  • Attitudes to dental hygienists: evaluation of the Dental Hygienist Beliefs Survey in a Swedish population of patients and students.

    • The aim was to evaluate and test the psychometric properties of the Dental Hygienist Beliefs Survey (DHBS) in a Swedish sample of different patient groups and students. It was hypothesized that negative dental hygienist beliefs would discriminate between fearful and non-fearful study groups. The DHBS was distributed together with the revised Dental Beliefs Survey (DBS-R) and the Dental Anxiety Scale (DAS). The study sample included 394 subjects (130 students, 144 general dental patients, 90 periodontal patients and 30 patients on a waiting list for dental fear treatment). The results verified that the DHBS discriminates well between dentally fearful and non-fearful study groups. The DHBS had high internal consistency (Cronbach's alpha = 0.96-0.98) in all the groups. The correlation between the DHBS and the DBS-R was high (rho = 0.82, P < 0.001). Furthermore, the DHBS correlated significantly with the DAS, as well as with a low but significant correlation to age (more negative attitudes in younger age groups) and gender (more negative attitudes amongst women). Regression analysis showed that gender and the DHBS items: 23, 16 and 28, i.e. items related to feeling helpless, worries/fears not being taken seriously and fear about 'bad news' possibly preventing treatment, were the most important predictors of dental fear. The results suggest that the DHBS may be a valid and reliable scale to use in order to assess patient's specific attitudes to dental hygienists. However, the psychometric properties including test-retest analysis and the underlying factor structure of the DHBS need to be further explored.

    • The aim of this study was to evaluate and to test the psychometric properties of a Swedish version of the revised Dental Beliefs Survey (DBS-R) in different patient groups and in a non-clinical sample of students. It was hypothesized that negative dental beliefs, assessed using the DBS-R, would discriminate between fearful and non-fearful study groups. The questionnaire was distributed together with the Dental Anxiety Scale (DAS). The sample included 550 adults who responded to the questionnaires (206 students, 177 general dental patients, 105 periodontal patients and 62 patients at a waiting list for dental-fear treatment). The internal drop-out rate was low. The results confirmed that the DBS-R discriminates well between fearful patients and the other study groups. The DBS-R had a high internal consistency in all the study groups. Furthermore, the DBS-R correlated significantly with age (higher values in younger age groups) and the DAS. Regression analyses showed that the DBS-R subdimensions of 'communication' and 'control'/or 'trust', respectively, were significant predictors for dental fear. The results suggest that the DBS-R is a reliable and valid instrument for use in different Swedish patient- and non-clinical population groups in order to assess attitudes to dentists. However, the underlying factor structure of the DBS-R needs to be further explored and established.

  • A brief motivational interviewing as an adjunct to periodontal therapy-A potential tool to reduce relapse in oral hygiene behaviours. A three-year study

Objectives: To evaluate (ii) whether inclusion of a single motivational interviewing (MI) session, as an adjunct to periodontal therapy, might be beneficial for preventing relapse in oral hygiene behaviours among patients treated for chronic periodontitis and (ii) whether individual and clinical characteristics can be of predictive value for retention of sufficient oral hygiene behaviours. Material & methods: This 3-year follow-up of a previously reported randomized controlled trial (RCT) study of 6-month duration included 26 patients. Patients in the test group had received one MI session by a clinical psychologist before initiation of the periodontal treatment. Otherwise, all patients followed the same treatment protocol for conventional educational intervention and non-surgical periodontal therapy. Efficacy variables assessed for evaluation of the standard of self-performed periodontal infection control were marginal bleeding index (MBI; primary efficacy variable) and plaque score (PI). Results: The patterns of change in MBI and PI scores were similar for test and control groups over the observation period. At 3 years, both groups showed a desirable mean full-mouth MBI of 15%, a figure that was comparable to that at the short-term evaluation after active periodontal treatment. The post-treatment MBI was the only variable identified as a predictor of retained adequate oral hygiene behaviours. Conclusion: A single MI session as an adjunct to conventional periodontal therapy could not be proven to be of long-term beneficial additive effect with regard to prevention of relapse in oral hygiene behaviour. Desirable standard of self-performed infection control after active periodontal treatment predicted the retention of sufficient oral hygiene behaviour over time.

