  • Peri-Implant Health and the Knowing-Doing Gap-A Digital Survey on Procedures and Therapies

    • Objectives: Peri-implant tissue maintenance and treatment is becoming a serious challenge in implantology. With increasing numbers of implants being placed, more cases of peri-implant mucositis and peri-implantitis is seen. A digital survey on peri-implant disease management was issued to experts in periodontology and implantology to identify the tools and procedures most commonly used today to treat peri-implant diseases and successfully manage peri-implant health. The primary aim was to assess whether there is consensus in the choice of treatment to manage peri-implant diseases and to prevent their recurrence once treated. The secondary aim was to obtain insight into future protocols and /or devices, and the research and development needed.Materials and Methods: Participants in this digital survey were professionals specialising in periodontology, oral surgery, and implant dentistry. The questionnaire included both a series of closed- and open-ended questions. A total of 16 countries participated. The survey was sent by e-mail to 70 individuals, 66 received the survey and 37 of receivers responded, two of the participants were excluded due to insufficient filling of the survey. In the end 35 respondents completed the survey.Results: Respondents agree that the efficacy of mechanical and chemical decontamination of implant surfaces needs to be improved and better documented. It is a common opinion that the current remedies, mostly adapted from periodontal practises, do not provide effective and reliable clinical outcomes when treating peri-implant ailments. There is a general agreement amongst experts that regularly scheduled (3-6-month intervals) maintenance treatments are essential for maintaining peri-implant health in patients experiencing implant complications. Respondents are also concerned about unnecessary use of systemic antibiotics for managing peri-implant health.Conclusion: Regardless of agreements in parts, there was no observed consensus on the most effective treatment options for treating peri-implantitis. The experts all agree it is an urgent need for well-designed, long-term follow-up randomised and controlled clinical trials comparing interventions to provide an evidence-based strategy for peri-implant health management.

    • After a reperfused myocardial infarction (MI), dynamic tissue changes occur (edema, inflammation, microvascular obstruction, hemorrhage, cardiomyocyte necrosis, and ultimately replacement by fibrosis). The extension and magnitude of these changes contribute to long-term prognosis after MI. Cardiac magnetic resonance (CMR) is the gold-standard technique for noninvasive myocardial tissue characterization. CMR is also the preferred methodology for the identification of potential benefits associated with new cardioprotective strategies both in experimental and clinical trials. However, there is a wide heterogeneity in CMR methodologies used in experimental and clinical trials, including time of post-MI scan, acquisition protocols, and, more importantly, selection of endpoints. There is a need for standardization of these methodologies to improve the translation into a real clinical benefit. The main objective of this scientific expert panel consensus document is to provide recommendations for CMR endpoint selection in experimental and clinical trials based on pathophysiology and its association with hard outcomes.

  • The role of urban resilience in research and its contribution to sustainability

    • Urban resilience is an emergent concept that is receiving an increasing attention. Its definition is linked with the ability of an urban system to resist, maintain continuity and recover through all stresses while adapting and transforming towards sustainability. The aim of this study is two-fold. First, we analyse the research output on urban resilience using bibliometric techniques and Web of Science (WoS) in order to define the scope and identify topics in the urban resilience literature. CiteSpace software is used to establish the intellectual framework. Second, their contribution to sustainability dimensions and Sustainable Development Goals (SDGs) based on the research output is explored.For that purpose, we examined 1014 publications during the period 1998–2020. The findings reveal urban resilience literature has grown since the 2009.A specialization in recent years from more theoretical research to a more practical is identified. In terms of contribution towards sustainability, the findings show the number of publications that directly address sustainability is scarce and more balanced to environmental and social perspectives. These findings can provide a better understanding of the patterns and trends in the field and provides first evidence of the contribution of academic research on urban resilience to sustainability.

  • A Generative AI-based Personalized Guidance Tool for Enhancing the Feedback to MOOC Learners

    • The widespread adoption of Massive Open Online Courses (MOOCs) has profoundly influenced higher education by granting learners access to an extensive array of educational materials. However, the substantial volume of data generated by MOOCs presents a considerable challenge for instructors who aim to assess and facilitate effective learner support. In this study, we introduce an innovative GenAI-based (Generative Artificial Intelligence) tool designed to assist and guide MOOC learners in understanding their progress in the course to enhance their performance and prevent dropout. Our proposed approach takes advantage of GenAI’s capabilities to analyze and understand anonymized learner educational data, including aspects such as course progression, assignment results, time spent on different types of content, timestamps, and other pertinent information. By applying natural language processing techniques, GenAI identifies patterns and trends within the data, enabling it to provide personalized guidance to learners to help them develop better learning strategies and enhance their performance in the course. The proposed tool, named GePeTo (Generative AI-based Personalized Guidance Tool), not only streamlines the process of analyzing large volumes of educational data but also equips instructors with practical insights into their learners’ performance and difficulties. GePeTo offers a promising solution for higher education institutions aiming to leverage the potential of MOOC data for effective learner assessment and support. Automating the analysis of educational data and delivering personalized guidance to learners will also facilitate instructors in making data-driven decisions. Ultimately, this will improve learning outcomes and educational experiences for learners in the digital age of education.

