  • Assessing Radiographic Response to 223Ra with an Automated Bone Scan Index in Metastatic Castration-Resistant Prostate Cancer Patients

    • For effective clinical management of patients being treated with 223Ra, there is a need for radiographic response biomarkers to minimize disease progression and to stratify patients for subsequent treatment options. The objective of this study was to evaluate an automated bone scan index (aBSI) as a quantitative assessment of bone scans for radiographic response in patients with metastatic castration-resistant prostate cancer (mCRPC). Methods: In a multicenter retrospective study, bone scans from patients with mCRPC treated with monthly injections of 223Ra were collected from 7 hospitals in Sweden. Patients with available bone scans before treatment with 223Ra and at treatment discontinuation were eligible for the study. The aBSI was generated at baseline and at treatment discontinuation. The Spearman rank correlation was used to correlate aBSI with the baseline covariates: alkaline phosphatase (ALP) and prostate-specific antigen (PSA). The Cox proportional-hazards model and Kaplan-Meier curve were used to evaluate the association of covariates at baseline and their change at treatment discontinuation with overall survival (OS). The concordance index (C-index) was used to evaluate the discriminating strength of covariates in predicting OS. Results: Bone scan images at baseline were available from 156 patients, and 67 patients had both a baseline and a treatment discontinuation bone scan (median, 5 doses; interquartile range, 3-6 doses). Baseline aBSI (median, 4.5; interquartile range, 2.4-6.5) was moderately correlated with ALP (r = 0.60, P < 0.0001) and with PSA (r = 0.38, P = 0.003). Among baseline covariates, aBSI (P = 0.01) and ALP (P = 0.001) were significantly associated with OS, whereas PSA values were not (P = 0.059). After treatment discontinuation, 36% (24/67), 80% (54/67), and 13% (9/67) of patients demonstrated a decline in aBSI, ALP, and PSA, respectively. As a continuous variable, the relative change in aBSI after treatment, compared with baseline, was significantly associated with OS (P < 0.0001), with a C-index of 0.67. Median OS in patients with both aBSI and ALP decline (median, 134 wk) was significantly longer than in patients with ALP decline only (median, 77 wk; P = 0.029). Conclusion: Both aBSI at baseline and its change at treatment discontinuation were significant parameters associated with OS. The study warrants prospective validation of aBSI as a quantitative imaging response biomarker to predict OS in patients with mCRPC treated with 223Ra.

  • Continuous preventive bridge maintenance in Sweden - Field experiment on the effect of washing on concrete bridges

    • Bridges are an important part of the infrastructure. For the bridges to have the longest possible service life with minimum repairs, the maintenance is of great importance. One type of bridge maintenance that is rarely researched is the continuous preventive maintenance. The continuous preventive maintenance consists of removal of vegetation, cleaning of bridge joints and drainage systems as well as high-pressure washing of the structure. The effects of washing is heavily discussed but not properly researched. A study on the effectiveness of washing concrete is therefore being conducted. A field experiment has been initiated where concrete specimens are installed on an edge beam of a road bridge. The specimens are of two recipes where one represents an old bridge with rather high water-cement ratio and the other one represents a new bridge with a low water-cement ratio. 50% of the specimens are washed annually, while the others are not. Each year samples are collected and tested for a chloride profile. The results for the first year of exposure have been determined. They are promising but are still only very preliminary. The effect of washing, if any, will be visible after a longer exposure.

    • As part of a report [1] on continuous preventive bridge maintenance, a survey was conducted with Swedish municipalities. The survey’s focus was on bridges and preventive maintenance, as well as the demands and performance control stated by the municipalities. The preventive maintenance the survey focused on was the annual maintenance concerning washing, cleaning of drainage system and removal of vegetation. The survey showed both similarities and differences within the municipalities but also in comparison with Swedish Transport Agency (STA).

  • Effect of High Pressure Washing on Chloride Ingress in Concrete : Development of an accelerated test method

    • Bridges constitute an important part of the infrastructure. For the bridges to have the longest possible service life with minimum repairs, the maintenance is of great importance. One type of bridge maintenance that is rarely researched is the continuous preventive maintenance. The continuous preventive maintenance consists of removal of vegetation, cleaning of bridge joints and drainage systems as well as high-pressure washing of the structure. The effects of washing is discussed but not properly researched.  A study on the effectiveness of washing concrete is therefore being conducted. An accelerated method is being developed to mimic field testing. The method has been developed through tests on small concrete specimens subjected to fluctuating temperature, fluctuating moisture, and repeatable exposure to de-icing salt in cycles. The specimens are of two recipes where one represents an old bridge with rather high water-cement ratio (0.6) and the other one represents a new bridge with a low water-cement ratio (0.4). The first two versions of the method are described. The second version shows promising results, but the method needs further development to incorporate additional factors.

    • Syftet med detta projekt är att studera funktionen av elektroosmos som åtgärd mot fukt-problem i byggnader i Sverige samt att utifrån detta ge rekommendationer kring om och i så fall när det är lämpligt/olämpligt att använda metodiken under de varierande fuktighets-förhållanden som vanligtvis råder i Sverige.I projektet har vi gjort en litteraturstudie, en modellering av fysikaliska processer, studie-besök i byggnader med installationer för elektroosmos och mätningar av fukt i en källarvägg där en installation för elektroosmos blivit gjord.Vi ser metoden elektroosmos som en möjlighet för att hantera fuktproblem i grunder. Det behövs dock en del av utveckling både kring tekniken för metoden och kring vad som ska kunna förväntas av den.Det behövs bättre möjligheter att kunna förutsäga att metoden kommer att fungera i ett visst fall. Att inte kunna förutsäga detta bör vara ett bekymmer för dem som marknadsför metoden.Elektroosmos kräver höga fuktnivåer för att fungera. Torkning ned till under kritiska fuktnivåer kräver komplettering med andra torkmetoder. Det krävs en tydlighet kring hur detta ska lösas.Det behövs en bättre förståelse kring hur elektroderna ska placeras för bästa funktion.Fuktvandring i jorden kan ha stor betydelse för processen i praktiken. Det är idé att undersöka om detta kan användas för att utveckla metoden.Det behövs klara regler kring hur elinstallationen ska utformas på ett säkert sätt.Eftersom jontransporten i betongen är av ganska liten omfattning så tror vi inte att elektroosmos kommer att påverka, eller skada, betongens egenskaper på lång sikt.
