  • Outcome of simple decompression of the compressed ulnar nerve at the elbow – influence of smoking, gender, and electrophysiological findings

    • Background: Compression of the ulnar nerve at elbow is frequently treated with simple decompression. Knowledge about factors influencing results of surgery of the nerve is limited and contradictory. The primary aim was to evaluate outcome of simple decompression of the nerve using a QuickDASH questionnaire, and to investigate any influence of smoking, gender, and preoperative electrophysiological findings. A second aim was to estimate the relation between QuickDASH score and a clinical assessment of outcome by the surgeon. Methods: Patients who were operated on with simple decompression of the ulnar nerve, excluding reoperations, from September 2009 to February 2011 were evaluated before and at 1 year after surgery using QuickDASH. Data were collected from medical records and from a self-reported health declaration. Results: There were no differences in QuickDASH scores or change in total score between smokers and non-smokers or between women and men. Nerve pathology, assessed by preoperative electrophysiology, did not affect outcome. The surgeon’s assessment of outcome mirrored QuickDASH score. Among all patients, 12/33 (36%) did not have a decrease in QuickDASH score >8, which is considered as a minimal clinically important difference. Conclusion: Smoking, gender, and preoperative electrophysiological findings do not affect outcome of surgery. There are a high number of patients who do not benefit from simple decompression of the ulnar nerve at the elbow. Patients who are planned for surgery should be informed that there is a risk for persistent problems. A simple outcome assessment by the surgeon mirrors QuickDASH score at 1 year.

  • Impact of smoking and preoperative electrophysiology on outcome after open carpal tunnel release

    • Background: The aim was to evaluate the influence of smoking and preoperative electrophysiology on the outcome of open carpal tunnel release. Methods: This retrospective observational study evaluated the outcome in 493 patients (531 hands) primary operated for carpal tunnel syndrome. Data were collected from medical records, health evaluations, and QuickDASH questionnaires before surgery and 1 year after. Results: Smokers had a higher QuickDASH score preoperatively as well as postoperatively, but the change in total score did not differ. The odds of having a postoperative QuickDASH score >10 were 2.5 times higher in smoking patients than in non-smoking patients. In 124/493 patients (25%), no clinically significant improvement was seen. Normal and extreme preoperative electrophysiology values were associated with higher postoperative scores. No correlation was found between preoperative QuickDASH scores and preoperative electrophysiology values. Conclusions: Smokers with carpal tunnel syndrome experience more symptoms preoperatively. Smokers have remaining symptoms after surgery. There is no correlation between preoperative QuickDASH scores and preoperative electrophysiology values. Patients with normal or near to normal preoperative electrophysiology results have limited improvement after surgery.

  • Monitoring of Anthropogenic Sediment Plumes in the Clarion-Clipperton Zone, NE Equatorial Pacific Ocean

    • The abyssal seafloor in the Clarion-Clipperton Zone (CCZ) in the NE Pacific hosts the largest abundance of polymetallic nodules in the deep sea and is being targeted as an area for potential deep-sea mining. During nodule mining, seafloor sediment will be brought into suspension by mining equipment, resulting in the formation of sediment plumes, which will affect benthic and pelagic life not naturally adapted to any major sediment transport and deposition events. To improve our understanding of sediment plume dispersion and to support the development of plume dispersion models in this specific deep-sea area, we conducted a small-scale, 12-hour disturbance experiment in the German exploration contract area in the CCZ using a chain dredge. Sediment plume dispersion and deposition was monitored using an array of optical and acoustic turbidity sensors and current meters placed on platforms on the seafloor, and by visual inspection of the seafloor before and after dredge deployment. We found that seafloor imagery could be used to qualitatively visualise the redeposited sediment up to a distance of 100 m from the source, and that sensors recording optical and acoustic backscatter are sensitive and adequate tools to monitor the horizontal and vertical dispersion of the generated sediment plume. Optical backscatter signals could be converted into absolute mass concentration of suspended sediment to provide quantitative data on sediment dispersion. Vertical profiles of acoustic backscatter recorded by current profilers provided qualitative insight into the vertical extent of the sediment plume. Our monitoring setup proved to be very useful for the monitoring of this small-scale experiment and can be seen as an exemplary strategy for monitoring studies of future, upscaled mining trials. We recommend that such larger trials include the use of AUVs for repeated seafloor imaging and water column plume mapping (optical and acoustical), as well as the use of in-situ particle size sensors and/or particle cameras to better constrain the effect of suspended particle aggregation on optical and acoustic backscatter signals.

