    • We have in an earlier paper, [1], presented a generalization, the Linked Dipole Chain Model LDC, of the model developed by Ciafaloni-Catani-Fiorani-Marchesini, [2], to decribe the structure functions and the states in Deep Inelastic Scattering events. In this paper we would like to present an investigation of a set of general features of the LDC Model, viz the behaviour of the structure functions (we will show how the result interpolates in a precise way in between the well-known DGLAP and BFKL approaches), the average bremsstrahlung emission in the states and finally a partitioning of the structure functions into three different channels: the ''ordinary'' parton-probe events, where all bremsstrahlung emissions are below the probe Q(2), the boson-gluon fusion events where the last splitting of the par ton chains contain transverse momentum above Q(2), and the resolved Rutherford scattering events between the probe and the hadron (where the largest transverse momentum occur somewhere along the emission lines away from the ends).

  • Effect of photopheresis on lymphocyte population in children with newly diagnosed type 1 diabetes

    • In recent years photopheresis has been claimed to be an effective form of immunomodulation. It has also been shown to have an effect on the disease process at the onset of type 1 diabetes. In a double-blind, placebo-controlled randomized study, we analyzed if the effect of photopheresis in children with newly diagnosed diabetes is related to changes in the balance of lymhocyte populations. We also analyzed if lymphocyte subsets were related to recent infection, mild or aggressive disease manifestations, heredity, or gender. Nineteen children received active treatment with photopheresis, while 21 children received sham pheresis (placebo group). No influence of a history of previous infection, heredity, or certain clinical parameters on lymphocyte subsets was found. At the onset of type 1 diabetes, girls showed a higher proportion and a larger number of T cells (CD3+) and T-helper cells (CD4+) and a higher proportion of naïve CD4 +CD45RA+ cells. In the placebo group, an increase in the number of subsets with the activated phenotype in both the CD4 (CD29 +) and the CD8 (CD11a+) compartments was noted during the course of the study. These changes did not occur in the photopheresis group. No relation between lymphocyte subsets and clinical outcome was found 1 year after the treatment with photopheresis. In conclusion, we found no major effect of photopheresis on lymphocyte populations in a group of children with newly diagnosed type 1 diabetes. However, in the placebo group the proportions of activated CD4 and CD8 cells increased over time. Since these changes did not occur in the actively treated group, our findings suggest that photopheresis may have some suppressive effects.

  • Photopheresis at onset of type 1 diabetes : A randomised, double blind, placebo controlled trial

    • Background - In recent years photopheresis, an extracorporeal form of photochemotherapy using psoralen and ultraviolet A irradiation of leucocytes, has been claimed to be an effective form of immunomodulation. Aim - To evaluate its effect in type 1 diabetes we performed a double blind, controlled study using placebo tablets and sham pheresis in the control group. Methods - A total of 49 children, aged 10-18 years of age at diagnosis of type 1 diabetes were included, 40 fulfilled the study and were followed for three years (19 received active treatment with photopheresis and 21 placebo treatment). Results - The actively treated children secreted significantly more C peptide in urine during follow up than control children. C peptide values in serum showed corresponding differences between the two groups. The insulin dose/kg body weight needed to achieve satisfactory HbA1c values was always lower in the photopheresis group, there was no difference between the groups regarding HbAlc values during follow up. The treatment was well accepted except for nausea (n = 3) and urticaria (n = 1) in the actively treated group. There were no differences regarding weight or height, or episodes of infection between the two groups during follow up. Conclusion - Photopheresis does have an effect in addition to its possible placebo effect, shown as a weak but significant effect on the disease process at the onset of type 1 diabetes, an effect still noted after three years of follow up.

  • The molecular signature of MDS stem cells supports a stem-cell origin of 5q-myelodysplastic syndromes

    • Global gene expression profiling of highly purified 5q-deleted CD34+CD38–Thy1+ cells in 5q– myelodysplastic syndromes (MDSs) supported that they might originate from and outcompete normal CD34+CD38–Thy1+ hematopoietic stem cells. Few but distinct differences in gene expression distinguished MDS and normal stem cells. Expression of BMI1, encoding a critical regulator of self-renewal, was up-regulated in 5q– stem cells. Whereas multiple previous MDS genetic screens failed to identify altered expression of the gene encoding the myeloid transcription factor CEBPA, stage-specific and extensive down-regulation of CEBPA was specifically observed in MDS progenitors. These studies establish the importance of molecular characterization of distinct stages of cancer stem and progenitor cells to enhance the resolution of stage-specific dysregulated gene expression.

