    • Dementia is a source of growing concern globally, and often impacts on social and commu-nicative functioning. INdependent LIving Support Functions for the Elderly (IN LIFE) was aproject carried out within the European Commission Research and Innovation programmeHorizon 2020 that resulted in the development of two digital communication aids for rem-iniscence intervention for elderly people with dementia and their communication partners.The purpose of this intervention study was to investigate the effects on quality of life forpeople with dementia when using these aids. People with dementia (N = 118) and their for-mal care-givers (N = 187) and relatives (N = 9) were given the communication aids for a per-iod of 4–12 weeks. To assess a range of outcomes, questionnaires developed within theproject were used along with the EQ-5D (European Quality of Life – 5 Dimensions) andQoL-AD (Quality of Life in Alzheimer’s Disease) questionnaires. Quality of life improvedamong people with dementia when measured using EQ-5D ( p < 0.05). There was also a cor-relation between the impact on the participants’ health and wellbeing, the carers’ rating ofthe usefulness of the digital communication aids and the care-givers’ satisfaction with usingtechnology ( p < 0.05). These results indicate that digital communication aids may be usefulin social interaction where one partner has dementia.

    • Denna rapport har tagits fram som slutrapportering av projektet ”Sampers4 -fortsättning” finansierat av Trafikverket, som genomförts under hösten 2017 och våren 2018. Detta projekt har tagit vid där tidigare omskattningsprojekt för Sampers regionala efterfrågemodeller slutat. De största förändringarna och förbättringarna i denna senaste omskattning är:att även de som inte angivit någon inkomst i RVU har fått en inkomst påkodad,att hushållsvariabler kunnat inkluderas i skattningen på grund av framsteg inom framtagning av syntetisk befolkning med hushållsdata som kan användas vid implementering,att konsistensen mellan utbud och efterfrågan förbättrats genom att impedansen från kollektivtrafikutläggningen använts i skattning av efterfrågemodeller för färdmedelsval.Projektet ”Sampers4 –fortsättning” har avgränsats till att gälla skattning av regionala efterfrågemodeller. Implementering av modellerna har påbörjats i tidigare omskattningsprojekt, men har inte ingått i detta projekt. Vi inser att mycket av vårt arbete i detta projekt inte kunnat göras utan det arbete som gjorts i tidigare omskattningsprojekt.

  • Risk of infections in patients with myeloproliferative neoplasms-a population-based cohort study of 8363 patients

    • Infections are a common complication in patients with many hematologic malignancies, however, whether patients with myeloproliferative neoplasms (MPN) also are at an increased risk of infections is largely unknown. To assess the risk of serious infections, we performed a large population-based matched cohort study in Sweden including 8 363 MPN patients and 32,405 controls using high-quality registers between the years 1992-2013 with follow-up until 2015. The hazard ratio (HR) of any infection was 2.0 (95% confidence interval 1.9-2.0), of bacterial infections 1.9 (1.8-2.0), and of viral infections 2.1 (1.9-2.3). One of the largest risk increases was that of sepsis, HR 2.6 (2.4-2.9). The HR of any infection was highest in primary myelofibrosis 3.7 (3.2-4.1), and significantly elevated in all MPN subtypes; 1.7 (1.6-1.8) in polycythemia vera and 1.7 (1.5-1.8) in essential thrombocythemia. There was no significant difference in risk of infections between untreated patients and patients treated with hydroxyurea or interferon-alpha during the years 2006-2013. These novel findings of an overall increased risk of infections in MPN patients, irrespective of common cytoreductive treatments, suggest the increased risk of infection is inherent to the MPN.

