  • Global burden of peripheral artery disease and its risk factors, 1990-2019 : a systematic analysis for the Global Burden of Disease Study 2019

    • Background: Peripheral artery disease is a growing public health problem. We aimed to estimate the global disease burden of peripheral artery disease, its risk factors, and temporospatial trends to inform policy and public measures.Methods: Data on peripheral artery disease were modelled using the Global Burden of Disease, Injuries, and Risk Factors Study (GBD) 2019 database. Prevalence, disability-adjusted life years (DALYs), and mortality estimates of peripheral artery disease were extracted from GBD 2019. Total DALYs and age-standardised DALY rate of peripheral artery disease attributed to modifiable risk factors were also assessed.Findings: In 2019, the number of people aged 40 years and older with peripheral artery disease was 113 million (95% uncertainty interval [UI] 99 center dot 2-128 center dot 4), with a global prevalence of 1 center dot 52% (95% UI 1 center dot 33-1 center dot 72), of which 42 center dot 6% was in countries with low to middle Socio-demographic Index (SDI). The global prevalence of peripheral artery disease was higher in older people, (14 center dot 91% [12 center dot 41-17 center dot 87] in those aged 80-84 years), and was generally higher in females than in males. Globally, the total number of DALYs attributable to modifiable risk factors in 2019 accounted for 69 center dot 4% (64 center dot 2-74 center dot 3) of total peripheral artery disease DALYs. The prevalence of peripheral artery disease was highest in countries with high SDI and lowest in countries with low SDI, whereas DALY and mortality rates showed U-shaped curves, with the highest burden in the high and low SDI quintiles.Interpretation: The total number of people with peripheral artery disease has increased globally from 1990 to 2019. Despite the lower prevalence of peripheral artery disease in males and low-income countries, these groups showed similar DALY rates to females and higher-income countries, highlighting disproportionate burden in these groups. Modifiable risk factors were responsible for around 70% of the global peripheral artery disease burden. Public measures could mitigate the burden of peripheral artery disease by modifying risk factors.

  • An Ephemeral Red Arc Appeared at 68 degrees MLat at a Pseudo Breakup During Geomagnetically Quiet Conditions

    • Various subauroral optical features have been studied by analyzing data collected during periods of geomagnetic disturbances. Most events have been typically found at geomagnetic latitudes of 45-60 degrees. In this study, however, we present a red arc event found at geomagnetic 68 degrees north (L approximate to 7.1) in the Scandinavian sector during a period of geomagnetically quiet conditions within a short intermission between two high-speed solar wind events. The red arc appeared to coincide with a pseudo breakup at geomagnetic 71-72 degrees N and a rapid equatorward expansion of the polar cap. However, the red arc disappeared in approximately 7 min. Simultaneous measurements with the Swarm A/C satellites indicated the appearance of the red arc at the ionospheric trough minimum and a conspicuous enhancement of the electron temperature, suggesting the generation of the arc by heat flux. Since there are meaningful differences in the red arc features from already-known subauroral optical features such as the stable auroral red (SAR) arc, we considered that the red arc is a new phenomenon. We suggest that the ephemeral red arc may represent the moment of SAR arc birth associated with substorm particle injection, which is generally masked by bright dynamic aurorae.

  • Energy Transfer Between Hot Protons and Electromagnetic Ion Cyclotron Waves in Compressional Pc5 Ultra-low Frequency Waves

    • The Magnetospheric Multiscale (MMS) spacecraft observed many enhancements of electromagnetic ion cyclotron (EMIC) waves in an event in the late afternoon outer magnetosphere. These enhancements occurred mainly in the troughs of magnetic field intensity associated with a compressional ultralow frequency (ULF) wave. The ULF wave had a period of similar to 2-5 min (Pc5 frequency range) and was almost static in the plasma rest frame. The magnetic and ion pressures were in antiphase. They are consistent with mirror-mode type structures. We apply the Wave-Particle Interaction Analyzer method, which can quantitatively investigate the energy transfer between hot anisotropic protons and EMIC waves, to burst-mode data obtained by the four MMS spacecraft. The energy transfer near the cyclotron resonance velocity was identified in the vicinity of the center of troughs of magnetic field intensity, which corresponds to the maxima of ion pressure in the compressional ULF wave. This result is consistent with the idea that the EMIC wave generation is modulated by ULF waves, and preferential locations for the cyclotron resonant energy transfer are the troughs of magnetic field intensity. In these troughs, relatively low resonance velocity due to the lower magnetic field intensity and the enhanced hot proton flux likely contribute to the enhanced energy transfer from hot protons to the EMIC waves by cyclotron resonance. Due to the compressional ULF wave, regions of the cyclotron resonant energy transfer can be narrow (only a few times of the gyroradii of hot resonant protons) in magnetic local time.

