  • Percutaneous tibial nerve stimulation - PTNS: an alternative treatment option for chronic therapy resistant anal fissure

    • BackgroundThe aim of the present study was to evaluate percutaneous tibial nerve stimulation (PTNS) for treatment resistant chronic anal fissure.MethodsConsecutive patients with chronic anal fissure were treated with neuromodulation via the posterior tibial nerve between October 2013 and January 2014. Patients had PTNS for 30 min on 10 consecutive days. All patients had failed conventional medical treatment. The visual analogue scale (VAS) score, St. Marks score, Wexner’s constipation score, Brief Pain Inventory (BPI-SF), bleeding and mucosal healing were evaluated before treatment, at termination, after 3 months, and then yearly for 3 years.ResultsTen patients (4 males and 6 females; mean age 49.8 years) were identified but only 9 were evaluated as one patient’s fissure healed before PTNS was started. At 3-year follow-up, fissures had remained completely healed in 5 out of 9 patients. All patients stopped bleeding and were almost completely pain-free at 3 years (VAS p = 0.010) and pain relief improved from 50% at completion to 90% at 3 years. The patients’ Wexner constipation scores improved significantly (p = 0.007).ConclusionsIn this small series, PTNS enhanced healing of chronic anal fissure and reduced pain and bleeding with an associated improvement in bowel function.

  • A Fatty Acid Based Bayesian Approach for Inferring Diet in Aquatic Consumers

    • We modified the stable isotope mixing model MixSIR to infer primary producer contributions to consumer diets based on their fatty acid composition. To parameterize the algorithm, we generated a 'consumer-resource library' of FA signatures of Daphnia fed different algal diets, using 34 feeding trials representing diverse phytoplankton lineages. This library corresponds to the resource or producer file in classic Bayesian mixing models such as MixSIR or SIAR. Because this library is based on the FA profiles of zooplankton consuming known diets, and not the FA profiles of algae directly, trophic modification of consumer lipids is directly accounted for. To test the model, we simulated hypothetical Daphnia comprised of 80% diatoms, 10% green algae, and 10% cryptophytes and compared the FA signatures of these known pseudo-mixtures to outputs generated by the mixing model. The algorithm inferred these simulated consumers were comprised of 82% (63-92%) [median (2.5th to 97.5th percentile credible interval)] diatoms, 11% (4-22%) green algae, and 6% (0-25%) cryptophytes. We used the same model with published phytoplankton stable isotope (SI) data for delta C-13 and delta N-15 to examine how a SI based approach resolved a similar scenario. With SI, the algorithm inferred that the simulated consumer assimilated 52% (4-91%) diatoms, 23% (1-78%) green algae, and 18% (1-73%) cyanobacteria. The accuracy and precision of SI based estimates was extremely sensitive to both resource and consumer uncertainty, as well as the trophic fractionation assumption. These results indicate that when using only two tracers with substantial uncertainty for the putative resources, as is often the case in this class of analyses, the underdetermined constraint in consumer-resource SI analyses may be intractable. The FA based approach alleviated the underdetermined constraint because many more FA biomarkers were utilized (n < 20), different primary producers (e.g., diatoms, green algae, and cryptophytes) have very characteristic FA compositions, and the FA profiles of many aquatic primary consumers are strongly influenced by their diets.

    • 1. The zooplankton is a key link in the transfer of energy from primary producers up through aquatic food webs. Previous efforts to quantify the importance of basal resources to aquatic consumers have used stable isotopes (SI) and simple ternary models, including only 'bulk' phytoplankton, bacteria or terrestrial particulate organic matter (t-POM). 2. We used a novel Bayesian mixing model based on fatty acids (FA) to quantify the dietary assimilation of seven basal resources, including five phytoplankton groups, pelagic bacteria and t-POM, to Cladocera in large boreal lakes in Finland. To account for trophic enrichment of FA from the diet to consumers, we parameterised the model with a resource library, from many feeding trials, consisting of Daphnia magna fed 22 diverse basal taxa. 3. The results of the feeding trials show that the distinctive FA profiles of algal groups are transferred to consumers. Moreover, the large number of FA variables (n = 22) used in the model avoids the limitations of underdetermined mixing problems, common to SI modelling, in cases when the number of resources outnumbers the tracer variables. 4. We show that cladocerans were generally supported by phytoplankton (86-94%), with little use of t-POM (1-9%) and bacteria (1-3%). Cladocerans used primarily high-quality phytoplankton (cryptophytes, diatoms and dinoflagellates) in both summer (51 +/- 22%) and autumn (79 +/- 12%), and the relative importance of medium-quality resources (cyanobacteria, chlorophytes and chrysophytes) declined from 37 +/- 23% in the summer to 8 +/- 2% in the autumn. 5. High-quality resources, rich in essential biochemical compounds, are critical in fuelling food webs in large lakes, even those with high concentrations of allochthonous organic matter.

