- Akuta psykiatriska sjukdomstillstånd har uppmärksammats allt mer på senare år. Inte minst har vi insett vikten av att ställa rätt diagnos och att sätta in adekvat behandling tidigt. Vår ambition är att förmedla dagens kunskapsläge när det gäller omhändertagande av patienter med akuta psykiatriska sjukdomstillstånd. Akutpsykiatri kan därmed utgöra ett värdefullt komplement till lokala riktlinjer och behandlingstraditioner. Boken behandlar ingående de praktiska frågor som kan uppstå i den akuta situationen, alltifrån patientbemötande till konkreta behandlingsförslag. Områden som katastrofpsykiatri och mångkulturell psykiatri belyses utförligt. Akutpsykiatri är i första hand avsedd att användas av jourläkare inom specialistpsykiatrin och primärvården men är också användbar för alla som i sin verksamhet möter akut psykiskt sjuka. Boken är framtagen i samarbete mellan företrädare för Sahlgrenska Universitetssjukhuset och Sahlgrenska Akademin, Göteborgs Universitet. Den förenar på så sätt en omfattande klinisk erfarenhet med senaste vetenskapliga rön inom akutpsykiatrin. Vår förhoppning är att kunna tillgodose det allt mer angelägna behovet av en gedigen och samtidigt lättanvänd akutpsykiatribok.

Incidence of Syndromes Associated With Frontotemporal Lobar Degeneration in 9 European Countries

- Diagnostic incidence data for syndromes associated with frontotemporal lobar degeneration (FTLD) in multinational studies are urgent in light of upcoming therapeutic approaches.ObjectiveTo assess the incidence of FTLD across Europe.Design, Setting, and ParticipantsThe Frontotemporal Dementia Incidence European Research Study (FRONTIERS) was a retrospective cohort study conducted from June 1, 2018, to May 31, 2019, using a population-based registry from 13 tertiary FTLD research clinics from the UK, the Netherlands, Finland, Sweden, Spain, Bulgaria, Serbia, Germany, and Italy and including all new FTLD-associated cases during the study period, with a combined catchment population of 11 023 643 person-years. Included patients fulfilled criteria for the behavioral variant of frontotemporal dementia (BVFTD), the nonfluent variant or semantic variant of primary progressive aphasia (PPA), unspecified PPA, progressive supranuclear palsy, corticobasal syndrome, or frontotemporal dementia with amyotrophic lateral sclerosis (FTD-ALS). Data were analyzed from July 19 to December 7, 2021.Main Outcomes and MeasuresRandom-intercept Poisson models were used to obtain estimates of the European FTLD incidence rate accounting for geographic heterogeneity.ResultsBased on 267 identified cases (mean [SD] patient age, 66.70 [9.02] years; 156 males [58.43%]), the estimated annual incidence rate for FTLD in Europe was 2.36 cases per 100 000 person-years (95% CI, 1.59-3.51 cases per 100 000 person-years). There was a progressive increase in FTLD incidence across age, reaching its peak at the age of 71 years, with 13.09 cases per 100 000 person-years (95% CI, 8.46-18.93 cases per 100 000 person-years) among men and 7.88 cases per 100 000 person-years (95% CI, 5.39-11.60 cases per 100 000 person-years) among women. Overall, the incidence was higher among men (2.84 cases per 100 000 person-years; 95% CI, 1.88-4.27 cases per 100 000 person-years) than among women (1.91 cases per 100 000 person-years; 95% CI, 1.26-2.91 cases per 100 000 person-years). BVFTD was the most common phenotype (107 cases [40.07%]), followed by PPA (76 [28.46%]) and extrapyramidal phenotypes (69 [25.84%]). FTD-ALS was the rarest phenotype (15 cases [5.62%]). A total of 95 patients with FTLD (35.58%) had a family history of dementia. The estimated number of new FTLD cases per year in Europe was 12 057.Conclusions and RelevanceThe findings suggest that FTLD-associated syndromes are more common than previously recognized, and diagnosis should be considered at any age. Improved knowledge of FTLD incidence may contribute to appropriate health and social care planning and in the design of future clinical trials.

- The assessment and management of asthma patients is challenging because of the complexity of the underlying inflammatory mechanisms and heterogeneity of their clinical presentation. Optimizing disease management requires therapy individualization that should rely on reliable biomarkers to unravel the phenotypes and endotypes of asthma. The secretory activity and turnover of eosinophils, as assessed by measuring eosinophil-derived proteins, may provide an accurate and complementary tool that mirrors the eosinophil activation status. Emerging evidence suggests that eosinophil-derived neurotoxin (EDN) has considerable potential as a precision medicine biomarker. In this review, we explore EDN suitability as biomarker in asthma management, with particular emphasis on its clinical significance in the management of both pediatric and adult populations.

- The implementation of evidence-based treatment methods for patients with severe mental illness must be deeply rooted in clinical case management and an ACT service delivery model, where the patient user can be involved in shared-decision making in the cycle of "assess-plan-act-follow up-feedback". In order to prepare and empower the client for the new role as a participating decision maker in the management of his/her own illness, various psychoeducational strategies are employed. The original family unit in the community of the Integrated Mental Health Care program (IC) was developed step-by-step through practice-based evidence and clinical expertise to include significant others as resource persons in a so called Resource Group, and therefore the program was subsequently named as "Resource group ACT" (RACT). The service delivery by community mental health teams involving the patient by way of resource groups as well as the psychoeducational treatment conditions involving both individual patients and family groups may contribute to the understanding of how RACT added clinical effectiveness in functioning and satisfaction. © 2015 Copyright © Taylor & Francis Group, LLC.