  • Attitudes and communicative factors related to oral health and periodontal treatment

    • The most important factor in the prevention and treatment of periodontal disease is the individual’s standard of daily self-performed oral hygiene. Consequently, a major task in periodontal treatment is to motivate the patient to efficient oral hygiene behaviour. Attitudes towards oral health issues, communicative factors and interpersonal relationships are suggested as important factors in this respect. The overall aim of this thesis was to study the significance of such factors in the prevention and treatment of periodontal disease. In Study I, attitudes towards oral health and experiences of periodontal treatment were explored through individual in-depth interviews with patients referred to a specialist clinic for periodontal treatment. In Study II, a partly new questionnaire, The Dental Hygienist Beliefs Survey (DHBS), was evaluated and tested among different patient groups and students. The questionnaire assesses patient confidence in the interaction with the dental hygienist. In Study III, dental hygienists views on communicative issues and interpersonal processes of importance in the prevention and treatment of periodontal disease were explored through individual in-depth interviews. The study sample consisted of dental hygienists working at general and specialist dental clinics. The constant comparative method for Grounded Theory was the qualitative method chosen for the data collection and analysis in Study I and III. Motivational Interviewing (MI) is a client-centred communicative method that can initiate beneficial behavioural change and improve the outcome when added to conventional treatment methods. Hence, Study IV was designed as a randomised controlled trial in order to evaluate the potential additive effect of a single session of MI on self-performed periodontal infection control. The study sample consisted of patients referred to a specialist clinic for periodontal treatment. The primary outcome variable was reduction in gingival bleeding. The results showed that patients in treatment for chronic periodontitis experienced feelings of vulnerability. The communication with the specialist team was of the utmost important to gain insight into and an understanding of the severity of the disease condition. This understanding and the knowledge gained about the ways to achieve oral health and prevent further disease progression increased the patients’ feeling of control of the situation (Study I). The DHBS was found to be a valid and reliable scale to assess patient-specific attitudes to dental hygienists. Moreover, negative dental hygienist beliefs were associated with dental anxiety (Study II). In-depth interviews with dental hygienists (DH) highlighted the importance of building a trustful relationship with the patient, feeling secure in one’s professional role as a DH and, last but not least, receiving support from colleagues and the clinical manager was essential in order to be successful in the prevention and treatment of periodontal diseases (Study III). A single freestanding MI session as a prelude to conventional educational intervention and non-surgical periodontal treatment had no significant additive effect on the individual’s standard of self-performed periodontal infection control in a short-term perspective (Study IV). In conclusion, the results emphasise that communicative factors and interpersonal processes are important issues in dental treatment in order to get the patient to understand the disease condition, acquire knowledge about ways to achieve oral health, prevent disease progression, decrease anxiety and increase the patient’s feelings of control of the oral health situation.

  • Patients' attitudes towards oral health and experiences of periodontal treatment: a qualitative interview study

    • Purpose: The aim of the present qualitative study was to analyse patients' attitudes towards oral health and experiences of periodontal treatment. Materials and Methods: Patients who were referred to a specialist clinic in periodontics were subjected to open-ended interviews before (Abrahamsson et al, 2008), as well as after, the phase of cause-related periodontal therapy. An interview guide was used with themes of interest. The time interval between the interviews was about 6 months. Because the study focused on psychosocial interactions, the qualitative method used for the analysis was Grounded Theory (Glaser and Strauss, 1967). Data collection and analysis were simultaneous processes that focus on psychosocial processes, existing problems and how these problems were dealt with. The interviews were tape-recorded and transcribed verbatim, and consequently analysed in a hierarchical coding process until saturation was met (n = 16). Results: In the analysis, a core category was identified: 'Understanding the seriousness of the disease condition'. This core concept was related to four additional categories and dimensions: 'The need to be treated respectfully', 'To gain insight', 'Frustration about the financial cost for treatment' and 'Feelings of control over the situation'. Taken together, the core category and its related subcategories described a psychosocial process that was related to periodontal treatment. Conclusions: The results elucidated the patients' vulnerability and the importance of patient communication in the treatment for chronic periodontitis in order for the patient to feel in control of the situation.