    • The different components of the right heart pulmonary circulation unit can be investigated by MRI and computed tomography. MRI has clear advantages over echocardiography for accurate definition of right heart function and structure and to derive functional information regarding the pulmonary vasculature. Computed tomography is superior for the assessment of parenchymal and vascular pathologies of the lung with indications in the diagnostic work-up of pulmonary hypertension, but with more limited capability to evaluate right ventricular function and in deriving pulmonary hemodynamics. Recent technical developments with these imaging modalities could allow a better evaluation of the right heart pulmonary circulation unit.

    • In contrast to the upfront setting in which the role of high-dose therapy with autologous hematopoietic cell transplantation (HCT) as consolidation of a first remission in patients with multiple myeloma (MM) is well established, the role of high-dose therapy with autologous or allogeneic HCT has not been extensively studied in MM patients relapsing after primary therapy. The International Myeloma Working Group together with the Blood and Marrow Transplant Clinical Trials Network, the American Society of Blood and Marrow Transplantation, and the European Society of Blood and Marrow Transplantation convened a meeting of MM experts to: (1) summarize current knowledge regarding the role of autologous or allogeneic HCT in MM patients progressing after primary therapy, (2) propose guidelines for the use of salvage HCT in MM, (3) identify knowledge gaps, (4) propose a research agenda, and (5) develop a collaborative initiative to move the research agenda forward. After reviewing the available data, the expert committee came to the following consensus statement for salvage autologous HCT: (1) In transplantation-eligible patients relapsing after primary therapy that did NOT include an autologous HCT, high-dose therapy with HCT as part of salvage therapy should be considered standard; (2) High-dose therapy and autologous HCT should be considered appropriate therapy for any patients relapsing after primary therapy that includes an autologous HCT with initial remission duration of more than 18 months; (3) High-dose therapy and autologous HCT can be used as a bridging strategy to allogeneic HCT; (4) The role of postsalvage HCT maintenance needs to be explored in the context of well-designed prospective trials that should include new agents, such as monoclonal antibodies, immune-modulating agents, and oral proteasome inhibitors; (5) Autologous HCT consolidation should be explored as a strategy to develop novel conditioning regimens or post-HCT strategies in patients with short (less than 18 months remissions) after primary therapy; and (6) Prospective randomized trials need to be performed to define the role of salvage autologous HCT in patients with MM relapsing after primary therapy comparing it to "best non-HCT" therapy. The expert committee also underscored the importance of collecting enough hematopoietic stem cells to perform 2 transplantations early in the course of the disease. Regarding allogeneic HCT, the expert committee agreed on the following consensus statements: (1) Allogeneic HCT should be considered appropriate therapy for any eligible patient with early relapse (less than 24 months) after primary therapy that included an autologous HCT and/or high-risk features (ie, cytogenetics, extramedullary disease, plasma cell leukemia, or high lactate dehydrogenase); (2) Allogeneic HCT should be performed in the context of a clinical trial if possible; (3) The role of postallogeneic HCT maintenance therapy needs to be explored in the context of well-designed prospective trials; and (4) Prospective randomized trials need to be performed to define the role salvage allogeneic HCT in patients with MM relapsing after primary therapy.

  • From Condition Monitoring to Maintenance Management in Electric Power System Generation with focus on Wind Turbines

    • With increase in the number of sensors installed on sub-assemblies of industrial components, the amount of data collected is rapidly increasing. These data hold information in the areas of operation of the system and evolution of health condition of the components. Therefore, extracting the knowledge from the data can bring about significant improvements in the aforementioned areas.This dissertation provides a path for achieving such an objective. It starts by analyzing the data at the sub-assembly level of the components and creates four frameworks for analysis of operation and maintenance (O&M) for past, present and future horizons at the component level. These frameworks allow improvement in operation, maintenance planning, cost reduction, efficiency and performance of the industrial components. Next, the dissertation evaluates whether such models can be linked with system level analysis and how providing such a link could provide additional improvements for system operators. Finally, preventive maintenance (PM) in generation maintenance scheduling (GMS) in electric power systems is reviewed and updated with recent advancements such as connection to the electricity market and detailed implementation of health condition indicators into the maintenance models. In particular, maintenance scheduling through game theory in deregulated power system, for offshore wind farm (OWF) and an islanded microgrid (MG) are investigated.The results demonstrate improvements in reducing cost and increasing profit for the market agents and system operators as well as asset owners. Moreover, the models also deliver an insight on how direct integration of the collected operation data through the developed component level models can assist in improving the operation and management of maintenance for the system.