  • Drift-Bounce Resonance Between Pc5 Pulsations and Ions at Multiple Energies in the Nightside Magnetosphere : Arase and MMS Observations

    • A Pc5 wave is observed by the Exploration of energization and Radiation in Geospace Arase satellite in the inner magnetosphere (L similar to 5.4-6.1) near postmidnight (L-magnetic local time similar to 1.8-2.5 hr) during the storm recovery phase on 27 March 2017. Its azimuthal wave number (m-number) is estimated using two independent methods with satellites and ground observations to be -8 to -15. The direct measurement of the m-number enables us to calculate the resonance energy. The flux oscillations of H+ and O+ ions at >= 56.3 keV are caused by drift resonance and those of O+ ions at <= 18.6 keV by bounce resonance. Resonances of O+ ions at multiple energies are simultaneously observed for the first time. The enhancement of the O+/H+ flux ratio at <= 18.6 keV indicates selective acceleration of O+ ions through bounce resonance. Plain Language Summary Geomagnetic pulsations are magnetic fluctuations excited by solar wind or plasma instabilities in the magnetosphere. Pc5 waves are continuous geomagnetic pulsations with a period of 150-600 s. A Pc5 wave was observed in the inner magnetosphere during a magnetic storm on 27 March 2017. It propagated westward with a wave number of 8 to 15 and resonated with charged particles, resulting in oscillations of the H+ and O+ ion fluxes at >= 56.3 keV and the O+ ion fluxes at <= 18.6 keV. Resonances of O+ ions at multiple energies are simultaneously observed for the first time. At the same time, the O+/H+ flux ratio at <= 18.6 keV enhanced corresponding to the O+ ion flux oscillations, which indicates selective acceleration of O+ ions through resonances.

  • High and Heterogeneous Prevalence of Asymptomatic and Sub-microscopic Malaria Infections on Islands in Lake Victoria, Kenya

    • Kenya is intensifying its national efforts in malaria control to achieve malaria elimination. Detailed characterization of malaria infection among populations living in the areas where the disease is endemic in Kenya is a crucial priority, especially for planning and evaluating future malaria elimination strategy. This study aimed to investigate the distribution and extent of malaria infection on islands in Lake Victoria of Kenya to aid in designing new interventions for malaria elimination. Five cross-sectional surveys were conducted between January 2012 and August 2014 on four islands (Mfangano, Takawiri, Kibuogi and Ngodhe) in Lake Victoria and a coastal mainland (Ungoye). Malaria prevalence varied significantly among settings: highest in Ungoye, followed by the large island of Mfangano and lowest in the three remaining small islands. Of the 3867 malaria infections detected by PCR, 91.8% were asymptomatic, 50.3% were sub-microscopic, of which 94% were also asymptomatic. We observed geographical differences and age dependency in both proportion of sub-microscopic infections and asymptomatic parasite carriage. Our findings highlighted the local heterogeneity in malaria prevalence on islands and a coastal area in Lake Victoria, and provided support for the inclusion of mass drug administration as a component of the intervention package to eliminate malaria on islands.

  • External validation of a deep learning algorithm for automated echocardiographic strain measurements

    • Aims Echocardiographic strain imaging reflects myocardial deformation and is a sensitive measure of cardiac function and wall-motion abnormalities. Deep learning (DL) algorithms could automate the interpretation of echocardiographic strain imaging.Methods and results We developed and trained an automated DL-based algorithm for left ventricular (LV) strain measurements in an internal dataset. Global longitudinal strain (GLS) was validated externally in (i) a real-world Taiwanese cohort of participants with and without heart failure (HF), (ii) a core-lab measured dataset from the multinational prevalence of microvascular dysfunction-HF and preserved ejection fraction (PROMIS-HFpEF) study, and regional strain in (iii) the HMC-QU-MI study of patients with suspected myocardial infarction. Outcomes included measures of agreement [bias, mean absolute difference (MAD), root-mean-squared-error (RMSE), and Pearson's correlation (R)] and area under the curve (AUC) to identify HF and regional wall-motion abnormalities. The DL workflow successfully analysed 3741 (89%) studies in the Taiwanese cohort, 176 (96%) in PROMIS-HFpEF, and 158 (98%) in HMC-QU-MI. Automated GLS showed good agreement with manual measurements (mean +/- SD): -18.9 +/- 4.5% vs. -18.2 +/- 4.4%, respectively, bias 0.68 +/- 2.52%, MAD 2.0 +/- 1.67, RMSE = 2.61, R = 0.84 in the Taiwanese cohort; and -15.4 +/- 4.1% vs. -15.9 +/- 3.6%, respectively, bias -0.65 +/- 2.71%, MAD 2.19 +/- 1.71, RMSE = 2.78, R = 0.76 in PROMIS-HFpEF. In the Taiwanese cohort, automated GLS accurately identified patients with HF (AUC = 0.89 for total HF and AUC = 0.98 for HF with reduced ejection fraction). In HMC-QU-MI, automated regional strain identified regional wall-motion abnormalities with an average AUC = 0.80.Conclusion DL algorithms can interpret echocardiographic strain images with similar accuracy as conventional measurements. These results highlight the potential of DL algorithms to democratize the use of cardiac strain measurements and reduce time-spent and costs for echo labs globally. Graphical Abstract
