    • I dag lever 80 procent av patienterna med cancer i barn- och ungdomsåren fem år efter diagnos.Ungefär 6 000–7 000 individer i Sverige är före detta barncancerpatienter.Sena komplikationer till sjukdom och behandling ses hos 60–70 procent av överlevarna.Extra utsatta är de med hjärntumör, de som strålbehandlats och vissa grupper som stamcellstransplanterats.Inte alla som behandlats för cancer i barn- och ungdomsåren drabbas av sena komplikationer.En kohortstudie bestående av alla i Norden som under 20 års ålder insjuknade i cancer (n ≈55 000) åren 1943–2008 har påbörjats 2010.Riktlinjer för uppföljning efter barncancer baserat på given behandling har tagits fram av Svenska arbetsgruppen för långtidsuppföljning efter barncancer (SALUB).

    • A new assay for HIV reverse transcriptase activity inhibiting antibodies (RTI-ab) was used for the analysis of a large collection of sera sampled before and after confirmation of HIV infection. In this assay HIV-RT was preincubated with diluted serum, after which residual RT activity was determined by a technique using a template coupled to macrobeads and 125I-lodo-deoxyuridine-triphosphate as the tracer-substrate. Of the 936 sera analysed, 818 were found positive for RTI-ab, and 824 were positive in Western blot (Wb). The prevalence of RTI-ab compared to Wb was therefore 99.3%. The corresponding figure for 930 sera analysed for envelope-ab, i.e., gp41-ab, was 823 positive, and of these 930 sera 815 were Wb positive, giving a comparative prevalence of 101%. In contrast, only 678 samples of 993 analyzed for core ab, i.e., p24, were positive, giving a prevalence of 77.0% as 880 of these samples were Wb positive. Thus, RTI-ab was as prevalent as gp41-ab, and although the analyses of RTI-ab amounts in different stages showed decreasing levels in stage IV compared to stages II or III, all of the sera except 1 were found positive in stages III and IV. Further, it was found that both the few RTI-ab negative samples in stage II and the few RTI-ab positive samples among Wb negative sera were sampled in connection with seroconversion. The specificity of the RTI-ab assay was 100% in a test of 200 serum samples from HIV negative blood donors. It was concluded that RTI-ab analyses can be made highly sensitive and specific and useful for studies of HIV infection.

    • This study presents and analyses search strings and user data for different Swedish lexicographical websites. The underlying empirical material was sourced from two relatively new websites, www.saob.se and the joint dic-tionary portal www.svenska.se, which collects and provides a single point of entry to three Swedish monolingual dictionaries financed by the Swedish Academy. Statistics are presented on the most common search strings, when and where the searches take place, and what devices and digital platforms that are commonly used while visiting the sites. In addition, the study addresses a number of questions and other forms of feedback received from dictionary users. Furthermore, the study provides suggestions and examplesof how the collected data can be utilised in upcoming updates and revisions of the dictionaries.

    • BackgroundA declining trend in mean cholesterol levels and smoking has been observed in high-income western countries during the last few decades, whereas obesity rates have increased. Simultaneously, mortality from coronary heart disease has decreased. The aim of the present study was to determine whether the trends in cardiovascular risk factors have continued in successive cohorts of middle-aged women over a period of 34years.MethodsSix population-based, cross-sectional samples of women (n=2294) mean age: 49.8years (range: 45-54), living in Gothenburg, Sweden, were investigated between 1980 and 2014.ResultsBody mass index (BMI) increased over time, with a mean BMI of 24.7kg/m(2) in 1980 to 25.7kg/m(2) in 2013-2014, corresponding to a weight gain of 4.5kg, together with an increase in the proportion of obese individuals (BMI >= 30kg/m(2)) from 10.4 to 16.6% (p=0.0012). The proportion of smokers and women with hypertension decreased from 34.5 to 12.8% (p=0.0006) and from 37.7 to 24.5% (p<0.0001) respectively. Mean total serum cholesterol levels decreased from 6.23 (SD 1.09) mmol/L in 1980 to 5.43 (SD 0.98) mmol/L in 2013-2014 (p<0.0001). Self-reported leisure time regular exercise increased from 7.8% in 1980 to 35.6% in 2013-2014 (p<0.0001). For women born in 1963, the prevalence ratio of not having any of five major cardiovascular risk factors was 1.82 (95% confidence interval (CI) 1.38-2.41), compared with women born in 1925-1934.ConclusionThe trend towards increasing obesity, more leisure-time physical activity and less smoking remains, while the decrease in serum cholesterol appears to have abated.

  • An association between Type 2 diabetes and alpha(1)-antitrypsin deficiency

    • Aims alpha(1)-Antitrypsin (AAT) is a serine protease inhibitor which recently has been shown to prevent Type 1 diabetes development, to prolong islet allograft survival and to inhibit pancreatic B-cell apoptosis in vivo. It has also been reported that Type 1 diabetic patients have significantly lower plasma concentrations of AAT, suggesting the potential role of AAT in the pathogenesis of Type 1 diabetes. We have investigated whether plasma AAT levels are altered in Type 2 diabetes. Methods The study included patients with Type 2 diabetes (n = 163) and non-diabetic control subjects matched for age, sex and smoking habits (n = 158) derived from the population-based Malmo Diet and Cancer study. Plasma samples were analysed for AAT concentration and phenotype and serum glucose, insulin, C-reactive protein and lipid levels were measured. Glycated haemoglobin was also measured. Results In the diabetic group, the women had higher mean plasma AAT levels than men ( P < 0.05). The mean plasma AAT levels did not differ between diabetic and control subjects. However, the number of individuals with low AAT levels (< 1.0 mg/ml) was 50% higher in the diabetic group (P < 0.05) and the frequency of AAT deficiency genotypes was 50% higher (NS) in diabetic compared with control subjects. In the group of diabetic patients with AAT < 1 mg/ml, AAT directly correlated with systolic blood pressure (P = 0.048) and inversely correlated with waist-hip ratio (P = 0.031). Conclusions Our results provide evidence that deficiency of AAT may be associated with an increased risk of developing Type 2 diabetes.

    • To deal with long-term sickness absence, inter-organizational and interprofessional projects for rehabilitation were established in Sweden in the late 1990s. This study explores the different views about the clients of such a project from the professionals of the organizations involved. Twenty-one key informants were interviewed in 1999 and 22 in 2002. The interviews were semi-structured and analysed with a phenomenographic approach. Four descriptive categories emerged: (A) the individual as consisting of components, (B) the individual in his/her context, (C) the individual as part of a group, and (D) the individual as part of the population. Category A included a paternalistic view and B included a maternalistic as well as an autonomous view. Both paternalism and maternalism imply an inequality in the relationship between the professional and the client. Category C had an organizational perspective, whereas category D had a community perspective. Both of these perspectives included an abstract view of the client. The conclusions were that the professionals had qualitatively different views on clients, which might lead to unequal encounters as well as differences in opportunities for rehabilitation. The results stress the importance of discussing what ethical values are desirable, not least in inter-organizational and interprofessional projects.