  • Mechanical non-surgical treatment of peri-implantitis : a double-blind randomized longitudinal clinical study. I: clinical results

    • Peri-implantitis is a frequent finding in patients with dental implants. The present study compared two non-surgical mechanical debridement methods of peri-implantitis. Material and Methods Thirty-seven subjects (mean age 61.5; S.D +/- 12.4), with one implant each, demonstrating peri-implantitis were randomized, and those treated either with titanium hand-instruments or with an ultrasonic device were enrolled. Data were obtained before treatment, and at 1, 3, and 6 months. Parametric and non-parametric statistics were used. Results Thirty-one subjects completed the study. The mean bone loss at implants in both groups was 1.5 mm (SD +/- 1.2 mm). No group differences for plaque or gingival indices were found at any time point. Baseline and 6-month mean probing pocket depths (PPD) at implants were 5.1 and 4.9 mm (p=0.30) in both groups. Plaque scores at treated implants decreased from 73% to 53% (p < 0.01). Bleeding scores also decreased (p < 0.01), with no group differences. No differences in the total bacterial counts were found over time. Higher total bacterial counts were found immediately after treatment (p < 0.01) and at 1 week for ultrasonic-treated implants (p < 0.05). Conclusions No group differences were found in the treatment outcomes. While plaque and bleeding scores improved, no effects on PPD were identified.

  • A population study of urinary incontinence and nocturia among women aged 20-59 years : prevalence, well-being and wish for treatment

    • BACKGROUND: The aim was to study urinary incontinence (UI) and nocturia in a female population; prevalence, effect on well-being, wish for treatment and result of treatment in primary health care.METHODS: A postal questionnaire was sent to all women aged 20-59 years who were scheduled for gynecological health examination by midwives in a primary health care district during one year. Questions concerning well-being were based on the Gothenburg QOL instrument. All women with incontinence were offered treatment by a midwife and a family doctor.RESULTS: Of the included 641 women, 491 (77%) answered the questionnaire. The prevalence of urinary incontinence was 27.7%, 3.5% having daily leakage. Nocturia occurred in 32 women (6.5%), 12 of whom were also incontinent. Self-assessed health, sleep, fitness and satisfaction with work situation decreased significantly with increased frequency of incontinence. Well-being was not correlated to type of incontinence. Nocturia correlated to poor health and sleep. About a quarter of the incontinent women started treatment when offered and 80% of those who completed the treatment program were subjectively improved. Wish for treatment was directly correlated to frequency of incontinence but not to type.CONCLUSIONS: Urinary incontinence and nocturia affect well-being in a negative way. Well-being and wish for treatment correlate to frequency of incontinence but not to type of incontinence. Most women with UI accept it, only about a quarter of incontinent women, or 6-7% of all women in the studied age group, want treatment. Treatment of female urinary incontinence in primary health care is successful.

