  • Metal Exchangeability in the REE-Enriched Biogenic Mn Oxide Birnessite from Ytterby, Sweden

    • A black substance exuding from fractures was observed in 2012 in Ytterby mine, Sweden, and identified in 2017 as birnessite with the composition Mx[Mn(III,IV)](2)O-4 center dot(H2O)n. M is usually calcium and sodium, with x around 0.5. The Ytterby birnessite is unique, with M being calcium, magnesium, and also rare earth elements (REEs) constituting up to 2% of the total metal content. The biogenic origin of the birnessite was established in 2018. Analysis of the microbial processes leading to the birnessite formation and the REE enrichment has continued since then. The process is fast and dynamic, as indicated by the depletion of manganese and of REE and other metals in the fracture water during the passage over the precipitation zone in the mine tunnel. Studies of the exchangeability of metals in the structure are the main objective of the present program. Exposure to solutions of sodium, calcium, lanthanum, and iron led to exchanges and altered distribution of the metals in the birnessite, however, generating phases with almost identical structures after the exchanges, and no new mineral phases were detected. Exchangeability was more efficient for trivalent elements (REE) over divalent (calcium) and monovalent (sodium) elements of a similar size (ionic radii 90-100 pm).

  • On the formation and metal exchangeability of the rare earth element enriched birnessite from the Ytterby mine, Sweden

    • A porous black substance exuding from fractures in an underground tunnel leading to the shaft of the Ytterby mine, Sweden, was observed in 2012 and characterized as a secondary manganese (Mn) oxide in 2015. The oxide was identified in 2017 as a birnessite variety, Mx[Mn(III,IV)]2O4∙(H2O)n  where M usually is Ca, Na and x is around 0.5, but the Ytterby birnessite appears to be unique with M being Ca, Mg but also yttrium and rare earth elements (YREE), constituting up to 2% of the metal content. The biogenic origin of the Ytterby birnessite was established in 2018. Studies of the formation of this unique birnessite phase has progressed during 2018-19 with detailed studies of the hydrochemistry of the fracture water as well of the exchangeability of the metals M in the structure: Na, Ca, Fe and La representing the YREE. Exposure to solutions of  Na, Ca, Fe, and La, respectively (1 M) led to exchanges and altered distribution of the metals constituting M, with a preference of YREE (trivalent) over Ca (divalent) over Na (monovalent), all of similar ionic radii, as well as higher affinity for YREE over Fe(III), being smaller. Fe(III) did not replace Mn(III) in the structure, despite the fact that their radii are almost identical. No discrete new Fe phase was indicated, and the structure of the birnessite phase was almost identical after exchanges of M, as indicated from XRD. The formation of birnessite in the fracture opening on the tunnel wall appears to be a fast and dynamic process, as indicated by a significant depletion of Mn as well as of YREE in the fracture water during the passage over the precipitation zone, from top to bottom.

  • The Early Chain of Care in Patients with Bacteraemia with the Emphasis on the Prehospital Setting

    • There is a lack of knowledge about the early phase of severe infection. This report describes the early chain of care in bacteraemia as follows: (a) compare patients who were and were not transported by the Emergency Medical Services (EMS); (b) describe various aspects of the EMS chain; and (c) describe factors of importance for the delay to the start of intravenous antibiotics. It was hypothesized that, for patients with suspected sepsis judged by the EMS clinician, the delay until the onset of antibiotic treatment would be shorter.All patients in the Municipality of Gothenburg (Sweden) with a positive blood culture, when assessed at the Laboratory of Bacteriology in the Municipality of Gothenburg, from February 1 through April 30, 2012 took part in the survey.In all, 696 patients fulfilled the inclusion criteria. Their mean age was 76 years and 52% were men. Of all patients, 308 (44%) had been in contact with the EMS and/or the emergency department (ED). Of these 308 patients, 232 (75%) were transported by the EMS and 188 (61%) had “true pathogens” in blood cultures. Patients who were transported by the EMS were older, included more men, and suffered from more severe symptoms and signs.The EMS nurse suspected sepsis in only six percent of the cases. These patients had a delay from arrival at hospital until the start of antibiotics of one hour and 19 minutes versus three hours and 21 minutes among the remaining patients (P =.0006). The corresponding figures for cases with “true pathogens” were one hour and 19 minutes versus three hours and 15 minutes (P =.009).Among patients with bacteraemia, 75% used the EMS, and these patients were older, included more men, and suffered from more severe symptoms and signs. The EMS nurse suspected sepsis in six percent of cases. Regardless of whether or not patients with true pathogens were isolated, a suspicion of sepsis by the EMS clinician at the scene was associated with a shorter delay to the start of antibiotic treatment.

  • Prehospital analgesia using nasal administration of S-ketamine : a case series

    • Pain is a problem that often has to be addressed in the prehospital setting. The delivery of analgesia may sometimes prove challenging due to problems establishing intravenous access or a harsh winter environment. To solve the problem of intravenous access, intranasal administration of drugs is used in some settings. In cases where vascular access was foreseen or proved hard to establish (one or two missed attempts) on the scene of the accident we use nasally administered S-Ketamine for prehospital analgesia. Here we describe the use of nasally administered S-Ketamine in 9 cases. The doses used were in the range of 0,45-1,25 mg/kg. 8 patients were treated in outdoor winter-conditions in Sweden. 1 patient was treated indoor. VAS-score decreased from a median of 10 (interquartile range 8-10) to 3 (interquartile range 2-4). Nasally administered S-Ketamine offers a possible last resource to be used in cases where establishing vascular access is difficult or impossible. Side-effects in these 9 cases were few and non serious. Nasally administered drugs offer a needleless approach that is advantageous for the patient as well as for health personnel in especially challenging selected cases. Nasal as opposed to intravenous analgesia may reduce the time spent on the scene of the accident and most likely reduces the need to expose the patient to the environment in especially challenging cases of prehospital analgesia. Nasal administration of S-ketamine is off label and as such we only use it as a last resource and propose that the effect and safety of the treatment should be further studied.

