    • This paper demonstrates that - under favorable conditions - by using multichannel recording and subsequent stacking of adjacent records marine airgun shots have been detected at offset distances up to 700 km, the maximum offset at which the authors attempted to record data.^Besides a powerful airgun array, a low noise environment at the recording site and the elimination of static shifts are the prerequisites to obtain refracted and reflected arrivals from the crust and upper mantle at such large offsets.^Primary arrivals detected at offsets between 400 and 700 km image the upper mantle from 70 to about 120 km depth.^Stacking of neighboring shots and/or receivers successfully increases the signal-to-noise ratio, if the traces have been corrected for offset differences, which requires knowledge of the apparent phase velocities.^The data presented here were collected in autumn 1989 during the BABEL Project on the Baltic Shield.

    • In the autumn of 1989 a co-operative experiment involving 12 research institutions in northwestern Europe collected 2268 km of deep seismic reflection profiles in the Gulf of Bothnia and the Baltic Sea. the 121 litre airgun array used for this profiling was also recorded by 62 muiticomponent land stations to provide coincident refraction surveys, fan-spreads, and 3-D seismic coverage of much of the Gulf of Bothnia. We thus have potentially both high-resolution impedance contrast images as well as more regional 3-D velocity models in both P- and S-waves. In the Bothnian Bay a south-dipping, non-reflective zone coincides with the conductive Archaean-Proterozoic boundary onshore in Finland. Between the Bothnian Bay and Bothnian Sea observed reflectivity geometries and velocity models at Moho depths suggest structures inherited from a 1.9Ga subduction zone; the upper crust here appears to have anomalously low velocity. Within the Bothnian Sea, reflectivity varies considerably beneath the metasedimentary/granitoid rocks of the Central Svecofennian Province (CSP) and the surrounding metavolcanic-arc rocks. Numerous dipping reflectors appear throughout the metavolcanic crust, whereas the CSP has little reflectivity. Wide-angle reflections indicate that the metasedimentary crust of the Bothnian Basin is 10 km thicker than the neighbouring Svecofennian subprovinces. Near the Åland archipelago Rapakivi granite plutons exhibit bright reflections, a contrast to the usual non-reflective plutons elsewhere in western Europe. Additional dipping reflections deep in the crust of this area may support models of rifting and crustal thinning during emplacement of the 1.70-1.54 Ga Rapakivi granites. Coeval gabbroic/anorthositic magmatism may explain the high reflectivity and high velocity of these plutons. the c. 1.25 Ga mafic sills and feeder dykes of the Central Scandinavian Dolerite Group also produce clear reflections on both near- and far-offset seismic sections. Continued modelling will produce better velocity models of the crust and better constrained contour maps of crustal thickness in this part of the Baltic shield.

  • Pain management after total hip arthroplasty at five different Danish hospitals : A prospective, observational cohort study of 501 patients

    • BACKGROUND: The available literature does not present a "gold standard" for post-operative pain treatment after total hip arthroplasty (THA). The aim of this prospective observational study was to explore and document post-operative pain treatment, including outcomes, in a large cohort of patients undergoing THA at five different Danish hospitals.METHODS: This prospective, multicentre, observational cohort study of 501 THA patients was performed at five different hospitals in the Capital Region and at the Region Zealand in Denmark, from April 2014 to April 2016. The study had two co-primary outcomes: Pain during mobilisation at 6 hours post-operatively (numeric rating scale [NRS] [0-10]) and morphine consumption 0-24 hours post-operatively.RESULTS: A large variety of analgesic treatments were used at the included hospitals and none of the hospitals used the same non-opioid basic analgesic regimen. For all patients at all hospitals, the NRS-pain level during mobilisation at 6 hours was 5 (3-6), (median [interquartile range]) and the 24-hour intravenous morphine (eqv) consumption was 25 mg (18-35). Although some statistically significant differences between hospitals were found for morphine use, no non-opioid analgesic regimen demonstrated consistent clinically relevant superior efficacy. In general, pain levels at rest were low to moderate and pain during mobilisation was moderate.CONCLUSIONS: Analgesic treatment routines differed between hospitals. Pain levels, however, did not differ substantially and were in general low at rest and moderate during mobilisation. No non-opioid analgesic treatment demonstrated consistent analgesic superiority.

    • Unexplained or idiopathic ketotic hypoglycemia (KH) is the most common type of hypoglycemia in children. The diagnosis is based on the exclusion of routine hormonal and metabolic causes of hypoglycemia. We aimed to identify novel genes that cause KH, as this may lead to a more targeted treatment. Deep phenotyping of ten preschool age at onset KH patients (boys, n = 5; girls, n = 5) was performed followed by trio exome sequencing and comprehensive bioinformatics analysis. Data analysis revealed four novel candidate genes: (1) NCOR1 in a patient with KH, iron deficiency and loose stools; (2) IGF2BP1 in a proband with KH, short stature and delayed bone age; (3) SLC5A2 in a proband with KH, intermittent glucosuria and extremely elevated p-GLP-1; and (4) NEK11 in a proband with ketotic hypoglycemia and liver affliction. These genes are associated with different metabolic processes, such as gluconeogenesis, translational regulation, and glucose transport. In conclusion, WES identified DNA variants in four different genes as potential novel causes of IKH, suggesting that IKH is a heterogeneous disorder that can be split into several novel diseases: NCOR1-KH, IGF2BP1-KH, SGLT2-KH or familial renal glucosuria KH, and NEK11-KH. Precision medicine treatment based on exome sequencing may lead to advances in the management of IKH.

  • Emplacement and 3D geometry of crustal-scale saucer-shaped intrusions in the Fennoscandian Shield

    • Saucer-shaped intrusions of tens of meters to tens of kilometres across have been observed both from surface geological mapping and geophysical observations. However, there is only one location where they have been reported to extend c. 100 km laterally, and emplaced both in a sedimentary basin and the crystalline basement down to 12 km depth. The legacy BABEL offshore seismic data, acquired over the central Fennoscandian Shield in 1989, have been recovered and reprocessed with the main goal of focusing on this series of globally unique crustal-scale saucer-shaped intrusions present onshore and offshore below the Bothnian Sea. The intrusions (c. 1.25 Ga), emplaced in an extensional setting, are observed within both sedimentary rocks (<1.5 Ga) and in the crystalline basement (>1.5 Ga). They have oval shapes with diameters ranging 30-100 km. The reprocessed seismic data provide evidence of up-doming of the lower crust (representing the melt reservoir) below the intrusions that, in turn, are observed at different depths in addition to a steep seismically transparent zone interpreted to be a discordant feeder dyke system. Relative age constraints and correlation with onshore saucer-shaped intrusions of different size suggest that they are internally connected and fed by each other from deeper to shallower levels. We argue for a nested emplacement mechanism and against a controlling role by the overlying sedimentary basin as the saucer-shaped intrusions are emplaced in both the sedimentary rocks as well as in the underlying crystalline basement. The interplay between magma pressure and overburden pressure, as well as the, at the time, ambient stress regime, are responsible for their extensive extent and rather constant thicknesses (c. 100-300 m). Saucer-shaped intrusions may therefore be present elsewhere in the crystalline basement to the same extent as observed in this study some of which are a significant source of raw materials.