  • Studie av bakgrundshalter av metaller hos svenska ungdomar

    • Försurande luftföroreningar leder till försurning av mark och vatten. Detta leder till mobilisering av vissa metaller/element (medan andra fixeras starkare i marken). Via dricksvatten och föda kan dessa förskjutningar påverka människans exponering. Ett bra sätt att studera detta är att mäta retentionen genom biologisk monitorering, d.v.s. bestämning av element i biologiskt prov, t ex blod. Upprepade mätningar över tid kan ge en god uppfattning om utvecklingen av miljö- och exponeringssituationen i ett givet område och användas för att utvärdera effekter av insatta åtgärder. För att kunna göra jämförelser och följa utvecklingen är det viktigt att ha tillgång till bakgrundshalter av miljöföroreningar, vilket är en av målsättningarna med detta projekt. Vänersborg-Trollhättan utgör ett område som starkt förorenats med både försurande luftföroreningar samt metaller. 1993/94 togs blodprov på sammanlagt 372 st 15-åringar i Trollhättan och Uppsala. 1995/96 togs nya prover på 292 av ungdomarna, som då var 17 år gamla. Även en rad andra förhållanden registrerades, bl a andra blodvariabler, som återspeglar järnstatus. Tretton element bestämdes i helblod och plasma med induktivt kopplad plasma masspektrometri. Genom intervju och enkät inhämtades uppgifter om bl a livsstilsfaktorer. Utifrån sju dagars kostregistrering beräknades intag av olika näringsämnen. En stor mängd data angående halter av olika element i helblod och serum hos svenska ungdomar har genererats. Särskilt för rubidium (Rb) och bly (Pb), men också för zink (Zn) och kvicksilver (Hg) var halterna högre i helblod än i plasma. För kadmium (Cd) var medianhalten högre i serum än i helblod. För kobolt (Co), koppar (Cu), selen (Se) och tallium (Tl) var koncentrationerna i helblod och serum ungefär lika höga. Det fanns signifikanta samband mellan halterna i helblod och plasma för Co, Cu, Zn, Se, Rb, volfram (W), Hg och Pb, men inte för rhodium (Rd), palladium (Pd), Cd, platina (Pt) eller Tl. 1993/94 fanns en rad skillnader i elementhalter mellan orterna: Halten av Pb (19%), W (75%) och Rb (9%) var högre i både helblod och plasma hos ungdomar i Trollhättan än i Uppsala. Även för Co, Se, Rb och Pd var koncentrationen högre i antingen helblod eller plasma. För inget av det 13 elementen hade Uppsala-ungdomarna högre halter. Mellan 1993/94 och 1995/96 (ungdomar studerade båda åren) visade halterna av Cu, Se och Cd ökande trender i både helblod och plasma, medan W, Hg och Pb sjönk. Det är inte klart hur stor del av detta som är en tidstrend, respektive återspeglar ökande ålder. En rad andra förhållanden påverkade elementhalterna: Flickorna hade högre halter i helblod av Co och Cu än pojkarna, som hade högre halt av Rb och Pb. Halten av Se i serum var associerat med intaget av selen. Koncentrationerna av Hg i både helblod och serum var relaterade till intaget av fisk, såväl totalt (rS=0,32; P=0,0001), som insjö- och annan fisk, serumhalten också med antalet amalgamfyllningar. Cd i helblod var starkt korrelerat till rökning och till dåligt järnstatus, återspeglat i serumhalten av ferritin (rS=-0,17; P=0,002). Det fanns ett stort antal signifikanta associationer mellan halter av olika element i blod, och särskilt i serum, mycket långt över det slumpen skulle kunna förklara. Det är oklart om detta sammanhänger med kopplad exponering eller metabolism, t ex bindningar till proteiner eller interaktion. Mängder av ytterligare bearbetningar återstår i detta omfattande datamaterial.

  • Low dose of hydroxyethyl starch impairs clot formation as assessed by viscoelastic devices

    • Objective. High doses of the synthetic colloid hydroxyethyl starch (HES) used for plasma expansion have been associated with impaired haemostasis and hypocoagulation. Less is known about effects on clot formation in the low haemodilutional range (less than 40%). This study evaluated the effects of low haemodilution with HES and albumin on coagulation using two different viscoelastic methods. Methods. Clot formation was studied in vitro in healthy donor blood after 10% and 30% haemodilution with 60 g/L HES 130/0.4 or 50 g/L albumin with free oscillation rheometry (FOR) and rotational thromboelastography. Results. Clotting time was not significantly affected at 10% haemodilution but was prolonged with both substances at 30% dilution (p less than 0.01-0.001). The effect was significantly more pronounced with HES than with albumin. The elasticity of the clot was slightly reduced at 10% dilution with albumin, more pronounced at 10% dilution with HES (p less than 0.05), further reduced at 30% dilution with albumin and to a still greater extent at 30% dilution with HES (p less than 0.05). With albumin the functional activity of fibrinogen was not reduced in excess of the dilutional effect. HES in contrast produced a further reduction in clot elasticity than caused by mere dilution at both 10% and 30% dilutions (p less than 0.001). Conclusions. There is an adverse effect on clot formation even at low grade haemodilution with both albumin and HES. The effect on coagulation is significantly more pronounced with HES than with albumin.