    • Målsättningen har varit att ta fram ny kunskap inom miljötillsynen och därigenom uppnå en effektivare miljötillsyn samt att få in nya vetenskapliga perspektiv på miljötillsyn.I rapporten studeras metoder för inspektioner och det kommunikativa samspelet mellan inspektören och företrädare för den verksamhet som inspekteras, hur den institutionella ramen för inspektionsprocessen fungerar samt visar på möjligheter att mäta effekterna av inspektioner och tillsyn.Naturvårdsverket kommer att ha resultatet som ett kunskapsunderlag i fortsatt arbete med tillsynsvägledning och utveckling av hur tillsyn och tillsynsvägledning kan följas upp och utvärderas.

    • OBJECTIVES: We have developed a method using transthoracic echocardiography in establishing optimal visualization of the aortic root, to reduce the amount of contrast medium used in each patient. BACKGROUND: During transcatheter aortic valve implantation, it is necessary to obtain an optimal fluoroscopic projection for deployment of the valve showing the aortic ostium with the three cusps aligned in the beam direction. This may require repeat aortic root angiograms at this stage of the procedure with a high amount of contrast medium with a risk of detrimental influence on renal function. METHODS: We studied the conventional way and an echo guided way to optimize visualisation of the aortic root. Echocardiography was used initially allowing easier alignment of the image intensifier with the transducer's direction. RESULTS: Contrast volumes, radiation/fluoroscopy exposure times, and postoperative creatinine levels were significantly less in patients having the echo-guided orientation of the optimal fluoroscopic angles compared with patients treated with the conventional approach. CONCLUSION: We present a user-friendly echo-guided method to facilitate fluoroscopy adjustment during transcatheter aortic valve implantation. In our series, the amounts of contrast medium and radiation have been significantly reduced, with a concomitant reduction in detrimental effects on renal function in the early postoperative phase.

  • Distinct metabolic hallmarks of WHO classified adult glioma subtypes

    • BACKGROUND: Gliomas are complex tumors with several genetic aberrations and diverse metabolic programs contributing to their aggressive phenotypes and poor prognoses. This study defines key metabolic features that can be used to differentiate between glioma subtypes, with potential for improved diagnostics and subtype targeted therapy.METHODS: Cross-platform global metabolomic profiling coupled with clinical, genetic, and pathological analysis of glioma tissue from 224 tumors - oligodendroglioma (n=31), astrocytoma (n=31) and glioblastoma (n=162) - were performed. Identified metabolic phenotypes were evaluated in accordance with the WHO classification, IDH-mutation, 1p/19q-codeletion, WHO-grading 2-4, and MGMT promoter methylation.RESULTS: Distinct metabolic phenotypes separate all six analyzed glioma subtypes. IDH-mutated subtypes, expressing 2-hydroxyglutaric acid, were clearly distinguished from IDH-wildtype subtypes. Considerable metabolic heterogeneity outside of the mutated IDH pathway were also evident, with key metabolites being high expression of glycerophosphates, inositols, monosaccharides and sugar alcohols and low levels of sphingosine and lysoglycerophospholipids in IDH-mutants. Among the IDH-mutated subtypes, we observed high levels of amino acids, especially glycine and 2-aminoadipic acid, in grade 4 glioma, and N-acetyl aspartic acid in low-grade astrocytoma and oligodendroglioma. Both IDH-wildtype and mutated oligodendroglioma and glioblastoma were characterized by high levels of acylcarnitines, likely driven by rapid cell growth and hypoxic features. We found elevated levels of 5-HIAA in gliosarcoma and a subtype of oligodendroglioma not yet defined as a specific entity, indicating a previously not described role for the serotonin pathway linked to glioma with bimorphic tissue.CONCLUSION: Key metabolic differences exist across adult glioma subtypes.

  • A Bond Wire Connection Implementation at mm-Wave Active Microstrip Antenna

    • A bond wire to microstrip line transition, featuring two impedance transformers, is presented for a mm-wave microstrip antenna and power amplifier (PA) integration using conventional bond wires. Simulated return loss bandwidth for the transition is 4 GHz with a minimum insertion loss of 1 dB. Measurement results for the designed active antenna are compared with measurements for a passive antenna and a standalone PA. The maximum gain is 29 dBi, compared with 16.7-dBi passive antenna maximum gain. The half-power gain bandwidth is 7%. The measured active antenna radiation pattern is in good agreement with simulations. The maximum deembedded PA output power is close to 9 dBm.

    • A microstrip antenna integration with a power amplifier is presented for the 60 GHz band. The antenna is a single layer 4×4 square patches array. The power amplifier is injection locked, designed in a 65 nm CMOS technology, with single-ended input and output signals. The integration is realized by means of bond wire connections. The antenna and the power amplifier are fabricated, characterized and measured separately. The integrated design is fabricated and measured, results are de-embedded to extract the power amplifier parameters and to compare with separate device measurements. At 56.5 GHz the integrated sample has about 12 dB gain increase with respect to the antenna sample without power amplifier. The presented design is targeting 60 GHz band wireless communication system applications.
