Conducting In-Depth Interviews via Mobile Phone with Persons with Common Mental Disorders and Multimorbidity : The Challenges and Advantages as Experienced by Participants and Researchers

- Qualitative interviews are generally conducted in person. As the coronavirus pandemic (COVID-19) prevents in-person interviews, methodological studies which investigate the use of the telephone for persons with different illness experiences are needed. The aim was to explore experiences of the use of telephone during semi-structured research interviews, from the perspective of participants and researchers. Data were collected from mobile phone interviews with 32 individuals who had common mental disorders or multimorbidity which were analyzed thematically, as well as field notes reflecting researchers' experiences. The findings reveal several advantages of conducting interviews using mobile phones: flexibility, balanced anonymity and power relations, as well as a positive effect on self-disclosure and emotional display (leading to less emotional work and social responsibility). Challenges included the loss of human encounter, intense listening, and worries about technology, as well as sounds or disturbances in the environment. However, the positive aspects of not seeing each other were regarded as more important. In addition, we present some strategies before, during, and after conducting telephone interviews. Telephone interviews can be a valuable first option for data collection, allowing more individuals to be given a fair opportunity to share their experiences.

Knowledge as a catalyst for change : social work in a divergent landscape

- Knowledge has become a key word in social work and a central catalyst for change when it comes to social justice. Not least in a society that is increasingly divergent and in need of handling different crises. Through knowledge there is an ability for creating spaces and practices of emancipation that can tackle these crises. At the same time, knowledge is perceived and used in different ways within social work. An ongoing debate across the fields of social work, and a contemporary challenge, is how to tackle these different perspectives on knowledge. In this workshop we take this debate as our starting point to explore how we can continue developing the concept of knowledge within social work. We will introduce a discussion by presenting results from a study of perspectives on knowledge. 30 focus groups have been done with actors within and around the social services, from users via case workers, development leaders, managers to politicians and policymakers, and also with universities. By highlighting the variation of perspectives on knowledge needed in social work, we want to initiate a discussion of how we, theoretically and empirically, can create a more advanced understanding of the concept of knowledge in the divergent landscape of social work. Are other concepts of knowledge needed in social work to handle this divergent landscape? How are these concepts linked to seeing social work as a catalyst for change when it comes to social justice? Can these concepts be used for creating spaces and practices of emancipation?

- Purpose This study aims to explore ethical challenges potentially arising from a problem-solving intervention with workplace involvement (PSI-WPI) in primary health care (with first-line manager involvement) for employees on sickness absence due to common mental disorders. Methods A qualitative design guided by the theoretical framework for systematic identification of ethical aspects of healthcare technologies. Semi-structured interviews were performed with coordinators (n=6), employees (n=13), and first-line managers (n=8). Reflexive thematic analysis was used to analyse and interpret themes. Results A main theme was identified “the workplace and healthcare hold different organizational value logics” and four sub-themes: “the PSI-WPI challenged the organizational goals and values of the workplace and healthcare”, “the PSI-WPI challenged organizational values on fairness”, “the PSI-WPI challenged the professional roles of first-line managers and rehabilitation coordinators” and “the PSI-WPI introduced a need for the employee to juggle the employee and patient roles”. Conclusion Different organizational value logics, values, and goals can introduce ethical challenges. We advise clarifying stakeholders’ roles and preparing employees and managers for the return to work process by providing sufficient information. The ethical challenges and suggested measures to minimize them, should be considered when planning return to work interventions that involve several stakeholders.

- Background Common mental disorders count for a large percentage of sick leave cases in Europe and in Sweden. Problem-solving with workplace involvement have shown promising results in reducing the number of sick leave days for employees on sick leave for these conditions. Engaging the workplace by for example including the first-line manager in the return-to-work process changes the usual role of the primary care. Hence, this study aims to explore ethical issues that potentially arise when introducing workplace involvement as part of a problem-solving intervention. Methods A qualitative study in the Swedish Primary Care using data from semi-structured interviews with rehabilitation coordinators (n = 6), employees on sick leave for common mental disorders (n = 13), and their first-line managers (n = 8). A theoretical framework for systematic identification of ethical aspects of healthcare technologies was used to guide the interviews and reporting of results. Content analysis was used to code the data, searching for latent content. Ethical issues related to the ethical values privacy, identity, autonomy, professional values, third party, equality and justice were identified and described. The analysis was concluded by a normative discussion. Results Ethical issues were identified such as difficulties for the employees to control personal information. A need to create an integrated role of a patient and an employee and for coordinators to act neutral instead of as a patient advocate. Managers needed to balance the needs of the organization with the needs of the employee. A pre-requisite for participation was agreeing to manager involvement which may affect the equality of the intervention. Conclusions A conversation about sharing of information, roles, responsibilities and expectations during the rehabilitation should be initiated early and be continuous. Managers need support in learning the “how to” when having an employee on sick leave due to a common mental disorder. Key messages The problem-solving intervention imposed ethical issues in regard to control over personal information and role shifting, which can be handled through discussions and awareness. By analysing ethical aspects and norms and values connected to the intervention, adaptations and solutions can be discussed and handled before full scale implementation.

