  • Global, regional, and national burden of neurological disorders, 1990–2016 : a systematic analysis for the Global Burden of Disease Study 2016

    • Background: Neurological disorders are increasingly recognised as major causes of death and disability worldwide. The aim of this analysis from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2016 is to provide the most comprehensive and up-to-date estimates of the global, regional, and national burden from neurological disorders.Methods: We estimated prevalence, incidence, deaths, and disability-adjusted life-years (DALYs; the sum of years of life lost [YLLs] and years lived with disability [YLDs]) by age and sex for 15 neurological disorder categories (tetanus, meningitis, encephalitis, stroke, brain and other CNS cancers, traumatic brain injury, spinal cord injury, Alzheimer's disease and other dementias, Parkinson's disease, multiple sclerosis, motor neuron diseases, idiopathic epilepsy, migraine, tension-type headache, and a residual category for other less common neurological disorders) in 195 countries from 1990 to 2016. DisMod-MR 2.1, a Bayesian meta-regression tool, was the main method of estimation of prevalence and incidence, and the Cause of Death Ensemble model (CODEm) was used for mortality estimation. We quantified the contribution of 84 risks and combinations of risk to the disease estimates for the 15 neurological disorder categories using the GBD comparative risk assessment approach.Findings: Globally, in 2016, neurological disorders were the leading cause of DALYs (276 million [95% UI 247–308]) and second leading cause of deaths (9·0 million [8·8–9·4]). The absolute number of deaths and DALYs from all neurological disorders combined increased (deaths by 39% [34–44] and DALYs by 15% [9–21]) whereas their age-standardised rates decreased (deaths by 28% [26–30] and DALYs by 27% [24–31]) between 1990 and 2016. The only neurological disorders that had a decrease in rates and absolute numbers of deaths and DALYs were tetanus, meningitis, and encephalitis. The four largest contributors of neurological DALYs were stroke (42·2% [38·6–46·1]), migraine (16·3% [11·7–20·8]), Alzheimer's and other dementias (10·4% [9·0–12·1]), and meningitis (7·9% [6·6–10·4]). For the combined neurological disorders, age-standardised DALY rates were significantly higher in males than in females (male-to-female ratio 1·12 [1·05–1·20]), but migraine, multiple sclerosis, and tension-type headache were more common and caused more burden in females, with male-to-female ratios of less than 0·7. The 84 risks quantified in GBD explain less than 10% of neurological disorder DALY burdens, except stroke, for which 88·8% (86·5–90·9) of DALYs are attributable to risk factors, and to a lesser extent Alzheimer's disease and other dementias (22·3% [11·8–35·1] of DALYs are risk attributable) and idiopathic epilepsy (14·1% [10·8–17·5] of DALYs are risk attributable).Interpretation: Globally, the burden of neurological disorders, as measured by the absolute number of DALYs, continues to increase. As populations are growing and ageing, and the prevalence of major disabling neurological disorders steeply increases with age, governments will face increasing demand for treatment, rehabilitation, and support services for neurological disorders. The scarcity of established modifiable risks for most of the neurological burden demonstrates that new knowledge is required to develop effective prevention and treatment strategies.Funding: Bill & Melinda Gates Foundation.

    • A novel emulsion copolymer of vinyl acetate (VAc) and 1-hexene was synthesized at ambient pressure. The feeding technique, initiation system and reaction time of the copolymerization were optimized based on molecular characteristics such as the weight contribution of 1-hexene in the copolymer chains and glass transition temperature (T-g) as well as on bulk properties like minimum film-formation temperature (MFFT) and solid content. According to nuclear magnetic resonance spectroscopy and differential scanning calorimetry results, the combination of starve feeding and redox initiation, within a reaction time of 4h, effectively led to the copolymerization at ambient pressure between highly reactive polar VAc monomers and non-polar 1-hexene monomers of low reactivity. The copolymer showed a lower T-g and MFFT, and a reasonable solid content compared to the poly(vinyl acetate) (PVAc) homopolymer. The consumption rate, hydrolysis of acetate groups and chain transfer reactions during the polymerization were followed using infrared spectroscopy. Based on the results, the undesirable reactions between the VAc blocks were hindered by the neighbouring 1-hexene molecules. Tensile testing revealed an improvement in the toughness and elongation at break of VAc-1-hexene films compared to PVAc films.

