Telemedicine : a complement to traditional referrals in oral medicine

- Introduction:Introducing telemedicine into clinical practice has not been without difficulties. Within the framework of the European Union project "Health Optimum," telemedicine consultations with specialists at the Department of Oral & Maxillofacial Surgery at Uppsala University Hospital (Uppsala, Sweden) have been offered to dentists in the public dental health service. The aim is to streamline the consultation process, improve/develop the skills of the participating dentists and dental hygienists, and save time and money for patients, healthcare authorities, and society.Subjects and Methods:Patient records are collected in a database for demonstration and discussion, and the system is also available for referrals. Both medical and dental photographs and x-rays are digitized in the same system. These can be viewed during telemedicine rounds and by the consultants at the hospital prior to a consultation. Secure, interactive conferencing software is used, which provides a quick, easy, and effective way to share video and data over the Internet. Both parties can demonstrate different parts of an image using a pointer or a drawing system. Conference phones are presently used for verbal communication.Results:Ten patients were discussed during telemedicine rounds (3 males and 7 females), all of whom would normally have been referred to a specialist. As a result of the telemedicine round, 2 were referred to a specialist, whereas diagnoses were made for the other 8, and treatment was suggested. The dental health clinic could thus provide treatment without the need for referral to a consultant.Conclusions:The telemedicine system described here allows patient care to be provided rapidly and more economically. Future plans include "live" rounds using a videocamera, providing the possibility to relay real-time information about the intraoral situation. A camera is being developed and should preferably be permanently installed chair side.

Akademisering av specialistsjuksköterskans utbildning i Sverige : Spänningsförhållanden med anledning av utbildningsreform i enlighet med Bologna

- Licentiatavhandlingens övergripande syfte var att studera konsekvenser av ökad akademisering och akademiskt lärande inom Svensk specialist-sjuksköterskeutbildning efter genomförande av ny utbildningsreform enligt Bologna.Bakgrund: Specialistsjuksköterskor förväntas i dagens hälso– och sjukvård att kunna bedriva en patientsäker och evidensbaserad vård, vilket kräver akademisk kompetens. Utbildningen till specialistsjuksköterska fördes i och med Bolognaprocessens införande i Sverige 2007 till avancerad utbildningsnivå. Både yrkesexamen och akademisk examen (magisterexamen) skulle inkluderas och integreras i samma utbildning. Detta medförde en förändring från en ämnesinriktad (samlingskodsinriktad) läroplan till en läroplan som syftar till att integrera ämnen (integrationskodsinriktad). Förutom ämnesintegrering skulle yrkesspecifika mål integreras med akademiska mål.Metod: Artikel I genomfördes i form av ett didaktiskt aktionsforskningsprojekt som pågick under 2½ år. En projektledningsgrupp (n=15-18) från fem lärosäten bildades. Denna grupp har aktivt deltagit i hela aktionsforskningsprocessen och vid projektmöten. Vid varje lärosäte utsågs en projektansvarig som ansvarade för den lokala aktionsforskningsprocessen. Dessa personer samlande in data i form av kursplaner, utbildningsplaner, studiehandledningar, minnesanteckningar från lärarmöten och möten med vårdverksamheter samt minnesanteckningar från forskargruppensmöten. Analys har gjorts med hjälp av konstant komparativ analysmetod. I artikel II användes konstruktivistisk Grounded Theory för att samla in och analysera data. Datainsamlingen påbörjades utifrån specialistsjuksköterskestudenters svar på en enkäts öppna fråga (n=120) i samband med utbildningens avslutning 2009 och 2010. Enkätsvaren reste frågor om specialistsjuksköterskans akademiska lärande. För att inhämta en djupare förståelse genomfördes intervjuer med specialistsjuksköterskor (n=12) som yrkesarbetat mellan 5 och 12 månader efter sin examen. Intervjuerna genomfördes från juni till oktober 2011. Avslutningsvis jämfördes svaren på den öppna enkätfrågan med analysen av intervjuerna.Resultat: Vid utbildningsreformens införande fanns pedagogiska motsättningar och didaktiska svårigheter som gav upphov till spänningsförhållanden mellan vårdutveckling, forskning, yrkesspecifika mål och akademiska mål. Spänningsförhållanden framkom också när specialistsjuksköterskorna inte erfor att deras akademiska kompetens tillvaratogs och värderades i vårdverksamheten. När stöd inte fanns för akademiskt lärande uppkom villrådighet och specialistsjuksköterskorna blev ambivalenta över om akademiskt lärande var meningsfullt. För att främja meningsfullt akademiskt lärande framhåller deltagarna betydelsen av samverkan och gemensamma arenor mellan lärosäten och vårdverksamhet där nyttoaspekten med akademiskt lärande tydliggörs. De spänningsförhållanden som framkommer var av både positiv och negativ karaktär.

- Aim: Epigenetic modifications, such as DNA methylation, are considered important in the regulation of target genes in cancer development. 5-Hydroxymethylcytosine (5hmC) was recently discovered to be related to the process of malignant transformation. The influence of DNA methylation in oral squamous cell carcinomas (OSCC) is not fully-understood. Therefore, the aim of the present study was to investigate the DNA methylation pattern in OSCC compared to healthy oral epithelium.