- Kapitlet handlar om hälso- och sjukvårdskuratorns arbete i Sverige - om vad kuratorn gör på jobbet och hur det kan förstås teoretiskt. Texten utgår empiriskt från mitt avhandlingsarbete, under vilket jag skuggade åtta hälso- och sjukvårdskuratorer, liksom samtalade med ytterligare åtta i en fokusgrupp. Deltagande kuratorer arbetade inom såväl somatisk - som psykiatrisk öppen- och slutenvård, exempelvis infektionssjukvård, geriatrik, psykossjukvård och allmän psykiatri, alla inom verksamheter riktade mot vuxna. Avsikten var att analysera det vardagliga arbetet genom att fokusera på kuratorns samspel med patienter och övrig personal, liksom kuratorns yrkesrelaterade självbild. Som bollplank i kapitlet används också andras forskning, liksom egna kliniska erfarenheter från nio års arbete som hälso- och sjukvårdskurator. Kapitlet inleds med en historisk och samtida kontextualisering av hälso- och sjukvårdskuratorns arbete i Sverige. Därefter diskuteras vad som kan förstås som arbetets funktioner: vilka betydelser arbetet fyller och/eller förväntas fylla för olika mottagare. Detta möjliggör en fördjupad förståelse för den rika variation av arbetsuppgifter som karaktäriserar arbetet, och kan medföra att det kan upplevas som svårt att beskriva och förstå. Dessa funktioner diskuteras sedan relaterat till hur de positionerar kuratorn i förhållande till patienter, anhöriga, annan personal och de sjukvårdande organisationerna. Slutligen relateras dessa positioner till ett processperspektiv på arbetet, där beskrivningen av arbetets funktioner och positioner relateras till vilka riktningar arbetet understödjer. Avsikten är att presentera ett teoretiskt ramverk som kan fördjupa analyser och reflektioner kring det konkreta arbetet, men också bidra till teoriutveckling i socialt arbete.

Universitetens funktion upplevs vara svår att paketera på ett konkurrenskraftigt sätt, i synnerhet när akademin blivit en av flera utbildningsaktörer. Vidare betonas relationen mellan den fria forskningen och praktiken, med vikten av att både vara nära och uppfattas som relevant. Samverkan lyfts som centralt, även om förutsättningarna beskrivs som utmanande, såväl relaterat till de egna organisationerna som till faktorer inom socialtjänstens verksamheter.

Det allmännas ensak: hälso- och sjuk-vårdskuratorers arbete med smittspårning av sexuellt överförbara infektioner

- Artikeln baseras på etnografiska studier av tre hälso- och sjukvårdskurato- rers arbete med smittspårning av sexuellt överförbara infektioner. Syftet är att analysera kuratorns patientnära arbete med smittspårning som en biopolitisk praktik, som bär på inneboende motstridigheter kopplade till lagstiftningens handlingsimperativ jämte patienters autonomi och integritet. Det empiriska materialet består av fältanteckningar, vilka analyseras ur ett diskuspsykolo- giskt perspektiv. Bland resultaten märks hur smittskyddsarbetets hälsofräm- jande ambitioner, uttryckta genom hälso- och sjukvårdslagen som en ”god sexuell och reproduktiv hälsa”, förutsätter konstruktionen av en kontrasteran- de ohälsosam och ”dålig” sexualitet, vilken också föranlett kontakten. Detta skapar paradoxala förutsättningar för kuratorernas arbete, då de är beroende av patienternas uppriktighet för att kunna följa smittskyddsarbetets intentioner. Kuratorerna balanserar denna paradox genom att betona den ”goda” sexu- alitetens möjligheter som potentiell framtida praktik, vilken patienterna görs ansvariga för att forma. The article is based on ethnographic studies of three health social workers and their contact tracing work regarding sexually transmitted infections. The aim is to analyze this work as biopolitical practice, involving intrinsic contradictories related to the imperatives of the law and patients ́ autonomy and integrity. The empirical material consists of field notes which are analyzed from a discursive psychological perspective. The results show how health promotion intentions, expressed in the health care act, as “a good sexual and reproductive health”, presupposes the construction of a contradictory unhealthy and “bad” sexuality, which has also caused the contact. This produces paradoxical prerequisites for the social workers, being dependent on the patients ́ sincerity to be able to carry out the intentions of the law. The health social workers handle this para- dox by emphasizing the “good” sexuality as a potential future practice, which patients are made responsible for shaping.