    • Resource-efficient wood chipping for forest-industrial processes demands large fractions of accept chips and small fractions of small-sized material, such as pin chips and fines. In Kraft pulping, a narrow distribution of wood chip thickness is important for even impregnation and for making high-quality pulp. Using newly developed forest-industrial drum-chipping technology, the investigation covered wood of varying moisture content, frozen versus unfrozen wood, and the use of different wood species. Using conventional techniques for analyzing wood chip dimensions, fast-grown spruce wood with high moisture content gave 4.2 % pin chips and fines, which was less than half of the fractions obtained with spruce wood with lower moisture content. A comparison between frozen and unfrozen pine resulted in slightly thinner and shorter chips for the frozen wood, but in both cases accept yields of up to ∼85 % were achieved. A comparison of different tree species (aspen, birch, pine, and spruce) resulted in larger accept fractions (∼90 %) for the hardwood species, even though the average length of these wood chips was as low as 17 mm. The results provide a first indication of how basic wood log properties affect the yields of accept chips and small-sized material when using modern industrial drum-chipping technology.

  • Assessment of Recreational Value in a Protected Forest Area Considering the New Environmental Paradigm (Case Study: Helen Forest, Southwestern Iran)

    • This study investigates the recreational value of the Helen protected forest area, incorporating the new environmental paradigm into economic valuation. Visitors’ willingness to pay and its association with scores reflecting the new environmental paradigm were assessed through contingent valuation and a dual-dimensional questionnaire. Two models are employed: a base model with socio-economic variables and a model integrating new environmental paradigm. Results indicate that 83.04% of visitors are willing to pay for recreational use. The proposed amount, visit frequency, new environmental paradigm, age, gender, education, and income were found to significantly influence the acceptance of willingness to pay. Results indicated a strong positive correlation exists between new environmental paradigm and willingness to pay, highlighting the influence of environmental perspective. Average willingness to pay per household per visit is IRR 190,390.4 (USD 0.53), translating to an annual recreational value of IRR 22,629,264,215 (USD 64,088) for the entire Helen forest. The average new environmental paradigm scores for visitors stand at 57.36, with the statement “plants and animals have similar rights to humans” receiving the highest score of 28.4. These findings emphasize the importance of considering environmental perspectives in managing recreational sites. Integrating a new environmental paradigm into valuation methods can inform sustainable management strategies that balance economic development with environmental conservation and social well-being.

    • A convenient approach to employ an elliptic load-deformation elastoplastic approximation has been proposed and used to evaluate the mechanical behavior of pinewood biofuel pellets from lab-scale compressive load-deformation measurements. Verification of model predictions has been attempted against apparent finite element method based simulations. A database of essential mechanical properties has been established.   • Global Impact of the COVID-19 Pandemic on Stroke Volumes and Cerebrovascular Events: A 1-Year Follow-up.