  • Exposure-response relationships between movements and postures of the wrist and carpal tunnel syndrome among male and female house painters : a retrospective cohort study

    • OBJECTIVES: To investigate exposure-response relationships between measured movements and postures of the wrist and the incidence of carpal tunnel syndrome (CTS), and any modifications by sex.METHODS: In 2011, we established a historical cohort of 9364 members of the Painters' Union in Denmark. Self-reported task distributions were obtained by questionnaire (53% responded) and combined with sex-specific task exposure matrices to get individual estimates of exposure intensity, that is, velocity of wrist flexion/extension, mean power frequency (MPF) and non-neutral wrist postures. Exposure duration was assessed from yearly working proportions. Registered first-time hospital discharge CTS diagnoses and CTS surgery were collected as outcomes. The cohort was followed from 1994 to 2010. Log-linear Poisson regression was used.RESULTS: For CTS diagnoses, the adjusted incidence rate ratios (IRRs) increased with increasing wrist velocity (IRR=1.37 (95% CI 1.10 to 1.71) per °/s) and MPF (IRR=1.53 (95% CI 1.21 to 1.91) per 0.01 Hz). For CTS surgery, the results were similar. The outcomes were not related to non-neutral postures or exposure duration. The adjusted IRRs for women were higher than those for men. There were no multiplicative interaction effects between exposure intensity, exposure duration and sex. However, the absolute incidence rates (IRs) increased at a steeper rate for women than for men, indicating an additive interaction.CONCLUSIONS: The incidence of CTS increased with increasing velocity of wrist flexion/extension and MPF of wrist movements. The relative increase in incidence rates was the same for women and men, but the absolute incidence rates increased at a steeper rate for women than for men.

    • Objectives: We conducted a large cohort study to investigate the association between work-related wrist movements and carpal tunnel syndrome (CTS). Methods: Electro-goniometric measurements of wrist movements were performed for 30 jobs (eg, office work, child care, laundry work and slaughterhouse work). We measured wrist angular velocity, mean power frequency (MPF) and range of motion (ROM). We established a cohort of Danish citizens born 1940-1979 who held one of these jobs from age 18-80 years, using Danish national registers with annual employment information from 1992 to 2014. We updated the cohort by calendar year with job-specific and sex-specific means of measured exposures. Dates of a first diagnosis or operation because of CTS were retrieved from the Danish National Patient Register. The risk of CTS by quintiles of preceding exposure levels was assessed by adjusted incidence rate ratios (IRRadj) using Poisson regression models. Results: We found a clear exposure-response association between wrist angular velocity and CTS with an IRRadj of 2.31 (95% CI 2.09 to 2.56) when exposed to the highest level compared with the lowest. MPF also showed an exposure-response pattern, although less clear, with an IRRadj of 1.83 (1.68 to 1.98) for the highest compared with the lowest exposure level. ROM showed no clear pattern. Exposure-response patterns were different for men and women. Conclusions: High levels of wrist movement were associated with an increased risk of CTS. Preventive strategies should be aimed at jobs with high levels of wrist movements such as cleaning, laundry work and slaughterhouse work.

    • The present study aimed to estimate possible differences in upper body muscular load between male and female house painters performing identical work tasks. Sex-related differences in muscular load may help explain why women, in general, have more musculoskeletal complaints than men. In a laboratory setting, 16 male and 16 female house painters performed nine standardised work tasks common to house painters. Unilateral electromyography (EMG) recordings were obtained from the supraspinatus muscle by intramuscular electrodes and from the trapezius, extensor and flexor carpi radialis muscles by surface electrodes. Relative muscular loads in %EMG(max) as well as exerted force in Newton, based on ramp calibrations, were assessed. Sex differences were tested using a mixed model approach. Women worked at about 50 % higher relative muscular loads than men in the supraspinatus and forearm muscles at all percentiles and in all tasks. Women exerted about 30 % less force in the trapezius muscle at the 50th percentile. Female house painters had a higher relative muscular load than their male colleagues without exerting more force. The effects of a higher relative muscular load accumulated over years of work may in part explain why musculoskeletal complaints in the upper body occur more frequently among women than men.

  • Carpal tunnel syndrome in repetitive work: A follow-up study

    • Background The Project on Research and Intervention in Monotonous work (PRIM) studied Danish workers for 3-4years to determine the effects of monotonous work. The present study is a subset of that cohort and compares prevalence and incidence differences of carpal tunnel syndrome (CTS) between workers with highly repetitive work tasks and workers with varied work tasks. Methods The baseline study included 731 participants. Follow-up examinations were performed after 6 and 18 months. The CTS diagnosis was based on symptom interviews and nerve conduction tests. The repetitiveness levels were determined with electrogoniometers and observation of cycle times. Results The overall prevalence of CTS was 1.6% on the working hand and 0.7% on the other hand. There was a significantly increased risk of CTS for every 10-hr increase of repetitive non-forceful work (OR = 1.84; 95% CI = 1.06-3.19) on the working hand. This result, however, was based on few cases and a low prevalence in the control group. The overall annual CTS incidence was 0.62% on the working hand and 0.44% on the other hand. For the repetitive work tasks, the mean power frequencies ranged from 0.53 to 0.79 Hz. Conclusions In the baseline study, highly repetitive work was associated with CTS. The CTS incidence was too low to perform any analyses of exposure differences.