- Using unique survey data from the Russian industrial city Taganrog in 1989 and 1998, we analyse changes in the gender division of labour among gainfully employed women and men, pre- and post-transition. In Soviet Taganrog, dual earner families predominated, but nevertheless men were usually primary earners, while women did the bulk of housework. After transition, contrary to early predictions, aggregate female and male employment rates have declined to a similar extent but the time-use data indicate increased gender specialisation among the employed .Thus, the dual earner norm mainly remains but the pre-existing gender difference within it has increased considerably, particularly among couples with pre-school children

In this study, the effect of low- to moderate doses of vasopressin, a potent splanchnic vasoconstrictor, on changes in portal and hepatic venous pressures and splanchnic and hepato-splanchnic blood flows were assessed in elective liver resection surgery.

Haemodynamic management in liver surgery

- Background: Liver resection surgery is a potentially curative treatment for liver tumours. The liver is a highly vascular organ, and substantial intra-operative blood loss is common. Increased blood loss negatively impacts both postoperative outcome and long-term survival. A low central venous pressure (CVP) has been suggested in order to reduce blood loss during liver surgery. The rationale is that low CVP reflects lower pressure in the hepatic venous system, which in turn reduces the driving force causing bleeding when the liver tissues are transected. Together with fluid restriction, strategies to achieve a low CVP (LCVP) include patient tilt (head up or down), zero PEEP, nitroglycerine, diuretics and neuraxial anaesthesia. Vasopressin reduces portal pressure in patients with portal hypertension and has been shown to reduce blood loss in liver transplantation. LCVP management in liver surgery is associated with reduced blood loss and may increase the risk of organ of hypo-perfusion. Aims: To investigate the effect of patient position (tilt), nitroglycerine, PEEP and vasopressin on portal and hepatic venous pressures and systemic haemodynamics. To assess the effect of vasopressin on portal and hepato-splanchnic blood flows. To determine whether pressure changes in the superior vena cava are reflected in the hepatic venous system. To retrospectively evaluate the effects of a new anaesthetic management protocol involving low CVP and goal directed therapy (GDT/LCVP) in liver resection surgery. Methods: We used tip-manometer catheters to directly measure changes in hepatic venous and portal pressures during 10° tilt (head up and down), nitroglycerine infusion, and alterations in PEEP. The effect of low-to-moderate doses of vasopressin on hepatic venous and portal flow and pressure was assessed with conventional fluid-filled catheters in these vessels, collection of samples for blood gas analysis and the application of Fick’s principle. The effects on systemic haemodynamics were also assessed. Patient data were obtained and compared from two cohorts, before and after the introduction of GDT/LCVP. Results: Patient tilt led to substantial changes in CVP and mean arterial pressure (MAP), but only minor effects on hepatic pressures. Increased PEEP resulted in small increases in hepatic and central venous pressures. Nitroglycerine caused a parallel decrease in systemic and hepatic venous pressures. Cardiac output decreased. With the addition of head down tilt, MAP, cardiac output and CVP increased. Hepatic venous pressure increased marginally, but did not return to baseline. Vasopressin had no effect on hepatic pressures, but led to decreases in portal and hepato-splanchnic blood flow. After the introduction of LCVP/GDT management, median intra-operative haemorrhage decreased by almost a litre, with no increase in post-operative complications. Conclusions: Changes in CVP reflect changes in hepatic venous pressure in the supine position, but not during patient tilt. Tilting is not effective in reducing hepatic venous pressures. Nitroglycerine reduces the hepatic and portal venous pressures, but adverse central hemodynamic effects may limit its application. Vasopressin reduces portal and hepatic blood flow with only minor effect on pressures. Introducing goal-directed therapy with a low CVP protocol led to a large reduction in intra-operative blood loss compared to previous anaesthetic management techniques. Konstnärligt arbete (övrigt vetenskapligt/konstnärligt)abstract
- Göteborgs första högre utbildning för flickor, Flickläroverket bildades 1929 och 1935 placerades skolans byggnad på stadens mest prestigefyllda plats och i dess kulturella centrum vid Götaplatsen. 1992 integrerades Flickläroverkets byggnad varsamt och omsorgsfullt i Artisten, men ska nu rivas för att ge plats för Konstnärliga fakultetens nya byggnad där både Högskolan för scen och musik och HDK-Valand ska beredas plats. Flickläroverket fungerar som ett pilotprojekt som syftar till att samla in, överföra och skapa nya minnen av en plats med hjälp av konstnärliga metoder. Återskapande av en historisk Bågdans som uppfördes varje år vid lucia, nya koreografier och konstnärliga ritualer, utställningar med arkivmaterial, samtal med kvinnor som gick på Flickläroverket och med konstnärer och forskare gav substans åt tiden och nya minnen att föra med in i den framtida byggnaden. Pilotprojektet skapar en grund för ytterligare forskning och kommer att resultera i en film och en konst- och forskningspublikation 2023.