    • Declines in stroke admission, IV thrombolysis (IVT), and mechanical thrombectomy volumes were reported during the first wave of the COVID-19 pandemic. There is a paucity of data on the longer-term effect of the pandemic on stroke volumes over the course of a year and through the second wave of the pandemic. We sought to measure the effect of the COVID-19 pandemic on the volumes of stroke admissions, intracranial hemorrhage (ICH), IVT, and mechanical thrombectomy over a 1-year period at the onset of the pandemic (March 1, 2020, to February 28, 2021) compared with the immediately preceding year (March 1, 2019, to February 29, 2020).We conducted a longitudinal retrospective study across 6 continents, 56 countries, and 275 stroke centers. We collected volume data for COVID-19 admissions and 4 stroke metrics: ischemic stroke admissions, ICH admissions, IVT treatments, and mechanical thrombectomy procedures. Diagnoses were identified by their ICD-10 codes or classifications in stroke databases.There were 148,895 stroke admissions in the 1 year immediately before compared with 138,453 admissions during the 1-year pandemic, representing a 7% decline (95% CI [95% CI 7.1-6.9]; p < 0.0001). ICH volumes declined from 29,585 to 28,156 (4.8% [5.1-4.6]; p < 0.0001) and IVT volume from 24,584 to 23,077 (6.1% [6.4-5.8]; p < 0.0001). Larger declines were observed at high-volume compared with low-volume centers (all p < 0.0001). There was no significant change in mechanical thrombectomy volumes (0.7% [0.6-0.9]; p = 0.49). Stroke was diagnosed in 1.3% [1.31-1.38] of 406,792 COVID-19 hospitalizations. SARS-CoV-2 infection was present in 2.9% ([2.82-2.97], 5,656/195,539) of all stroke hospitalizations.There was a global decline and shift to lower-volume centers of stroke admission volumes, ICH volumes, and IVT volumes during the 1st year of the COVID-19 pandemic compared with the prior year. Mechanical thrombectomy volumes were preserved. These results suggest preservation in the stroke care of higher severity of disease through the first pandemic year.This study is registered under NCT04934020.

    • To achieve net-zero carbon emissions by 2050, the UK government emphasizes the pivotal role of sustainable bioenergy in electricity, transportation, and heating. However, challenges persist in handling biomass particulate solids in production facilities, leading to economic impacts. This study investigates the flow characteristics of stemwood chips from four tree species using a novel drum chipper. Experimental analyses include bulk density measurements, silo discharge studies, biomass flow property assessments, and wall friction measurements. Comparative analyses are performed using Jenike's procedure for building wedge-shaped silos, with a focus on predicting the critical opening size to prevent arching. Additionally, the paper delves into the creation of statistical models aimed at identifying key factors influencing the flow behaviour during silo discharge. Emphasis is placed on understanding potential discrepancies between theoretical predictions and experimental results concerning critical silo openings for arch-free discharge. The results contribute to understanding the factors influencing the flow behaviour of wood chips, informing silo design considerations. Our findings suggest limitations in applying traditional silo design methods, urging further research for more accurate predictions.

    • Torque measurements were performed with the powder cell mounted on an Anton Parr rheometer using glass beads of two different sizes and different impeller geometries. A discrete element method simulator was used to perform simulations of the experiments to compare calculated torque values with those of the experimental torque values. Experimental torques obtained with flat two-blade impeller helped to find the correct sliding friction coefficient between beads. Experimental torques obtained with the circular impeller allowed to obtain the sliding friction coefficient between the impeller and beads. To estimate the wall friction coefficient, it was necessary to activate particle shear on the wall in configurations in which the gap between the impeller and wall is comparable with the bead size. It was verified that at the low shear rates used in this paper, the particle restitution coefficient is not significant and, therefore, higher impeller rotational velocities should be used to calibrate this parameter. Also, the differences between the torque values and time series of different impeller shapes were attributed to the different shearing surfaces and to the formation of instantaneous high magnitude force chains.

    • Storage, transfer and feeding of biomass particulate solids often cause problems at biobased production sites. Irregular flow and blockage in storage units create supply uncertainties and production stops with economically detrimental consequences. Despite the industrial awareness of these problems, reliable analysis methods for prediction of larger particle size biomass bulk solid flow properties are lacking.In this study, the silo discharge and arching behavior of different size fractions of wood chips from beech and pine forest residues were analyzed, utilizing a parallelepiped bin and a wedge-shaped hopper. The wood chip assortments were analyzed by Schulze shear testing, angle of repose, Hausner ratio, and bulk density. Obtained material data was utilized to build regression models versus the experimentally obtained critical hopper outlet size for material flow.The best fitted regression model for explaining the critical hopper outlet size was obtained with an equation with linear proportionality to the hopper half opening angle and to the square of the angle of repose. This regression model was well fitted to both assortments and over the whole particle size range, indicating robustness and that the proposed model can be useful to diagnose the silo discharge behavior for broad ranges of wood chip materials. The extension of the results of this paper to larger scale silos requires further investigation.
