuals with increased risk of complications at diagnosis.MethodsWe did data-driven cluster analysis (k-means and hierarchical clustering) in patients with newly diagnosed diabetes (n=8980) from the Swedish All New Diabetics in Scania cohort. Clusters were based on six variables (glutamate decarboxylase antibodies, age at diagnosis, BMI, HbA1c, and homoeostatic model assessment 2 estimates of β-cell function and insulin resistance), and were related to prospective data from patient records on development of complications and prescription of medication. Replication was done in three independent cohorts: the Scania Diabetes Registry (n=1466), All New Diabetics in Uppsala (n=844), and Diabetes Registry Vaasa (n=3485). Cox regression and logistic regression were used to compare time to medication, time to reaching the treatment goal, and risk of diabetic complications and genetic associations.FindingsWe identified five replicable clusters of patients with diabetes, which had significantly different patient characteristics and risk of diabetic complications. In particular, individuals in cluster 3 (most resistant to insulin) had significantly higher risk of diabetic kidney disease than individuals in clusters 4 and 5, but had been prescribed similar diabetes treatment. Cluster 2 (insulin deficient) had the highest risk of retinopathy. In support of the clustering, genetic associations in the clusters differed from those seen in traditional type 2 diabetes.InterpretationWe stratified patients into five subgroups with differing disease progression and risk of diabetic complications. This new substratification might eventually help to tailor and target early treatment to patients who would benefit most, thereby representing a first step towards precision medicine in diabetes.

I denna antologi problematiserar historiker och musik- och kulturvetare några aspekter av fenomenet Mahler: Hur har Gustav Mahler och hans musik uppfattats i Sverige och Tyskland under 1900-talet? Vilket förhållande hade Arnold Schönberg till Mahler? Hur kan Mahler placeras in i framväxten av den moderna musiketnologin? Hur kan man teoretiskt förstå de beröringspunkter som finns mellan text, bild och Mahlers musik utifrån några skivomslag? Bidragen har sitt ursprung i den föredrags- och konsertserie som arrangerades våren 2011 i Lund med anledning av att det var 100 år sedan Mahler avled. Medverkande var Lunds universitets musikcentrum Odeum, representanter från musik- och historievetenskaperna vid samma universitet och från Svenskt visarkiv. Dessutom framförde musikstuderande från Musikhögskolan i Malmö musik av Mahler.

    • I samband med det forskningsprojektet Ingrepp genomfördes en Ingreppfestival med avseende på att i ett öppet publikt sammanhang diskutera utgångspunkterna forskningsgruppen formulerat i tabloiden Ingrepp – en tidning om städer, planering och konst, vilket delades ut gratis en vecka innan festivalen. I tidningen hittade man läsning kring städer, stadsutveckling, konst och konstnärlig forskning vilken hade bäring på den utvecklingsprocess som pågick och pågår på Södra Älvstranden i Göteborg. Både tabloiden och festivalen var ett direkt uttryck för gruppen vilja att ta ut diskussionen på gatan och i sitt sammanhang, men också ett sätt att pröva konstens och akademins möjligheter att gripa in och delta i samhälleliga skeenden. Festivalen vilken turnerade runt på fyra platser under fyra dagar längs Södra Älvstranden innefattade allt ifrån inbjudna samtal/debatter/seminarier till workshops kring Södra Älvstrandens framtid till tipspromenader, picnics och olika former av performativa iscensättningar under bar himmel.

    • In this article We argue that Ernst Cassirer's philosophy of symbolic forms is an indispensible philosophical-anthropological companion to rhetoric. We propose that appropriating Cassirer's understanding of symbolic forms enables rhetoric to go beyond the dominant perspective of language oriented theory and fully commit to a widened -Understanding of rhetoric as the study of how social meaning is created, performed and transformed. To clearly bring out the thrust of our enlarged rhetorical-philosophical anthropological approach we have structured our argument partly as a contrastive critique of Thomas A. Discenna's recent (Rhetorica 32/3; 2014) attempt to include Cassirer in the rhetorical tradition through a reading of the 1929 debate in Davos between Cassirer and Martin Heidegger; partly through a presentation of the aspects of Cassirer's thought that we find most important for developing a rhetorical-philosophical-anthropology of social meaning.

  • The Epistemology of Rhetoric : Plato, Doxa and Post-Truth

    • This thesis aims to develop an epistemology of rhetoric in light of the apparent contemporary post-truth condition of society. Epistemology is hereby understood as concerned with principles for knowledge production within the academic discipline of rhetoric, as well as with an understanding of knowledge production in the public realm.The first part of the thesis investigates the opposition between opinion (doxa) and true knowledge (epistēmē), which has been said to be at the very heart of the birth of rhetoric itself. I show through readings of Plato's Gorgias, Phaedrus, Theaetetus, Meno and Republic that the contemporary rendering of this birth is, however, haunted by simplification and misrepresentation. Nevertheless, locating and scrutinising these cracks provides avenues toward a contemporary epistemology of rhetoric, and thus to an alternative to the traditional way of re-assessing rhetoric in the wake of Aristotle.Part 2 investigates fives routes for a contemporary re-invention of doxa. Barthes represents the structuralist and poststructuralist route, Hariman rhetorical ontology, Amossy the pragmatic study of doxa, and Rosengren rhetorical-philosophical anthropology. These engagements draw inspiration from the general direction of Rosengren’s work, while striving for the analytical sharpness of Barthes and Amossy, as well as highlighting the importance of understanding of the function of metadiscourse. In part 3, I present ways to reconsider rhetorical theory and how rhetoricians tend to understand argumentation. Seven principles for rhetorical theory are presented, sketching an understanding of the human position that includes embodiment, takes all forms of symbolic influence into account, and considers the sociality and historicity of being, as well as the capacity for human agency. The thesis concludes by presenting a model for rhetorical argumentation which portrays rhetoric as a process of sedimentation and erosion, constituted from an acknowledgement of the reasonableness of everyday wisdom, the establishment of new myths, and the denaturalisation of prevalent beliefs, desires and identities. In its final section, I return to the ethical challenge at the heart of the post-truth narrative, arguing for the potential inherent in the heroic figure of a champion of autonomy.