    • Considerable data relate major depressive disorder (MDD) with aberrant immune system functioning. Pro inflammatory cytokines facilitate metabolism of tryptophan along the kynurenine pathway (KP) putatively resulting in reduced neuroprotective and increased neurotoxic KP metabolites in MDD, in addition to modulating metabolic and immune function. This central nervous system hypothesis has, however, only been tested in the periphery. Here, we measured KP-metabolite levels in both plasma and cerebrospinal fluid (CSF) of depressed patients (n = 63/36 respectively) and healthy controls (n = 48/33). Further, we assessed the relation between KP abnormalities and brain-structure volumes, as well as body mass index (BMI), an index of metabolic disturbance associated with atypical depression. Plasma levels of picolinic acid (PIC), the kynurenic/quinolinic acid ratio (KYNA/QUIN), and PIC/QUIN were lower in MDD, but QUIN levels were increased. In the CSF, we found lower PIC in MDD. Confirming previous work, MDD patients had lower hippocampal, and amygdalar volumes. Hippocampal and amygdalar volumes were correlated positively with plasma KYNA/QUIN ratio in MDD patients. BMI was increased in the MDD group relative to the control group. Moreover, BMI was inversely correlated with plasma and CSF PIC and PIC/QUIN, and positively correlated with plasma QUIN levels in MDD. Our results partially confirm previous peripheral KP findings and extend them to the CSF in MDD. We present the novel finding that abnormalities in KP metabolites are related to metabolic disturbances in depression, but the relation between KP metabolites and depression-associated brain atrophy might not be as direct as previously hypothesized.

  • A Case of Pacemaker Endocarditis Caused by Aerococcus urinae

    • Background: Aerococcus urinae has lately been acknowledged as a cause of infective endocarditis (IE) especially in older males with underlying urinary tract disorders. In this population, cardiac implanted electronical devices (CIED) are not uncommon, but despite the capacity of A. urinae to form biofilm in vitro, no cases of aerococcal CIED infections have been reported to date.Case Presentation: An 84-year-old male with pacemaker was admitted with dysuria one month after a transurethral procedure for urinary bladder cancer. A. urinae was isolated from urine and blood. Transesophageal echocardiography (TEE) was without signs of vegetation on valves or pacing cables. The patient was treated with a twelve-day course of β-lactam antibiotics. Forty days after the initial admission, the patient was readmitted due to malaise, general pain of the joints, chills, and renewed blood cultures grew A. urinae. TEE demonstrated a 10 × 5 mm vegetation on either the tricuspid valve or one of the pacing cables. The pacemaker system was completely removed and demonstrated macroscopic signs of infection. A temporary system was implanted, and after 14 days of penicillin G treatment, a new system permanent system was implanted. Total treatment time was 40 days. Recovery was uneventful.Conclusion: This report demonstrates that A. urinae can cause CIED infection. In patients with A. urinae bacteremia and a CIED, this risk must be considered, especially if bacteremia reoccurs.

  • Unpacking inscriptions – A sociomateriality perspective on the design process of a milk package

    • Today’s trend of fast-paced innovation in the field of, for example, information technology results in an acclaim of quick changes. At the same time, product innovation is only one facet of market offerings as industry standard comprise multiple interests that stabilize object characteristics, something which complicates change. Such industry standards are not unique to advanced technological objects. This thesis illustrates how the design process of a milk package, an exemplary piece of a mundane engineered object, is dependent on inscriptions wherein various interests and expert knowledge shape how this milk package is engineered and used in a practical setting. This thesis starts out from an object perspective to explore the milk package’s design process, but also emphasizes the environment and the specific conditions through which it travels. Based on a sociomateriality perspective, the study emphasizes a relational ontology and identifies the social and material conditions that influence the design of an engineered object. Over its lifespan, the milk package is used in different contexts and for different purposes (e.g., as a container of food, an item in a logistic system, and waste product) which emphasize various actors’ interests. On the basis of these mechanisms, the milk package is best described as an ‘engineered object multiple’ wherein what the thesis describes as inscription domains play a key role in determining the physical, aesthetic, and symbolic properties of the object. The concept of inscription domains enable detailed exploration of how an object is stabilized (or modified) by multiple interests, as advanced by various actors. Seen this way, new design processes unfold on the basis of previous inscriptions and become the result of compromises between different, and sometimes conflicting, interests. Based on this specific case on the engineering of the milk package, a number of theoretical contributions as well as managerial and policy implications are formulated, but so too are calls for more studies of how mundane engineered objects are constitutive of everyday life.