    • Trophic polymorphism has found to be common in many taxa and is a suggested mechanism of ecological speciation. To characterize the trophic linkages of specific morphotypes of organisms as well as a time-integrated niche use, several methods are available. In this study, we present data of multiple techniques to investigate the trophic divergence of Eurasian perch (Perca fluviatilis) that displays well-studied trophic polymorphism associated with littoral and pelagic habitats in lakes. We combined bulk stable isotope and fatty acid analyses on the muscle tissue of perch from three different lakes in Sweden with analyses of stomach content. By comparing the three methods, we aimed at providing a broad and highly resolved picture on the trophic divergence in freshwater fish. The degree in morphological divergence varied between perch caught in the three different lakes. Generally, perch caught in the pelagic zone were more streamlined compared to the ones caught in the littoral zone that had a deeper body, as shown by geometric morphometrics. The three diet assessment methods revealed different levels of information. Data on stomach content showed some preferences for specific dietary items in littoral and pelagic perch, but general trophic specialization could not be concluded due to the small sample size. Analyses of delta C-13 and delta N-15, however, confirmed these results as a long-term pattern connected to specific habitat use in two of the three lakes. Fatty acid signatures of perch reflected partly those of the prey items of the specific habitats. Although the proportions of the essential fatty acid 22:6n-3 were lower in littoral resources, the proportions in littoral fish were similar to the ones caught in the pelagic zone. We concluded that although a fundamental contribution from littoral resources exists in littoral phenotypes, a minor reliance on pelagic prey items is obviously needed to provide essential compounds. Thus, by combining the methods to characterize direct resource use (i.e., stomach analyses) with others that utilize trophic biomarkers (i.e., analyses of stable isotopes and fatty acids), we were able to illustrate the degree of variation in trophic divergence of perch but also shed some light on potential trade-offs that are related to resource specialization in freshwater fish.

    • Due to altered biogeochemical processes related to climate change, highly colored dissolved organic carbon (DOC) from terrestrial sources will lead to a water brownification in many freshwater systems of the Northern Hemisphere. This will create deteriorated visual conditions that have been found to affect habitat-specific morphological variations in Eurasian perch (Perca fluviatilis) in a previous study. So far, potential drivers and ultimate causes of these findings have not been identified. We conducted a field study to investigate the connection between morphological divergence and polyunsaturated fatty acid (PUFA) composition of perch from six lakes across a gradient of DOC concentration. We expected a decrease in the prevalence of PUFAs, which are important for perch growth and divergence with increasing DOC concentrations, due to the restructuring effects of DOC on aquatic food webs. In general, rate of morphological divergence in perch decreased with increasing DOC concentrations. Proportions of specific PUFAs (22:6n-3, 18:3n-3, 20:5n-3, and 20:4n-6) identified to primarily contribute to overall differences between perch caught in clear and brown-water lakes tended to be connected to overall decline of morphological divergence. However, no overall significant relationship was found, indicating no severe limitation of essential fatty acids for perch inhabiting brown water lakes. We further broaden our approach by conducting a laboratory experiment on foraging efficiency of perch. Therefore, we induced pelagic and littoral phenotypes by differences in habitat-structure and feeding mode and recorded attack rate in a feeding experiment. Generally, fish were less efficient in foraging on littoral prey (Ephemeroptera) when visual conditions were degraded by brown water color. We concluded that browning water may have a strong effect on the forager's ability to find particular food resources, resulting in the reduced development of evolutionary traits, such as habitat-specific morphological divergence.
